OBJECTIVES: On September 2, 2010 and November 25, 2011, the Korean Research Project on Integrated Exposure Assessment to Hazardous Materials for Food Safety (KRIEFS) organized two international symposia in Seoul, Korea. KRIEFS, established by the Korean Food and Drug Administration in March 2010, envisioned these international symposia as means to obtain advanced experiences from other countries and to reflect on and refine the KRIEFS's survey design and methods. METHODS: For the first symposium KRIEFS invited human biomonitoring (HBM) experts from Germany, the US, and Canada. The visiting HBM experts shared the details of their national studies while the KRIEFS discussed study design, as well as the methods, results and policymaking processes of the KRIEFS project. The second symposium was organized to share the survey design and results on endocrine disruptors from Taiwan. RESULTS: The Speaker from Germany introduced the German Environmental Survey and shared their experiences in HBM design, such as the process of developing reference values, and discussed the new HBM plan in Germany and the European Union. The Representative from Canada shared insights from national HBM approach. In the case of the US, the speaker focused on risk communication with subjects in epidemiological studies. In the second international symposium, the speaker shared the experience of endocrine disruptors'studies from Taiwan. CONCLUSIONS: KRIEFS was able to better understand previous nationwide HBM research designs, policy making process, and risk communication with research subjects.

Lichen nitidus is an uncommon chronic inflammatory disease characterized by multiple minute discrete flesh-colored dome shaped papules. Its distribution is frequently localized on trunk, arm or penis. There are a few cases reported to occur on palm and sole. We report a case of lichen nitidus which presents erythematous or brownish colored hyperkeratotic plaques, composed of 1-2 mm sized small papules on the right palm. Its hitopathologic findings were consistent with a diagnosis of lichen nitidus.

Hypereosinophilic syndrome (HES) is a heterogeneous disorder characterized by persistent hypereosinophilia with the evidence of organ dysfunction caused by eosinophilic involvement. HES can be induced by various secondary causes, including helminthic infections, adverse drug reactions, and allergic diseases. Primary/clonal bone marrow disease, including genetic mutations in platelet driven growth factor receptor alpha (PDGFRA), platelet driven growth factor receptor beta (PDGFRB), and fibroblast growth factor receptor 1 (FGFR1) could be its causes. Although corticosteroids are the mainstay of therapy in confirmed HES, imatinib is considered a definitive treatment for HES with these mutations. However, there have been few reports about HES with these genetic mutations in Korea. Here, we report a patient who presented with sudden onset of congestive heart failure and hypereosinophilia, proved to have PDGFRB rearrangement, and was controlled successfully with imatinib after left ventricle thrombectomy.

Intractable diarrhea associated with secondary amyloidosis in rheumatoid arthritis (RA) is a serious clinical entity with poor prognosis. We describe a 39-year-old male RA patient who presented with intractable diarrhea. Biopsy findings of terminal ileum and colon revealed amyloidosis secondary to RA. Effective treatment of rheumatoid arthritis resulted in remission of intractable diarrhea caused by amyloid protein deposition.

OBJECTIVE: To evaluate the clinical factors that contribute to femoral neuropathy subsequent to abdominal hysterectomy. METHODS: From March 1993 to March 2000, retrospective study on 17 cases of femoral neuropathy subsequent to abdominal hysterectomy was performed. Sixty eight patients who had normal neurologic finding on the lower extremities after the same operation were used as a control group. RESULTS: The incidence of femoral neuropathy subsequent to abdominal hysterectomy was 1.67%. Age, body weight, parity and type of skin incision were significantly different between study and control group. The mean age of the study group was 38.1+6.1 years and that of the control group was 43.2+/-8.9 years(p<0.05). The mean body weight of the study group was 52.9 6.4kg, while that of the control group was 57.8+/-7.4kg(p<0.05). The mean parity of the study group was 1.9+/-0.7 as opposed to 2.6+/-1.3 for the control group(p<0.05). Pfannenstiel`s incision was performed in 13 cases(76.5%) in the study group as opposed to 34 cases(50%) in the control group(p<0.05). There were no correlation between these two groups with respect to patient height, operative time, transfusion and change in hemoglobin level. In the study group, 13 cases(76.5%) with femoral neuropathy on the left side were found, 3 cases(17.6%) on the right side and 1 case(5.9%) on both side. Left side femoral neuropathy was more common than the right(p<0.05). Spontaneous recovery occurred in 16 cases of the study group within 4 months and, although residual symptoms were noted in the remaining 1 case, no serious sequelae have been observed. CONCLUSION: It was suggested that pelvic retractor compresses the femoral nerve during the abdominal hysterectomy. The patients age, body weight, parity and a type of skin incision could be contributing factors to femoral neuropathy.

