Gefitinib (iressa),an epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor,has antitumor activity in non-small-cell lung cancer (NSCLC).Despite the dramatic response to this inhibitor,most patients nevertheless ultimately have relapses because of the secondary resistance.In human lung adenocar-cinomas with EGFR mutation,a second-site point mutation that substitutes methionine for threonine at position 790(T790M) is associated with approximately half of cases of secondary resistance to gefitinib.Other mecha-nisras that contribute to gefitinib resistance include EGFR receptor internalization and MET gene amplification.

5.Experimental study of preventive effect of recombinant streptokinase on cerebral vasospasm after subarachnoid hemorrhage

Objective To study the preventive effect of recombinant streptokinase (r-SK) on cerebral vasospasm (CVS) after subarachnoid hemorrhage(SAH).Methods Rabbit models of SAH were made by double blood injection into cisterna magna. A tube was inserted into lateral ventricle after 24 h. In group treating one, 1.25 mg r-SK were injected into lateral ventricle, and the tube was opened for drainage after closed 6 h, and then 6 h later, injection, closed pipe and drainage were recycled for another 2 times. In the group SAH , 1 ml normal saline (NS) was injected into lateral ventricle, and then the way was the same as group treating one. In group treating two, 3.75 mg r-SK were injected for once. 6 h, 12 h ,1 d, 3 d, 5 d and 7 d later, 1 ml CSF was drained from the lateral ventricle in groups SAH , treating one and treating two ,respectively. The level of OxyHb in CSF was detected, angiograms of basilar artery(BA) were performed before and after 7 d of SAH,calculation the rate of calibre and histological examinations were also performed. Results (1)The level of OxyHb in CSF of group SAH was increased gradually while decreased in groups treating one and treating two. 6.Laparoscopic Subtotal Cholecystectomy:Report of 168 Cases

Objective To investigate the feasibility and technique of laparoscopic subtotal cholecystectomy(LSC).Methods Totally 168 patients were converted to LSC because of failure in laparoscopic cholecystectomy(LC).During the LSC,the Calot's triangle was separated and then the Hartmann's pouch was incised to decreased the intracystic pressure for the removal of the stones.Results Among the cases,5 patients were converted to open surgery for subtotal resection of the gallbladder.LSC was completed after clipping the cystic duct and artery in 122 patients;in the other 41 cases,the gallbladder was cut at the Hartmann's pouch to clip the bile duct and artery or suture the neck of the gallbladder,and then LSC was performed.The median operation time was(65.5?15.2)min,and the intraoperative blood loss was(71.5?15.5)ml.The time to resume the diet was(20.4?6.3)h postoperation.After the operation,7 patients developed local complications(4.2%),and the mean postoperative hospital stay was(4.2?2.6)d.Of the patients,105 were followed up for(25.5?6.5)months,during this period,5 patients had dyspepsia,3 had right shoulder pain,and 9 had right hypochondrium pain.Conclusions LSC is feasible for patients with complicated cholecystitis.It is important to control the perioperative hemorrhage and bile leakage.
7.Expression of VEGF-C protein and its correlation with cervical lymphatic node metastasis in laryngeal squamous carcinoma

Objective To study the expression of vascular endothelial growth factor C (VEGF-C), which is known to be capable of inducing cell proliferation of lymphatic endothelium and development of lymphatic vessels, and to study its correlation with cervical lymph node metastasis in human laryngeal carcinomas. Methods The expression of VEGF-C protein in the cancer, its adjacent tissue, and normal tissue obtained from resected specimens of 46 cases of laryngeal cancer was assessed with an immunohistochemical staining assay followed by light microscopic examination, and its relationship with cervical lymph node metastasis was analyzed. Results Immunohistochemical staining of VEGF-C was different obviously in different localities of the resected. The expression of VEGF-C protein was observed in all the central portion, peripheral tissue, and normal tissue of the postoperative specimens. The positive rate of intratumoral VEGF-C was significantly higher than that of adjacent and normal tissue. It was also noted that the positive rate of intratumoral VEGF-C was well correlated with pathological grades, T stages and stages of lymph node metastasis. Conclusions The results suggested that VEGF-C may be a factor of lymphatic metastasis of laryngeal cancer, and it is possible to be used as a predictor of lmphatic metastasis of laryngeal cancer.
8.Closed retroperitoneoscopic decortication of 11 cases of renal cysts

Objective To evaluate the efficacy of closed retroperitoneoscopic decortication of renal cysts.Methods Closed retroperitoneoscopic decortication of renal cysts was performed in 11 cases from December 1996 to September 2003 in this hospital.A 1~2 cm skin incision 2 cm above the iliac crest was made in the middle axillary line.A 10 mm trocar was introduced into the retroperitoneal cavity.A self-made water-filled balloon was used to create a retroperitoneal space for clearing the renal cyst.An electrocautery scalpel was used to excise the cyst wall until 0.5~1.0 cm away from the renal parenchyma.Results The procedure was successfully completed in 10 cases,with an operation time of 30~100 min(mean,77.5 min) and an intraoperative hemorrhage volume of 20~60 ml(mean,30.9 ml).A conversion to open surgery was required in 1 case because of difficult exposure of the renal cyst.Peritoneal injury was encountered in 1 case.The length of postoperative hospital stay was 6~9 days(mean,7.4 days).(Follow-up) observations in 7 cases for 7~26 months(mean,15.2 months) showed no recurrence of renal cyst under ultrasonography.Conclusions Closed retroperitoneoscopic decortication of renal cysts is a surgical option with little invasion,quick recovery,and good efficacy.
9.Contrast Study of CT Image Quality in Two Different High-pressure Syringe Ways

Objective To discuss the consequences of CT image quality in two different contrast enhancement ways of high pressure syringe. Methods 100 cases by using high-pressure syringes of Lightspeed 16-raw spiral CT and Stellant D-type high-pressure syringe of Medrad, the dose of constrast medium was 4.0 ml/s and the total dose was 80 ml, which was divided into A and B group enhancement scanning. The canalis singularis high pressure syringe was used in 50 cases of A group that no isotonic Na chloride flush, in contrast to 50 samples of B group that were underwent the double isotonic Na chloride flush. The method of evaluation is double-blind observation. Results The number of positive is 40 samples which has radioactive constructed defect in A group. The masculine proportion is 80%. The number of the negative is 10 samples. The proportion is 20%.
