1.Computerization of Surgical Pathology Reporting and Data Storage by Automatic Coding System using Personal Computer.

The authors developed a computer program for use in report printing as well as data storage and retrieval system at the surgical pathology and its efficacy was evaluated at the Department of Pathology, Seoul National University Hospital. This program used IBM PC XT and was written in DBASE III plus language. The main features of the program included an automatic coding and decoding of the diagnosis, automatic searching of the previous biopsy during gross dictation, powerful word processing function and flexibility of the program. The data storage was carried out during the typewriting of the report, so that the typist's workload became markedly reduced. Two kinds of data files wer stored in the hard disk ; the temporary file contained full informations and the permanent file contained the core data only. Searching of a specific case was performed by pathology accession number, chart number, patient's name or by SNOMED code within a second. All the cases were arranged by copied to the diskette during the daily service automatically, with which data were easily restored in case of hard disk failure. The advantages of this program using a persosnal computer were discussed with comparison to those of larger computer system. Based on the experience of 8 months usage in Seoul National University, we assume that this program gives a sufficient solution to the surgical pathology service of many institutes where a large computer system with well designed software is not available yet.

3.A Case of Herpes Simplex Virus Esophagitis in a Renal Transplant Child.

Herpes simplex esophagitis can occur in those with normal immune function, but is more often seen in those who are immunocompromised. In one series, 5 percent of post-kidney transplant recipients had herpes esophagitis. We experienced a case of herpes simplex esophagitis, following renal transplantation in a 9 year old male. He complained of epigastric pain, nausea and blood-tinged vomiting. Endoscopic examination showed volcano ulcer, mucosal friability and multiple confluent ulcers covered by whitish exudates on elevated margin in the middle and lower esophagus. Microscopic findings revealed multinucleated giant cells, margination of chromatin, intense nonspecific inflammation and strong positive for herpes simplex virus immunohistochemical staining. Esophageal lesions and symptoms improved after acyclovir therapy.

4.A Clinical Study on Coenzyme Q10(Neuquinon(R)) in the Treatment of Congestive Heart Failure.

Coenzyme Q is concentrated in Golgi apparatus membranes and mitochondria, but not in other membranes. Although it is difficult to prove the metabolic action of coenzyme Q administered exogenously in clinical cases, the effect of this substance can be evaluated by criteria based on clinical findings. In an attempt to evaluate the effect of coenzyme Q for the treatment of 67 patients(male 26 cases, female 41 cases) of congestive heart failure, we administered Coenzyme Q1030mg daily for 4 to 8 weeks. Most of them were valvular heart disease(74.6%) and hypertension (14.9%). Clinical effects were evaluated at least 4 weeks later by the criteria using a scoring method of severity of congestive heart failure which was devised by Ishiyama, etc. In summary, a definite effect was found in 13 cases(19%) and a mild effect was observed in 46 cases(69%). During treatment there were no significant side effects, and also no significant changes in heart rate and blood pressure.

5.An Anesthetic Experience for Resection of Pheochromocytoma - 1 cases .

The author reports an anesthetic experience of right adrenalectomy. The patient was 38 years old male, having suffered from pheochromocytoma. Thiopental sodium, 300mg of 2.5% was used for induction, followed by N2O, O2, and halothane for maintenance and 0.1% S.C.C. solution in 500 ml of 5% D/W solution, dripped intravenously for muscle relaxation. AS the time of surgical manipulation of the gland, blood pressure was suddenly raised to 250/160mmHg, and a few minutes later massive bleeding, about 1800ml in amount, was detected in the operating and then the heart was arrested. Resuscitation was performed with adequate oxygenation and medications. The patient recovered well without any sequelae. The importance of preoperative preparation, sufficient sedation, smooth anesthetic induction, complete analgesia, good muscle relaxation, adequate ventilation and proper cardiovascular control are discussed together with the choice of anesthetic agents and practices.

6.An Anesthetic Experienee with Unilsteral Adrenalectomy for Cushing`s Syndrome due to Adrenocortical Adenama - 1 Case .

