6.The Preventive and Therapeutic Effects of Aronox Extract on Metabolic Abnormality and Hypertension.

BACKGROUND: Aronox is an anthocyanin-rich extract from Aronia melanocarpa E which is known to have anti-inflammatory effect in atherosclerosis due to high anti-oxidative activity. This study was conducted to evaluate the preventive and therapeutic effect of Aronox on hypertension and metabolic status in spontaneously hypertensive rats (SHR). METHODS: Seven-week-old male SHR were orally administrated with Aronox (low dose; 100 mg/kg, n = 4 or high dose; 200 mg/kg, n = 3) or amlodipine (10 mg/kg, n = 7) and sham (n = 10) for 8 weeks. Aronox was administered 30 mg/kg for the first 3 weeks and then increased to 100 mg/kg (low dose group) or 100 mg/kg to 200 mg/kg (high dose group). Systolic blood pressure (SBP) was measured every week by tail cuff method. At 8 weeks, fasting lipid level was measured. Heart and kidney stained with Masson's trichrome. RESULTS: Aronox or amlodipine treatment showed significantly lower SBP compared with sham (202.2 +/- 10.2 mm Hg in low dose group, 202.0 +/- 12.6 mm Hg in high dose group and 187.4 +/- 22.7 mm Hg in amlodipine group vs. 224.4 +/- 12 mm Hg in sham-SHR, p < 0.005). There were no significant differences in cardiac and renal weight corrected by body weight among 3 groups. Aronox and amlodipine treatments significantly decreased fasting glucose and showed a trend of decrease in triglyceride level. Aronox or amlodipine treatment for 8 weeks showed less collagen deposition changes compared to sham. CONCLUSIONS: Aronox showed significant antihypertensive effects, decreased fasting glucose, and less cardiac and renal fibrosis in SHR. These results suggest that Aronox can be used as an adjuvant therapy or functional food in hypertension.

7.The Significance of Cervical Erosion with Benign Cellular Change on Papanicolaou Smear.

The significance of cervical erosion with benign cellular change on Papanicolaou smear has not been evaluated. A retrospective review of 430 coloscopically directed punch biopsies performed over 15 months revealed 206 cases who had a benign cellular change on cytologic smear. Of these 206 smears, 48(23%) had clear cervix without erosion, 99(48%) had mild erosion on cervix, 41(20%) had moderate cervical erosion and 18(9%) had severe cervical erosion. The false negative rate that revealed more than mild dysplasia on histologic diagnosis was 2% in patients without cervical erosion, 3% in patients with mild cervical erosion, 5 % in patients with moderate cervical erosion, and 11% in patients with severe cervical er- osion. Koilocytotic change that suggested human papilloma virus infection was 17% in each group regardless cervical erosion. In conclusion, benign cellular change on Pap smears in patients with severe cervical erosion may be associated with significant cervical cancer pathology.

8.Sympathetic Skin Responses after Stellate Ganglion Block in the Patient with Reflex Sympathetic Dystrophy.

OBJECTIVE: The purposes of this study were to measure the effect of Stellate ganglion block(SGB) objectively and quantitatively by the use of sympathetic skin response(SSR), and to evaluate the cumulative effects and complications of repetitive SGB and to find out optimal numbers of injection per one cycle in the patients with reflex sympathetic dystrophy(RSD). METHOD: Six patients with RSD were evaluated with a SSR test before and after the injection of 1% lidocaine 4 ml by SGB method. RESULTS: There was a significant prolongation of latencies in SSR of the lesion side of sixty mixed cases by the SGB methods and SSR tests. There were no significant changes in the latency and amplitude of SSR from the lesion side between pre- and post injection states. There was a significant decrease of amplitude in the sound side after the injection. The differences of the amplitudes between pre- and post injections were significantly higher in the lesion side than the sound side. The degree of pain of the patients with RSD was evaluated by visual analogue scale(VAS), which scored on pre and post injection state decreased from 10 to 6.5 by 5 times injections, but did not decrease by more injections. CONCLUSION: We concluded that SGB is more effective in the RSD lesion side than the sound side and the SSR is a useful test for evaluating the effect of SGB.

10.Case Development on Nurses' Ethical Dilemmas with Physicians' and Nurses' Decision Making.

PURPOSE: This study was done to develop a realistic clinical case and investigate nurses' decision-making about nurses' ethical dilemmas with physicians in the fields of nursing practice. METHODS: Case development and a hypothetical case study were used. Participants were 52 nurses. Data were collected in 2012 and 2013 using an open-ended questionnaire and interviews and analyzed using content analysis and descriptive statistics. RESULTS: Various dilemma situations between nurses and physicians, such as violence, deathbed, medication-prescription, and physicians' incapacity-unfairness, were suggested. A clinical dilemma case about medication-prescription was developed based on nurses' experiences. Nurses' responses to the developed case situation and responses were classified into five types. Various reasons were given for making the decisions and about 56% of the nurses decided to notify their supervisor without deleting nursing records. CONCLUSION: In this study, a realistic clinical dilemma case was developed, and nurses' ethical decision making was identified. These findings can be used in developing effective strategies for nurses to solve ethical dilemmas and to improve ethical decision-making abilities.
