1.Prediction of an actual birth within one week by ultrasonographic examination at 38 weeks' of gestation.

OBJECTIVE: The aim of this study is to predict spontaneous labor onset delivery within 7 days in low risk pregnant women at 38 weeks' of gestation by ultrasonographic examination of cervical changes. MATERIAL AND METHODS: This prospective study included 110 singleton low risk pregnancies between 37(+0) and 37(+6) weeks of gestation. Fifteen cases were lost during follow-up and finally 95 pregnant women (58 nulliparous, 37 multiparous) were analysed. The study period was from Oct/2005 to May/2007. Four cervical changes (length, gland thickness, funneling and canal formation) were evaluated. Main outcome was remaining day to delivery after the examination. Remaining days to actual delivery with spontaneous labor onset were recorded and the pregnancies were divided into two groups according to remaining days (within 7 days, over 7 days) to compare predicting power of delivery within 7 days. ROC curves were drawn to find out cut-off values of cervical length and gland thickness. Sensitivity, specificity, positive predictive value and negative predictive value were extracted from four cervical changes. RESULTS: Mean cervical length of pregnant women at 38 weeks' of gestation was 25.8 (+/-10.0) mm and mean cervical gland thickness was 4.3 (+/-1.2) mm. Funnelings of uterine cervix were detected in 13 cases (13.7%), canal formations in 6 cases (6.3%). All four cervical changes were statistically valuable to predict delivery within 7 days and the cervical length showed highest sensitivity. When the cervical length was measured under 20 mm, the possibility of delivery within 7 days was 78.6% (p<0.001). The cervical gland thickness less than 4 mm could predict the delivery within 7 days with sensitivity of 57.1% (p<0.01). Sensitivities of funneling and canal formation for delivery within 7 days were 54.5%, 36.4% each. CONCLUSION: Ultrasonographic examination of the cervical changes in low risk singleton pregnancy at 38 weeks' of gestation are valuable for predicting spontaneous labor onset delivery within 7 days. Among four cervical changes, cervical length is most sensitive ultrasonographic marker.

2.Subjective Symptoms of Peri-Implantitis and Satisfaction according to Prosthesis Methods.

From March 1, 2016 to August 31, 2016, 375 dental implant patients between the ages of 30 and 70 years to evaluate their subjective symptoms and satisfaction according to prosthesis methods. The collected data were analyzed using the chi-square distribution test, t-test, one-way analysis of variance, and multiple logistic regression. Of the patients with subjective symptoms of peri-implantitis, 40.5% reported food impaction, 49.1% reported bleeding, 61.1% reported pain, 61.9% reported halitosis, and the overall satisfaction level with the implants was 3.95 points. In the prosthesis methods, implant-teeth connective group showed the lowest level of the food impaction (21.3%), the pain (35.5%), and the halitosis (36.6%) (p<0.001), and the lowest bleeding was the implant single (33.8%) (p<0.05). Patient satisfaction was higher in the implant-teeth connective group (4.06 points) and the group without bone grafting (4.03 points) (p<0.01). Factor analysis showed that implant satisfaction was 0.43 times lower in the single implant group and 0.44 times lower in the implant-implant connective group than in the implant-teeth connective group. As a result of the study, it is expected that the method of extending the life of adjacent teeth and restoring the function of the tooth defect region is an implant-teeth connection method, so that it can be utilized positively through various clinical studies.

3.Small Cell Lung Cancer at Subcarina Presenting as a Metastatic Brain Tumor.

A 59-year-old man was rushed to the emergency room. The patient complained of headache with impaired memory function. Brain MRI showed a necrotic tumor in Lt cerebral hemisphere, with severe peritumoral edema (Fig. 1). Pathologic examination of the brain lesion confirmed that the tumor was a small cell lung cancer (SCLC). Chest computed tomography revealed a large soft tissue mass with central necrosis at subcarinal area in spite of an initial normal chest X-ray (Fig. 2). Bronchoscopic biopsy of the polypoid mass at subcarina revealed that the mass was a SCLC (Fig. 3). This is the case of SCLC only with an extrapulmonary symptoms despite of a normal chest X-ray. When metastatic brain tumor was found, appropriate chest evaluation should be performed even though chest X-ray was normal because brain is a common site of invasion of lung cancer.

5.Evaluation of the Efficacy and Tolerability of Isradipine in the Treatment of Mild to Moderate Hypertension.

An open clinical trial was conducted to evaluate the efficacy and tolerability of isradipine in 30 cases (male 16, female 14 cases, average age 52.6+/-7.94) of mild to moderate essential hypertension using 1.25-2.5mg twice a day for 8 weeks of active treatment. Blood pressure was significantly reduced from 168.5+/-14.33/108.3+/-6.37mmHg, 163.7+/-9.74/105.5+/-7.1mmHg to 141.0+/-13.69/92.0+/-9.27mmHg, 138.8+/-13.46/92.3+/-11.16mmHg in sitting and standing position respectively. The extent of reduction was 27.5/16.3mmHg in sitting position and 29.9/13.2mmHg in standing position. This comprised the mean response rate in terms of reduction of DBP of 10mmHg or more being 90% and the normalization rate, deficed as DBP lowering to 90mmHg or below, being 70%. Heart rate, hematology and blood chemistry including blood sugar and lipids were not changed significantly after treatment with isradipine. No significantl side effect was observed except 2 cases of mild transient facial flushing and nausea during the treatment, so could proceed the trial without drug discontinuation in all 30 cases. The results suggest that isradipine is one of the useful and safe drugs in the treatment of mild to moderate essential hypertension.

