2.A Study on the Stress and Coping Patterns of Mothers with Cerebral Palsy Children.

Mothers with cerebral palsy children have much stress that is related to the care of children with cerebral palsy and to other household duties, and this state of the mother has an effect on the cerebral palsy child and on other household member. Mothers in such stressful situations use various coping patterns. The purpose of this study was as follows : to develop instruments that can be used for measuring the stress and coping patterns of mothers with cerebral palsy children, and to test a hypothetical model on the relationship between the mother's stress, her coping patterns and the variable affecting the stress and coping patterns. The result of this study can be summarized as follows : 1. The stress scale was composed of 44 items and Cronbach's alpha was .94, and the coping pattern scale was composed of 19 items and Cronbach's alpha was .80. The mean score of stress scale was 136.12 out of a total of 220, and the mean score of the coping scale was 72.87 in a total of 95. 2. In test of the hypothetical model, it was found that extra-care demand, the support of the husband, the degree of handicap, health status and self-esteem had statistically significant influence on the mother's stress(r=.285,-.262,-.133,-.126). And the support of the husband, formal support, informal support, and economic status were found to have statistically significant influence on the mother's coping patterns(r=.412, .178, .178, .138).

3.The Effects of Laughter Therapy for the Relief of Employment-Stress in Korean Student Nurses by Assessing Psychological Stress Salivary Cortisol and Subjective Happiness

Korean student nurses may be exposed to stress caused by their future employment (employment stress). The aim of this study was to investigate the effects of a Laughter Program on psychological stress, by assessing salivary cortisol and the subjective happiness of student nurses in order to relieve employment stress. A quasi-experimental, non-equivalent, control-group, and pre-test/post-test was conducted in 4th year student nurses ( The results of the study showed that the Laughter Program was effective in relieving employment stress and increasing the subjective well-being of student nurses. Psychological stress ( This study is an effective evidence-based intervention to reduce student nurses employment stress and improve subjective happiness.

5.Analgesic Effects according to the Dose of Continuous Epidural Infusion of Morphine and Clonidine after Epidural Anesthesia for Cesarean Section.

BACKGROUND: Epidurally administered clonidine represents an approach to control the pain after cesarean section that produces analgesia by an alpha 2-adrenergic mechanism and may provide postoperative analgesia without nausea, pruritus, and respiratory depression associated with systemic or intraspinal opioid administration. This study was undertaken to evaluate the analgesic effect according to the dose of appropriate bolus and the combination of epidural morphine and clonidine after cesarean section. METHOD: Forty five women, ASA physical status 1 or 2, scheduled for elective cesarean section were randomly assigned to receive epidural administration for postoperative pain control. Group A (n=15) received 1 mg morphine and 75 g clonidine bolus followed by 4 mg morphine and 150 g clonidine. Group B received 2 mg morphine and 75 g clonidine bolus followed by 4 mg morphine and 300 g clonidine. Group C received 3 mg morphine and 75 g clonidine bolus followed by 4 mg morphine and 450 g clonidine. Each combination of drug was injected at Bromage scale 1 after surgery. RESULTS: All groups showed relatively stable hemodynamic status. Analgesic effect was significantly better in Group B and C than Group A (p<0.05). Sedative effect was not a problem in all groups. The incidence of side effects in Group C was more than Group B, which than Group A, but almost cases did not require treatment. CONCLUSION: 2 mg morphine and 75 g clonidine bolus followed by 4 mg morphine and 300 g clonidine with excellent analgesic effect and less side effects is considered as adequate dose in postoperative pain control after cesarean section with epidural anesthesia.

6.Orthogonal versus Parallel Plating for Distal Humeral Fractures.

In orthopedic trauma surgery, treatment of intraarticular distal humerus fractures is a challenge. With development of implants and biomechanical studies, surgical strategies with recommendations including preoperative computed tomography images, proper approaches and open reduction and internal fixation with dual plates have emerged. In addition, as an effort to provide stable fixation to permit early elbow motion, different methods of internal fixation, particularly plate configuration, have evolved. Using dual plates, either oriented parallel to each other or orthogonal, stable fixation has been achieved and satisfactory clinical outcomes have been reported. With rationales and advantages/disadvantages of each plate configuration, both techniques are selected according to surgeons' preference, and, in specific cases, one could be preferred over another. The key to successful fixation by either technique is obtaining anatomical reduction with restoration of two stable columns of the distal humerus.

7.Pregnancy Rate by Intrauterine Insemination (IUI) with Controlled Ovarian Hyperstimulation (COH).

