1.Effect of MK-801 on the Prevention and Treatment of Tardive Dyskinesia.

Tardive dyskinesia(TD) is a serious side effect of long-term treatment with neuroleptic medications. To investigate if glutamatergic hyperfunction is associated with TD, effect of MK-801 on the prevention and treatment of TD was studied using a rat model, i.e., vacuous chewing movements(VCM). When comparing VCM scores of Group I(haldol decanoate+MK-801) with that of Group II(hadol decanoate+phosphate buffer saline), late measured VCM scores of former were significantly lower than that of latter, meaning that MK-801 is effective in the prevention of VCM. Furthermore, when MK-801 is also effective in the treatment of VCM. From the above results, it is suggested that glutamatergic hyperfunction might be involved in the development of TD and MK801 could be effective in the prevention and treatment of it.

5.A Comparative Study of Recurrence Rate in Bilateral Pterygium Surgery: Conjunctival Autograft Transplantation Versus Bare Scleral Techinque.

PURPOSE: When the patients with both recurrent and primary pterygia, We performed conjunctival autograft transplantation for recurrent pterygium and bare sclera technique for primary pteygium. We then compared the recurrence of each case in order to find out the usefulness of each surgery and effective mothod of surgery. METHODS: We performed follow-up on 26 eyes of 13 patients for 14 months (mean); 8 patients (16 eyes) were female, and 5 patients (10 eyes) male. The mean age of the patients was 49.5 years ranging from 37 to 63. In the operation, all patients received with 0.02% Mitomycin C for 1 minute. RESULTS: Pterygium recurred in 3 of 13 eyes which underwent bare sclera technigue, and did not recur in 13 eyes undergoing conjunctival autograft transplantation, but there was no significant difference because of a small number of study cases (P=0.07). We found postoperative complications such as corneal fila-ment and conjunctival granuloma in the case of bare sclera technique; but none in conjunctival autograft transplantation. CONCLUSIONS: We recommend a conjunctival autograft transplantation with application of low concen-tration of Mitomycin C as a safe and effective method for recurrent pterygium or pterygium with high recurrence rate.

8.Correction of Malunited Fracture of Zygoma Through Limited Incisions.

It is difficult to get a satisfactoryresult for the correction of malunited fracture of zygoma. Triple osteotomy and reposition of malunited zygoma is accepted as the better surgical method than camouflage surgery by means of onlays, if the orbital floor is to be reconstructed. The surgical approach can be divided into bicoronal, periorbital, intraoral and old scar. In 7 patients with malunited fracture of zygoma, the authors used a limited approach through extension of periorbital incision and intraoral incision instead of wide exposure including bicoronal incision. And we performed triple wteotomy and advancement of zygoma complex. The patients were followed for 4.5 months with acceptable result, and this approach was an effective method for the relatively simple tripod typemalunited fracture of zygoma. The authors obtained following conclusions: 1. Preoperative evaluation through thorough measurement of X-rays, investigation of photographs and detail communication with the patients was an important process.

9.A Case of Sjogren-Larsson Syndrome.

Sjogren-Larsson syndrome is a rare autosomal recessive neurocutaneous disorder due to a deficiency of the fatty aldehyde dehydrogenase. It consists of ichthyosis, spastic di- or quardriplegia and mental retardation. We report a case of a 9-year-old boy who was suspected to have Sjogren-Larsson syndrome. He had congenital ichthyosis, spastic diplegia and mental retardation. In addition, there was macular degeneration on the fundus. His neurological symptoms were improved after selective posterior rhizotomy and rehabilitation therapy. However, the dermatological problem did not subside with conservative treatment.

10.A Clinical Study of 40 Patients with Tsutsugamushi Disease in Chungcheung Province.

Forty patients(9 male and 31 female) with tsutsugamushi disease were evaluated clinically. The diagnosis of tsutsugamushi disease was confirmed by indirect immunofluoresent test or isolation of causative agent. More than 77% of patients were over 40 years of age. After the incubation period(5 to 13 days, average 8 days) symptoms developed suddenly. The frequent symtoms were fever(100%), chill(100%), headache(100%), rnalaise(97%), and myalgia(95%). On physical exammination the rash, eschar, and lymphadenopathy were found in 97%, 82%, and 67% respectively. Eschar was observed in the trunk, especially axillary, abdominal and inguinal area. Serum transarninase(SGOT snd SGPT) was elevated(89 and 87%). Urinalysis revealed proteinuria in 35%(12/34) and hematuria in 19%(7/36). Chest roentgram and ECG demonstrated abnormalities in 39%(14/36) and 73%(26/36) respectively, The treatment with doxycycline was very effective in all cases.
