1.Similar Degree of Degeneration in the Articular and Bursal Layers of Delaminated Rotator Cuff Tear.

BACKGROUND: The purpose of the study was to compare the degree of degeneration of the articular and bursal layers of delaminated supraspinatus tendons based on histological examination. METHODS: Fifty-four patients with a full-thickness rotator cuff tear were included in the study. Tendon specimens were harvested during arthroscopic rotator cuff repair from the lateral torn edges of the articular and bursal layers of the delaminated tear. Harvested samples were stained with H&E dye and evaluated based on a semi-quantitative grading scale. RESULTS: There were no significant differences in the seven histological characteristics of tendon degeneration: fiber structure, fiber arrangement, round nuclei, regional variations in cellularity, vascularity, collagen stainability, and hyalinization between the articular and bursal layers of the delaminated rotator cuff tear (all p>0.05). Total degeneration scores of articular and bursal sides were 13.1 ± 3.85 points and 13.2 ± 3.42 points, respectively, and were not significantly different (p=0.958). CONCLUSIONS: The study demonstrates that tendon degeneration was similar in the articular and bursal sides of the delaminated fullthickness rotator cuff tear, suggesting that degeneration would be a main etiology for the rotator cuff tear not only in the articular side but also in the bursal side. Considering potential disadvantages of subacromial decompression, this study tentatively suggests routine use of subacromial decompression as well as the need for halting or recovery from rotator cuff degeneration for better rotator cuff repair.

2.The Results of Followed-up Study for 2 to 3 Years after Bronchiolitis by Respiratory Syncytial Virus(RSV).

PURPOSE: A large number of studies have demonstrated that bronchiolitis by RSV can develop into recurrent wheezing or asthma in infancy. The severity on first attack, atopic finding and evironmental factors can be a risk factors for this recurrent wheezing but not yet defined clearly. We studied to know the risk factors to cause recurrent wheezing after bronchiolitis by RSV. METHODS: Sixty three children who were hospitalized for bronchiolitis by RSV from June 1996 through May 1997 were followed-up. They were classified into recurrent group with wheezing of more than 3 times and no-recurrent group and compared. RESULTS: 1) The recurrent group was 46%(29 of 63 cases) and was higher in both male and whom first wheezing occured in less than 6 months of age. 2) On the first admission, the degree of respiratory distress was more severe in recurrent group. 3) On the first admission, serum ECP was more higher in the recurrent group. And during follow-up, frequency of increased serum IgE and positive skin test to D.p, D.f was higher in the recurrent group. 4) The household environment, personal history of patients and familial history of atopy did not affect on recurrence of wheezing. CONCLUSION: This study demonstrated that younger age of lesser than 6 month of age and severity of respiratory distress on the first admission and positive skin test to D.p, D.f and higher serum IgE during follow-up had significant association with recurrent wheezing.

3.Bilateral Extensive Syphilitic Punctate Inner Retinitis Triggering Macular Ischemia after Intravitreal Triamcinolone Injections

Purpose:
To report a case of extensive syphilitic punctate inner retinitis (SPIR) triggering bilateral macular ischemia after intravitreal triamcinolone injections, and the multimodal retinal imaging findings.Case summary: A 69-year-old male patient with nonproliferative diabetic retinopathy was transferred to our hospital because of bilateral visual deterioration (to counting fingers) after the first intravitreal triamcinolone injection. Fundus examination revealed numerous yellow punctate precipitates in the superficial retinae, retinal arteriolitis, and vitritis. The punctate lesions and surrounding retinal regions showed decreased vascular density on optical coherence tomography angiography, and focal hypofluorescence on fluorescein angiography. The patient was positive for all of the Venereal Disease Research Laboratory test, the fluorescent treponemal antibody-absorption test, and Treponema pallidum hemagglutination; we diagnosed bilateral SPIR. After treatment with aqueous crystalline penicillin solutions (24 million units per day for 14 days), the punctate lesions reduced but arteriolitis progressed to obliterative vasculitis. After 6 months, the bilateral SPIR and vitritis resolved, and the bilateral visual acuity improved to 20/100. However, inner retinal and macular ischemia persisted because of capillary nonperfusion attributable to obliterative vasculitis.
Conclusions
Extensive SPIR can develop after an initial intravitreal steroid injection; the inner retinal ischemia and visual loss may persist after treatment because obliterative vasculitis develops. Therefore, patients scheduled for intravitreal steroid injections should be screened for syphilis infection.
4.CT of head and neck lymphoma.

