2.Renal Angiomyolipoma with the Regional Lymph Nodes Involvement.

A 21-year-old woman, suspected of having a left renal mass on excretory urography, was referred to our hospital. Computed tomography of the abdomen demonstrated scattered areas of low density compatible with fat inside the tumor and enhanced outside by contrast media. Also regional lymph nodes were enlarged. Angiography showed avascular renal mass. Preoperative diagnosis was malignant umor of kidney with lymph node metastasis or angiomyolipoma involving the tkidney and the lymph nodes. Left radical nephrectomy was undertaken. Microscopic examination revealed angiomyolipomas of the kidney and the regional lymph nodes. The tumors of the lymph nodes are considered to originate multicentrically with that of the kidney. This is the first reported case of multicentric angiomyolipoma in Korea.

5.True Hermaphroditism.

True hermaphroditism is one of the rare congenital anomalies. Additional four cases of this anomaly are added to the 2 cases which were reported previously from the Dept. of Urology, Busan National University Hospital. Clinical review was performed about these 6 cases. The results were as follows; 1. The incidence of true hermaphroditism is 0.17% in 3,456 uro1ogicaladnrissions from Jan. 1964 to Mar. 1982, 1.6% in 380 admissions with congenital anomalies of the external genitalia and 25% in 24 admissions with hypospadias and cryptorchism during this period. 2. Hypospadias and cryptorchisrn are found in 6 cases, bifid scrotum 3 cases, and labioscrotal folds in 2 cases. Roentgenograms of retrograde urethrography were available in 4 cases and revealed blind vaginal pouch in 3 cases. Among these 3 cases, one had patent fallopian tube at the cephalad end of the pouch. The other one had well developed vagina and the uterus with patent fallopian tube. 3. The genital ducts were developed ipsilaterally in 9 gonads composed of pure testicular or ovarian tissues but developed to those of male pattern in 3 gonads containing the ovarian and testicular tissues. Among these 3 cases two cases did not have the vas deferens. 4. Of 6 gonads containing the testicular tissues, two were in the scrotum, 2 in inguinal and remained 2 were in the peritoneal cavity. Of gonads containing the ovarian tissues, seven were in the peritoneal cavity and one in the retroperitoneum. 5. Sex chromatin was positive in 3 cases, negative in 3 cases. 6. Of 5 cases who had been reared as male, two were assigned to female, one to male and remained 2 were not decided the sex. The other who had been reared as female was assigned to female. 7. The type of true hermaphroditism is lateral in 3 cases (Type I)and unilateral in 3 cases (Type IIIb 2 cases, Type IIIa I case)by the classification of Jones and Scott.

6.A Case of Sarcoidosis.

We report a case of sarcoidosis which developed in the skin, lung and eyes in a 50-year-old woman. The skin lesions showed two types of cutaneous manifestations which were subcutaneous nodules on her back and erytrematous papules on the face for several months. Our patient's seurm angiotensin converting enzyme (ACE) level was elevated at 33.5 IU/L(reference range, 8.3-21.4 IU/L) and the purified protein derivative(PPD) skin test had a negative result. On her roentgenographic examinations, we could find multiple nodular densities with hazziness on both her lungs and radionuclides(Gallium 67) uptakes in right lower lung field. The ratio of helper to suppressor T cells was 9 in bronchial lavage. Diagnosis of sarcoidosis was made on the basis of histologic findings of non-caseating granulomas from skin and lung tissue. Treatment was initiated with oral alternate-day prednisone 40mg. The cutaneous lesions and dyspnea improved in 2 and 4 weeks respectively, and serum ACE level returned to normal in 2 months.

8.Dynamic MRI of Breast Fibroadenoma: Pathologic Correlation.

PURPOSE: To analyze the dynamic MR imaging of breast fibroadenoma according to the histologic type for differentiation from breast carcinoma. MATERIALS AND METHODS: Dynamic MR images of 26 lesions from 22 breasts in 19 patients showing atypical clinical features or film mammogram and ultrasound manifestations were performed. We analyzed the speed and the maximal amount of contrast enhancement and the patterns, such as shape, border, and internal signal intensity, among the histologic types during five minutes after contrast injection. RESULTS: The speed and maximal amount of contrast enhancement of fibroadenoma were in descending order of myxoid, sclerotic, glandular, and calcified types. Among these, the value of maximal amount of contrast enhancement of myxoid and sclerotic type were more than 700 NU, but only myxoid type was enhanced more than 700NU within the first 1 minute after contrast injection, similar to the findings of carcinoma. In general, fibroadenoma showed the tendency of smooth surface(69%), well-defined border(88%) with safety rim, and internal homogeneous signal intensity(65%). However, sclerotic type of fibroadenoma had relatively high incidence of heterogeneous internal signal intensity(78%) after Gd-DTPA injection. CONCLUSION: Dynamic MR imaging of atypical breast fibroadenoma mimicking breast malignancy was very useful in differentiation it from carcinoma and had the benefit of classifying fibroadenoma according to its histologic types.

9.Bacteriological Study of Urinary Tract Infections.

Urine specimens were obtained from one hundred and thirty nine patients in-out-patient and in-patient service of B.N.U.H., Dept. of Urology, with the presumptive diagnosis of urinary tract infections or indwelling catheter. These specimens were processed by Triphenyltetrazolium Chloride test, quantitative test and identification of the organisms. The results were as follows: 1. Triphenyltetrazolium Chloride test was an easily performable and a fairmely accurate method for screening of urinary tract infections. 2. The urine specimens showed false negative reactions to triphenyltetrazoium Chloride in some cases with moderate (10(4) to 10(6) bacteria per milliliter) growth of bacteria or in the antibiotics used patients. 3. The incidence of urinary tract infections with gram negative bacilli are ninety-two percent. In their series, incidence of E. coli decreased and that of other gram negative bacilli increased such as Enterobacter, Proteus and Serratia, etc. This shift in the pathogen spectrum occurs mostly in the patients of complicated or lower urinary tract infections, in-patients, antibiotics used patients and the patients with indwelling catheter.