Partial hydatidiform mole and coexisting fetus is a rare entity with antecedent high risk of maternal and fetal complications, and risk of persistent trophoblastic disease in later life. Here, we report a case of twin pregnancy with live fetus identified as 45,X and normal placenta and another partial mole. Ultrasound scan at 10 weeks showed a hydrops fetus with a focal area of multicystic placenta. The patient underwent chorionic villus sampling and amniocentesis for chromosomal analysis, and the result was 45,X. Based on these finding, the patient then underwent induced abortion. Pathological examination (immunohistochemical staining) of the placenta confirmed the partial mole. This report suggests that careful prenatal ultrasonography and appropriate karyotyping in a molar pregnancy and coexisting fetus enable early diagnosis and may be beneficial for prognosis.

Klebsiella pneumoniae carbapenemase-producing K. pneumoniae (KPC-KP) has been disseminating nationwide due to clonal spread and is taking a serious action at the national level in Korea. The mobilized colistin resistance (MCR1) gene confers plasmid-mediated resistance to colistin and is known to be capable of horizontal transfer between different strains of a bacterial species. We have experienced a fatal case of the patient who developed MCR1-possessing, ST307/Tn4401a[blaKPC2] K. pneumonia bacteremia in the community of non-capital region after being diagnosed as pancreatic cancer with multiple liver metastases and treated in the capital region. The ST307/Tn4401a[blaKPC2] K. pneumonia was the most commonly disseminated clone in Korea. Our strain is the first MCR1 and KPC2 co-producing K. pneumonia in Korea and our case is the critical example that the multi-drug resistant clone can cause inter-regional spread and the community-onset fatal infections. Fortunately, our patient was admitted to the intensive care unit on the day of visit, and the contact precaution was well maintained throughout and KPC-KP was not spread to other patients. The high risk patients for KPC-KP need to be screened actively, detected rapidly and preemptively isolated to prevent outbreak of KPC-KP. Inter-facility communications are essential and the nationwide epidemiologic data of KPC-KP should be analyzed and reported regularly to prevent spread of KPC-KP. The prompt identification of species and antimicrobial susceptibilities for successful treatment against KPC-KP should be emphasized as well.

OBJECTIVES: Maternal depression has a detrimental effect on baby growth. Recent reports suggest that depressive symptoms are more likely to occur during pregnancy than in the postpartum period. In Korea, there are relatively few studies of depression during pregnancy compared to those related to postpartum depression. The purpose of this study is to identify factors associated with antenatal depression. METHODS: The study included 143 pregnant women who had completed the Korean version of the Edinburgh Postnatal Depression Scale (K-EPDS), the Korea-Marital Satisfaction Inventory's global distress scale, the Rosenberg Self-Esteem Scale, and the Connor-Davidson Resilience Scale-2. Based on the K-EPDS scores, we divided the participants into two groups. Logistic regression was performed to identify factors associated with antenatal depression. RESULTS: Thirty (21%) of the subjects were evaluated as being depressed, pregnant women. Pregnant women with high self-esteem and marital satisfaction were less likely to have depression. Similarly, those who are younger and those with an abortion history were more likely to have depression. Past psychiatric history and family history were not significantly different between the two groups. CONCLUSION Dissatisfaction with marriage, low self-esteem, younger age, and abortion history were closely related to the presence of antenatal depression. The results of this study can be used as baseline data for the development of family-based education programs and early antenatal depression policies.