Lymphoma is the second most common neoplasm in the head and neck, and is the most common cause of unilateral neck mass in patients between 21 and 40 years of age. This report is a retrospective review of histologically proven lymphomas in 42 patients regarding histologic type, clinical stage, and CT imaging patterns. CT imaging plays an important role in making diagnosis, planning treatment, and evaluating recurrence after treatment. CT imaging patterns are classified into 4 types: Type 1 is nodal lymphoma, Type 2 extranodal lymphoma, Type 3 combined nodal and extranodal lymphoma, and Type 4 multifocal extranodal lymphoma. In conclusion, Lymphoma should be considered when multiple, nonnecrotic, homogenous lymph nodes are located in deep lymphatic chains (especially when they are large and bilateral or when both are the superficial and deep lymph node chains are involved simultaneously) and no mucosal abnormality of the aerodigestive tract is observed. Additionary, when a large nasopharyngeal mass lesion shows limited or equivocal bone destruction or a mass is identified on two sides of a nasal bone without frank destruction and when multiple sites of disease are identified in extranodal tissues.

Premenstrual syndrome (t=-2.67,p=.008) and fatigue (t=-2.46,p=.015) affected perceived productivity with an explanatory power of 23.6%. CONCLUSION: Based on these findings, the study highlighted the need for effective management programs to tackle fatigue and premenstrual syndrome among clinical nurses' major health issues in order to reduce their productivity loss.

7.Analysis of Women's Body Mass Index, Weight Perception and Obesity-related Quality of Life.

PURPOSE: Women's weight perception and obesity-related quality of life were analyzed according to BMI (Body Mass Index). METHODS: A survey was conducted using self-report questionnaires from 178 participants. The data analysis included descriptive statistics, frequency, percentage, t-test, ANOVA, Fisher's exact test and Kendall's tau using the SPSS version 14.0. RESULTS: There were significant differences in BMI according to age (F=8.037, p=<.001), weight perception (F=60.71, p<.001), weight control experience (F=2.504, p=.013), weight control method (F=5.839, p=.001) and weight control success (F=-2.451, p=.016). There was a significant difference in obesity-related quality of life according to weight perception in the low weight group (F=5.587, p=.021) and in the obesity group (t=3.419, p=.003). Higher correlations were found between the women's BMI group, weight perception and obesity-related quality of life. CONCLUSION: A program to provide appropriate information for weight perception is needed for the low weight group. There is a need for a program about weight reduction as well as weight perception for the obesity group.

8.Nobiletin Exhibits Neuroprotective Effects against Mitochondrial Complex I Inhibition via Regulating Apoptotic Signaling

Nobiletin, a polymethoxylated flavonoid found in citrus, has been studied because of its modulatory functions in cellular signaling cascades, and effects to prevent mitochondrial calcium overload and neuronal cell death. Particularly, we previously reported that nobiletin induced changes in the mitochondrial membrane potential through K + channel regulation, suggesting that nobiletin might exert neuroprotective effects via regulating mitochondrial functions associated with the electron transport chain (ETC) system. This study investigated whether nobiletin regulated mitochondrial dysfunction mediated by ETC system downregulation by inhibiting complex I (CI) and complex III (CIII) in pure mitochondria and the cortical neurons of rats. The results showed that nobiletin significantly reduced mitochondrial reactive oxygen species (ROS) production, inhibited apoptotic signaling, enhanced ATP production and then restored neuronal viability under conditions of CI inhibition, but not CIII inhibition.These effects were attributed to the downregulation of translocation of apoptosis-induced factor (AIF), and the upregulation of CI activity and the expression of antioxidant enzymes such as Nrf2 and HO-1. Together with our previous study, these results indicate that the neuroprotective effects of nobiletin under mitochondrial dysfunction may be associated with its function to activate antioxidant signaling cascades. Our findings suggest the possibility that nobiletin has therapeutic potential in treating oxidative neurological and neurodegenerative diseases mediated by mitochondrial dysfunction.
9.Nursing College Life Experiences of North Korean Defectors

PURPOSE: The purpose of this study is to understand nursing college life experiences of North Korean Defectors and identify their meanings. METHODS: The study collected data through individual in-depth interviews among six undergraduates or graduates from nursing colleges, using phenomenological research methodology of Colaizzi-one of qualitative research approaches. RESULTS: Six categories drawn as a result of research include ‘Be bumped against hard reality wall’, ‘Bondage of discrimination and prejudice’, ‘Endure and stand with strength of faith’, ‘Myself grown up along with work’, ‘Becoming one amid differences’, and ‘Stepping towards unification’. CONCLUSION: The result of this study would contribute to understanding academic and interpersonal difficulties North Korean defectors might experience at nursing colleges. And it may also help people to learn that they would play an important role in integration of the nursing fields of South and North Korea as well as the nation's unification. Along the way, the results of the study could be basic data to establish national policy helping North Korean defectors adapt to nursing college life, and develop the supporting system of colleges as well as setting up appropriate supports and measures from the perspective of the nursing field.
