4.Solid and Papillary Epithelial Neoplasm of the Pancreas: Radiologic and Pathologic Correlationt.

PURPOSE: Computed tomographic(CT), ultrasonographic(US) findings of solid and papillary epithelial neoplasm of the pancreas were correlated with pathologic findings for the better understanding of this disease entity. METHODS AND MATERIALS: A retrospective review of CT and US of 14 cases of solid and papillary epithelial neoplasm of the pancreas was carried out in terms of the margin, internal architecture, caicification and septation, and this was correlated with gross pathologic findings. RESULTS: CT and US findings were well defined round masses consisting of both solid and cystic components. Five cases were cystic, four cases were solid and five cases were mixed. Cystic portion of the tumor represented variable degree of hemorrhagic necrosis. Six cases contained foci of calcification, which were linear, marginal and amorphous. Marginal calcification interfered US examination of the mass in three cases. Internal septurn was demonstrated in four cases on CT, one case on US and three cases on gross specimen. CONCLUSIONS: Our results indicate that calcification and internal septurn were considered as a part of radiologic findings in solid and papillary epithelial neoplasm of the pancreas.

5.Conservative Treatment of Dysphagia.

Dysphagia can be caused by an anatomic abnormality or neurological dysfunction in either one or combination of all of these factors. Especially, dysphagia is one of common complications of stroke and related with the survival as well as prognosis of functional recovery of cerebrovascular accident. Irrespective of its primary causes of swallowing difficulty, the options for conservative treatment of dysphagia are adopted according to the functional status of the patient. The most important goals of the conservative management are promoting oropharyngeal transit and protecting the airway from aspiration. In this review, major non-operative therapeutic methods are illustrated: behavioral interventions including compensatory maneuvers, diet preparation, tube feeding and prostheses. Also, the clinical application of them is discussed. Each of these treatments should be applied to patients cautiously after a thorough consideration of the functional recovery and risk evaluation of aspiration. Furthermore, the nutritional requirement of patients ought to be taken into account.

7.A Case of Nasopharyngeal Teratoma Showing Upper Airway Obstruction.

Neoplasms of the nasophrynx are rare in neonates and infants. The most common nasopharyngeal tumor encountered in this age group is teratoma-neoplasm containing multiple heterotopic tissues. Four histologic types of nasopharyngeal teratoma occur-dermoid, teratoid, true teratoma, epignathiof which dermoid is most frequently seen. We have experienced a case of nasopharyngeal teratoma in a female neonate with signs and symptoms of upper airway obstruction. It was diagnosed by cervical. CT and confirmed as ture teratoma by histological examination. Nesopharyngeal teratoma was removed by contact Nd:YAG laser. In addition, we briefly reviewed the literature of neonatal nasopharyngeal teratoma.

8.Clinical Observation of Skin Inlay Urethroplasty in 11 Cases of Posterior Urethral Injury.

A clinical observation was made on 11 patients of posterior urethral injury who performed skin inlay urethroplasty in the Department of Urology, St. Mary`s Hospital, Catholic Medical College during the period from January, 1970 to December, 1983. The results were as follows: 1. Age distribution of these patients showed the highest range in the age 0-9 and 10-19(8 cases) 2. The most common cause of the posterior urethral injury was traffic accident.(9 cases), falling down accident (1 case) and blunt trauma (1 case). 3. The concomitant diseases at the time of urethral injury were pelvic bone fracture (9 cases), other bone fractures (3 cases), bladder rupture (2 case), other viscera injuries (2 cases) and retroperitoneal hematoma (1 case). 4. Involved sites of urethral injury were membranous urethra (9 cases) and bulbo-membranous junction (2 cases). 5. The previous procedures before skin inlay urethroplasty were suprapubic cystostomy only in 3 cases, and primary realignment or resection and end to end anastomosis in 8 cases. The 3 cases, who performed suprapubic cystostomy only had a long stricture in the posterior urethra, and the 8 cases were failed in primary realignment or resection & end to end anastomosis previously 6. The postoperative complications after skin inlay urethroplasty were meatal stenosis in 5 cases after primary stage, urethral stricture in 4 cases after secondary stage. The meatal stenosis was managed by urethral dilatation with metal sound, meatotomy and urethrotomy in 2 cases, and the remainder are undergoing of urethral dilatation at present due to recurrentmeatal stenosis. The urethral strictures in 4 cases after secondary stage were managed by urethral dilatation with metal sound, and urethrotomy in all cases satisfyingly. 7. Overall success rate were 73% (8 cases).

Diaper dermatitis is an infant dermatosis associated with the wearing of diapers, Despite of its high frequency in clinical pediatrics the clinical characteristics are not well described in the korean literatvre. We investigated the clinical and mycological characteritics of diaper dermatitis in 50 serial patients. Clinically diaper dermatitis was classified. into five categories,irritant contact derniatitis(ID), candidiasis(CD). combined rash of ID and CD(CR.), intertrigo, and miliari. Of 50 patients with diaper dermatitis, 18 had ID; 16, CI3; 10, CR; 5, intertrigo, and 1, miliaria, The mean age of patients with ID (11.8 months) was higher than that of patients with CD (4.0 months). Candida albicans(C. albicans) w is isolated from 25 patients with diaper dermatitis. The rate of positive cultures by clinical diagnosis was 130% in CD, 11.1% in ID, 60, 0% in CR, and 20.0% in intertrigo. C. albicons was isolated from 23 patients(82.1%) of patients with clinically suspected Ci") and 2 of 22 patients without the characteristic erythematous papulesThe presence of eryhematous papules was the most characteristic clinical feature of infection, Other candida species isolated from cut,aneous lesions were C. tropicalsis, C.'. parapsiolosis, and C. stellatoidea.

10.The Effect of Steroid on Heat Shock Protein 70 Expression in mdx Mice.

OBJECTIVE: To investigate the effect of steroid administration on the apoptosis and heat shock protein 70 (HSP70) expression after exercise in the animal model of Duchenne muscular dystrophy. METHOD: We measured Bcl-2, BAX and HSP70 expression by western blotting. 20 control and 20 mdx mice were divided into free-living (n=10) and exercise (n=10) groups. Free-living and exercise groups were further divided into steroid-treated and sham-treated groups to evaluate the effect of steroid administration. RESULTS: Apoptosis was most prominent in the sham-treated exercise group, while apoptosis was significantly reduced in the steroid-treated exercise group. HSP70 expression was maximized in sham-treated exercise group, whereas steroid administration inhibited HSP70 expression after exercise in muscular dystrophy animal model. Exercise loading was found to cause severe apoptosis but steroid administration alleviated apoptotic damage in mdx mice. CONCLUSION: HSP70 expression was suppressed in the steroid-treated exercise group, which suggests steroid might have major preventive effect in exercise-induced apoptosis of muscular dystrophy animal model.
