3.Anterior Chamber Depth, Corneal Thickness and Corneal Endothelial Change following Decreased Intraocular Pressure.

PURPOSE: To determine the intraocular pressure (IOP) lowering effects on the change of anterior chamber depth (ACD), corneal thickness, corneal cell area and density in normal eyes. METHODS: We investigated 58 eyes of 29 volunteers for this study whose refractive error was +/-3 D, IOP was below 21 mmHg, anterior segment and fundus was within normal limit. All of them had normal visual fields. They were randomly classified into two groups. Anterior chamber depth, IOP, central corneal thickness, cell density, cell area, and coefficient of variation of corneal endothelium were measured by single observer in each group. After oral medication of 50% glycerin 1 cc/kg (body weight) in group 1 and Diamox 500 mg and K-contin 600 mg in group 2, IOP, anterior chamber depth, central corneal thickness, cell density, cell area, and coefficient of variation were measured at 1, 2, and 3 hours. RESULTS: In glycerin group after 1 hour, decreased IOP was stastically significant (P<0.05). In Diamox group after 2 hours, decreased IOP was stastically significant (P<0.05). But the change of anterior chamber depth, central corneal thickness, cell density, cell area, and coeff icient of variation was not stastically significant (p>0.05). There was no significant correlation between decreased IOP and the change of anterior chamber depth, central corneal thickness, cell density, cell area, and coefficient of variation. The IOP change was not statistically different between the two groups. CONCLUSIONS: There was no stastically significant correlation between decreased IOP in normal range and the change of ACD, central corneal thickness, cell density, cell area, and coefficient of variation.

5.Arteriovenous Hemangioma Developed after Fine Needle Biopsy of Thyroid.

Fine needle biopsy of nodules on the thyroid is simple diagnostic method and it is generally a safe procedure. The main side effect is pain and hematoma. Secondary hemangioma development after biopsy is extremely rare. A 63-year-old woman presented with a 5-year history of solitary erythematous papule on neck. Spontaneous bleeding recorded six times within 5 years. There is not vascular anastomosis in ultrasonographic imaging. A histopathologic study showed vessel-like arteriole around small veins in dermis. No arteriovenous hemangioma case has been previously reported to be related to fine needle biopsy. Our case showed possibilities of arteriovenous hemangioma developments after fine needle biopsy.

7.Clinical Characteristics of Lichen Planus.

BACKGROUND: The clinical presentation of lichen planus varies depending on the area involved. It occurs in less than 1% of the world's population: also, the prevalence in Korea is relatively low. However, the number of our outpatients has been increasing rapidly. OBJECTIVE: We studied the clinical characteristics of 100 patients with biopsy proven lichen planus. METHODS: We reviewed the medical records and clinical photographs of 100 patients who had been diagnosed with lichen planus during the last 10 years, from January 2002 to December 2011. RESULTS: Over the past 10 years, patients with Lichen Planus accounted for 0.196% among the new outpatients of this dermatology. The rate of Lichen Planus increased when it occurred on the skin, except for the Mucosal area. Of the 100 outpatients, 62% were men and 38% were women. The mean age at diagnosis was 54 years, and more than half of the patients were between 40 and 60 years of age. The most common site of lichen planus was the lip, which was found in 44% of the patients. Oral lesions were the second most commonly involved site at 28%, followed by the body at 20% and the genital area at 8%. The patients complained of various symptoms such as pain, burning sensation, pruritus, and irritation. Various treatments had been tried by all patients. CONCLUSION: This study of domestic patients with Lichen Planus is the biggest clinical research performed at a single center. The clinical features of patients in this survey share many similarities with those reported previously, but showed some differences too.

8.Neutrophilic Eccinre Hidroadenitis in Acquired Immune Deficiency Syndrome.

Neutrophilic eccrine hidradenitis (NEH) usually occurs in patients receiving chemotherapy for acute myelogenous leukemia or other malignant diseases. We experienced a case of NEH occurring in an HIV (human immunodeficiency virus)-positive 35-year-old male patient. The patient presented with a 1-year history of recurrent erythematous papule on his upper extremity. Laboratory studies showed that Anti-HIV is positive and HIV RNA PCR is 1.02x106. Histopathologic study showed neutrophilic infiltration within and around the eccrine glands and degeneration of the eccrine glands.

9.Association of the 5-HT5A Receptor Gene Polymorphisms and Citalopram Response in Korean Patients with Major Depressive Disorder.

OBJECTIVES: Some reports have suggested that 5-HT5A polymorphism allelic association was associated with depression, however, there has been no report about relationship between the 5-HT5A gene and antidepressant response. We conducted the association study of the 5-HT5A receptor gene polymorphisms (-19G/C,12A/T) and response to citalopram in Korean patients with major depressive disorder(MDD). METHODS: A total of 106 patients with major depressive disorder were included in this study. The patient's symptoms were measured by 21-item Hamilton Depression Rating Scale(HAMD) at baseline, week 1, week 2, week 4 and week 8 during citalopram treatment. A Responder to citalopram was defined by 50% reduction of total HAMD scores. To analyze genetic polymorphisms, a polymerase chain reaction based method was used. RESULTS: At week 8, responders were 62, non-responders were 44. No significant differences of genotypes or allelic association in 19G/C and 12A/T polymorphisms were observed between responsive and non-responsive patients. CONCLUSION: These results do not support the hypothesis that this polymorphism of the HT5A receptor gene is involved in the therapeutic response to citalopram.