Anesthesia was induced with intravenous thiopental, followed by succinylcholine, N2O-O2-halothane and pancuronium. An endotracheal semiclosed circle absorption technic with controled ventilation was performed. Under continuous monitoring of blood pressure, pulse, EKG, CVP and urine output, the patient tolerated during the anesthetic period well. And high blood pressure was adequately controlled by increasing the percentage of halothane concentration only.

7.A Case of Imipramine(Tofranil(R)) Poisoning with Cardiac Arrhythmias.

Imipramine(Tofranil(R)) is one of the tricyclic antidepressants commonly used in depressive symptoms or enuresis. An accidental or nonaccidental poisoning of imipramine is common today. A 16 years old girl was admitted to Seoul National university Hospital because of unocnsciousness and frequent attacks of seizure after the impulsive ingestion of 1.4gm of imipramine. She showed variable arrhythmias such as complete RBBB, secod degree AV block and ventricular tachycardia and recovered from poisoning without residual myocardial damage. We present a case of imipramine poisoning with cardiac arrhythmias with review of literatures.

8.Clinical Effect of Procetofene(Lipanthyl(R)) on the Serum Lipids in the Hyperlipidemic Patients.

We observed the levels of serum cholesterol, triglyceride and HDL-cholesterol in 28-hyperlipidemic patients after treatment with procetofene(Lipanthyl(R)), a new hypolipidemic agent. The results were as follows. 1. The hyperlipidemic patients were 7 cases of pure hypercholesterolemia, 12 cases of mixed hyperlipidemia and 9 cases of pure hypertriglyceridemia. 2. All the patients were treated with daily dose of 200 to 400mg, usually 300mg, and duration of more than 12 weeks. 3. The serum cholesterol decreased significantly at the rate of 29% in pure hypercholes terolemia and 29% in mixed hyperlipidemia after treatment for 12 weeks. 4. The serum triglyceride decreased significantly at the rate of 58% in mixed hyperlipidemia and 42% in pure hypertriglyceridemia after treatment for 12 weeks. 5. The serum HDL-cholesterol increased at the rate of 10% in pure hypercholesterolemia, 14% in mixed hyperlipidemia and 26% in pure hypertriglyceridemia after treatment for 12 weeks, but the increase rate was statistically significant only in pure hypertriglyceridemia. 6. Transient epigastric discomfort was complained by 2 patients, but subsided spontaneously with continuous treatment. 7. In view of these results, procetofene appears to be an effective and well tolerated agent for the treatment of all the types of hyperlipidemia.

9.Needle Localization Biopsy of Nonpalpable Lesions of the Breast.

BACKGROUND: For the purpose of early detection of breast cancer, a localization biopsy is necessary in case of mammographically or ultrasonographically suspicious, but nonpalpable, breast lesions. METHODS: One hundred nine consecutive localization biopsies for nonpalpable lesions of the breast were performed upon 101 patients from May 1992 to September 1998. RESULTS: The localizations were done in 48 cases under the guidance of mammography and in 61 cases under the guidance of ultrasonography. Complications occurred in 15 cases (13.7%). The mean volume of the excised specimens was 11.6 cm3. The miss rate was 3.6% (4 cases). Malignant lesions of the breast were found in 16 cases (14.7%). Among the malignant lesions, 10 cases involved invasive ductal cancer. Among the patients diagnosed as having breast cancer, 8 patients were treated with a conservative operation, 5 patients with a modified radical mastectomy, and 2 patients with observation; 1 patient was discharged. The chance of a biopsy containing a malignant lesion was 16.7% if the biopsy was done because of mass, 9.8% for a microcalcification, and 11.1% for an abnormal asymmetric density. CONCLUSIONS: A needle localization biopsy can be performed accurately under local anethesia, and the volume of the excised specimen can be minimized, so the cosmetic effect is excellent and proper early treatment is possible in diagnosed cases of breast cancer.