6.Clinical Effects of Isosorbide 5-Mononitrate(Elantan(R)) on Angina Pectoris.

An open trial was carried out to investigate the efficacy and tolerance of isosorbide 5-mononitrate(Elantan(R)) in 30 patients diagnosed as angina pectoris at Pusan national university hospital. Drugs were given 20mg two times daily for 2 to 3 weeks and the results were assessed in terms of effects on anginal pain and untoward side effects experienced during administration of medication. Treament resulted in an overall improvement in 25 patients(83.4%), complete abolition of anginal attacks in 17 patients(56.7%) and reduction in frequency of attacks in 8 patients(26.7%). So called "Nitrated headache" was observed in 5 patients(16.6%) but improved with proceeding of administration in 3 patients. This trial indicates that isosorbide 5-mononitrate should be efficacious as well as tolerated in patients with coronary heart disease.

Excessive calcium is thought to be a critical step in various neurodegenerative processes including ischemia. Calbindin D28k (CB), calretinin (CR), and parvalbumin (PV), members of the EF-hand calcium-binding protein family, are thought to play a neuroprotective role in various pathologic conditions by serving as a buffer against excessive calcium. The expression of CB, PV and CR in the ischemic rat retina induced by increasing intraocular pressure was investigated at the transcript and protein levels, by means of the quantitative real-time reverse transcription-polymerase chain reaction, western blot and immunohistochemistry. The transcript and protein levels of CB, which is strongly expressed in the horizontal cells in both normal and affected retinas, were not changed significantly and the number of CB-expressing horizontal cells remained unchanged throughout the experimental period 8 weeks after ischemia/reperfusion injury. At both the transcript and protein levels, however, CR, which is strongly expressed in several types of amacrine, ganglion, and displaced amacrine cells in both normal and affected retinas, was decreased. CR-expressing ganglion cell number was particularly decreased in ischemic retinas. Similar to the CR, PV transcript and protein levels, and PV-expressing AII amacrine cell number were decreased. Interestingly, in ischemic retinas PV was transiently expressed in putative cone bipolar cell types possibly those that connect with AII amacrine cells via gap junctions. These results suggest that these three calcium binding proteins may play different neuroprotective roles in ischemic insult by their ability to buffer calcium in the rat retina.

8.A Rare Case of Fat-Forming Variant of Solitary Fibrous Tumor Presenting as a Pleural Mass.

The fat-forming variant of solitary fibrous tumors (SFTs) is a rare soft tissue neoplasm that was previously referred to as a lipomatous hemangiopericytoma (L-HPC). The most common affected site is deep soft tissue. Here, we present the first case, worldwide, of a fat-forming variant of SFT of the pleura. A 74-year-old man presented with left lower chest pain. Chest radiographs showed a mass-like lesion at the left lower lung field and chest computed tomography revealed a 12 cm fat-containing enhancing mass that was well-separated, lobulated and inhomogeneous. Radiology findings suggested a liposarcoma. Percutaneous needle biopsy was performed and pathological diagnosis of the mass was a fat-forming variant of SFT. Surgical resection was carried out and there has been no recurrence to date. So, a benign fat-forming variant of SFT must be considered as one of the differential diagnoses of lipomatous tumors of the pleura.

Noonan syndrome is an autosomal dominant disorder characterized by typical facial features, congenital heart disease, and short stature. Diagnosis is difficult only with clinical symptoms and it is recently confirmed with gene study. The genotype-phenotype correlations have been reported. We report a newborn with KRAS gene mutation. This is the second report of case with KRAS gene mutation in Korea. So we hope this case will be a help to diagnosis and treatment of Noonan syndrome from birth.

10.The Experience of Applying an Australian Red Blood Cell Safety Stock Calculation to Korean Hospitals.

BACKGROUND: The management of red blood cell inventory in hospital's blood bank is crucial. The Australian Red Cross Blood Service developed a RBC safety stock calculation method (abbreviated as the ‘Australian formula’). In this study, we applied this method to four Korean hospitals to calculate the safe RBC stock level. METHODS: The hospitals included in this study were three tertiary teaching hospitals and one teaching hospital. The number of hospital beds in these hospitals were 1093, 1330, 1400, and 854, respectively. The data were collected from the Korea Blood Inventory Monitoring System of Centers for Disease Control & Prevention. The target/minimal/maximal RBC inventory levels and inventory days (inventory level/average daily usage) by ABO blood types were calculated using the daily red cell transfusion, wastage, and supply data between May and October 2016. RESULTS: The enrolled hospitals showed different levels for the target/minimal/maximal RBC inventory according to each blood group. The average of RBC inventory days in the four hospitals was 4.2 days. For each blood group, RBC inventory days were 3.2~4.4 days for O blood group type, 3.5~4.7 days for A blood group, 3.9~4.5 days for B blood group, and 3.9~5.5 days for AB blood group. CONCLUSION: Because the optimal RBC inventory levels are different depending on the hospital characteristics and the ABO blood group, it is necessary to set the RBC inventory levels for each hospital distinctly. The data obtained in this study will help manage blood product inventory in various hospital blood banks.