The effectiveness of intrauterine insemination (IUI) combined with controlled ovarian hyperstimulation (COH) in the treatment of infertility with various etiologies was compared in a total of 152 cycles. Patients received a maximum of three IUI cycles for the treatment. Severe male (<2 x 10(6)motile sperm) or age facto. (> 39 y) patients were excluded in this study. Pregnancy was classified as clinical if a gestational sac was seen on ultrasound. The overall clinical pregnancy rate was 7.9% per cycle (12/152) and 9.7% per patient (12/124). The pregnancy rates were 0% in unstimulated natural (0/l8), 7.5% in CC (3/40), 8.2% in CC+hMG (4/49), 5.9% in GnRH-a ultrashort (1/17), 5.9% in GnRH-a long (1/17) and 27.3% in dual suppression cycles (3/11), respectively. The pregnancy rate was higher in dual suppression cycle than other stimulated cycles, but this was not significant. The multiple pregnancy rates were 25.0% (2 twins and 1 triplet). No patient developed ovarian hyperstimulation. Abortion rates were 66.7% in CC (2/3) and 100% in ultrashort cycles (1/1). The livebirth rate was 5.9% per cycle (9/152) and 7.3% per patient (9/124). There were no differences in age, duration of infertility, follicle size, total ampules of gonadotropins and days of stimulation between pregnant and non-pregnant groups. However, significant(P<0.05) differences were observed in the level of estradiol (E2) on the day of hCG injection (3,266.6+/-214.2 vs 2,202.7+/-139.4 pg/ml) and total motile sperm count (212.1+/-63.4 vs 105.1+/-9.9 x 10(6)) between pregnant group and non-pregnant group. These results suggest that lUl combined with successful ovarian stimulation tends to improve the chance of pregnancy as compared to lUl without COH and a total motile sperm count may be considered predictive of the success for pregnancy.

8.A Case Report of Frontometaphyseal Dysplasia.

Frontometaphyseal dysplasia is an uncommon genetic syndrome affecting bone and connective tissue. This condition is characterized by the prominence of supraorbital ridges, hyperostosis of the skull, mandibular hypoplasia and malocclusion with antegonial notching, underdeveloped paranasal flaring of the long bone, and a widened iliac wing. Frontometaphyseal dysplasia has an X-linked dominant trait. Clinical manifestations are more severe in males and have extreme variability in females. We present an 11-year-old boy who had frontometaphyseal dysplasia with disfigured face due to bilaterally urinary tract malformation and chronic urinary tract infection. To improve facial appearance, his prominent supraorbital ridges were contoured by ostectomy and burring through bicoronal incision. Urologic operation was performed simultaneously. Histologically, resected bony specimen revealed bony tissue with normal trabecular pattern suggesting exostotic bone. The patient healed without any postoperative complications and he and his parents were satisfied with his final facial morphology.

9.The Types and Perceptions of Touch in Nurse-Elderly Patient Interactions.

This study examined the types of touch received by elderly patients from nurses, and explored the elderly patient' and nurses' perceptions of touch occurring during nursing activities. Non-participant observation and semi-structured interviews were the methods employed for data collection. The study subjects were composed of 24 nurses of the three medical wards, and their 83 elderly patients who were hospitalized at S Hospital in Seoul from February 15th to March 15th in 1996. The Observation Schedule, the Perception of Touch Instrument, and the Patient classification were study instruments. The most common types of touch occurring during the nurse-elderly patient interactions were : task touch(82.9%), caring touch(4.9%), and concurrent type of touch (12.2%). The mean score of the elderly patients' perception of touch was higher at 30.45(range 5-35)compared to the mean score of the nurses' perception of touch which was 23.01(range 5-35). Elderly patients received the nurses' touch much more positively than the nurses. But the two scores were not significantly correlated(r=.06, 29). The elderly patient's touch perception score was higher(p<.05) in the group that was touched upon approach than the group was touched later. Elderly patients preferred caring touch to task touch, but it was not significant. Nurses' touch perception scores were higher(p<.05) for female than male. Elderly patients felt most comfortable when the nurses administered the touch on their painful site.

10.Continuous Axillary Brachial Plexus Block after Upper Extremity Operation.

Background: The continuous brachial plexus block brings about the prolongation of surgical anesthesia, postoperative pain relief, and sympathectomy. Methods: The twenty-five patients scheduled for upper extremity operations had been taken the continuous axillary brachial plexus block procedure to relief postoperative pain and, to get the sympathetic block effect. We used local anesthetics less than maximum recommended dose, 0.125% bupivacaine continuously infused at 4 ml/hour via catheter. We adopted the faces pain rating scale to evaluate postoperative pain degree. The faces pain rating scale composed of six faces, based upon the degree of pain. The patients has been asked to choose these rating scale according to their painfulness three times a day for five days. Results: Clinically, we have found the effective decrease in the degree of pain. Systemic toxic reactions, hematoma and infection sign at injected site were not found. Conclusion: The pain of upper extremity after operation was controlled by continuous infusion of 0.125% bupivacaine, 4 ml/hr, usually.
