1.Malignant Myoepithelioma Arising in a Recurrent Pleomorphic Adenoma: A case report.

Malignant myoepithelioma is a rare neoplasm of salivary gland which may either arise de novo or develop in a pre-existing pleomorphic adenoma. The malignant myoepithelioma occurs in 0.45% of major salivary gland tumors. Malignant myoepitheliomas arising in the pleomorphic adenoma number less than 20 in English literature and 1 in Korea. We describe a case of malignant myoepithelioma arising in a recurrent pleomorphic adenoma of the left parotid gland of a 61-year-old man. The tumor was ill-defined and composed of polygonal or plasmacytoid myoepithelial cells. Infiltration to surrounding tissue, hemorrhage, necrosis, increased mitotic activity and vascular tumor emboli indicated its malignant nature. There were several satellite nodules with histologic features of typical pleomorphic adenoma. Immunohistochemically, tumor cells were reactive for S-100 protein, AE1/AE3, vimentin, smooth muscle actin and glial fibrillary acid protein.

2.Two cases of Cantrell's syndrome

Congenital absence of the lower sternum, defect of the abdominal wall, defects of the anterior diaphragm, pericardial defects and cardiac anomalies had been described as a syndrome by Cantrell et al. Developing mechanism of the syndrome was embryologically defined. These defects arise apparently from combined anomalies of developing of the dorsal mesoderm. The authors present two cases of Cantrell's syndrome developed in one day old and eight months old female patient in which angiocardiography was done at the Department of Radiology, Seoul National University Hospital.

3.Chest PA findings of leukemia in children

Leukemia is the commonest malignancy in childhood, accounting for about one-third of all malignancies in thosebelow the age of 15 years. Authors analysed the chest PA findings of 177 cases of leukemia in childhood below theage of 15 years, diagnosed at Seoul National University Hospital in recent 5 years since 1978. The summaries ofthe analysis are as follows; 1. Among 177 cases, 121 cases were male and 56 cases were female. The malepredominant than the female with the ratio of 2.2:1. In 177 cases, 77% of the patients were under the 10 years ofage. 2. The incidence of subtype of leukemia was as follows; The acute form was 158 cases (89%), acute lymphocyticleukemia, 55%, acut myelocytic group, 31.6%, chronic myelocytic type, 10%. And acute lymphocytic leukemia was61.4% of acute form. 3. The incidence of abnormality in chest PA was 32.2%(57 cases) . Among these 57 cases,abnormality in lung field was 35 cases (19.8% of total patients) hilar and mediastinal lymph nodes enlargement was19 cases(10.7%). Pleural effusion was 13 cases (7.3%) and cardiac shadow enlargement in chest PA was 3 cases.

Cardiac size of 291 new-born infants was measured using the method illustated on Fig 1. Among the 291 infants,53 were asphyxiated, and asphxia was only regared from Apgar score below 6 on 1 min. and 5 min. Remaining 238infants were normal, and classified to group with lung abnormalities and without lung abnormalities on chest A-Pfilm. The results are as follows; 1. The average CTR. of normal group was 52.37(C/T1;54.89, C/T2;49.43, C/T3;49.15, C/T;55.97) 2. The average CTR. of asphxiated group was 54.91 (C/T1;57.13, C/T2;51.69, C/T3;51.94,C/T4;58.25) 3. Consequently, asphyxiated infants revealed larger cardiac size than normal infant group.

6.Needs Assessment for Functionalities in Electronic Health Record Systems in General Hospitals.

OBJECTIVE: As an electronic health record system is implementing in Korean health care sectors, concerns about key functionalities of electronic health record systems are increasing. The purpose of this study was to identify core functions and set the priority in electronic health record systems under the Korean contexts in order to assure and improve the quality of the systems. METHODS: A survey was conducted using questionnaire developed by the study team based on literature review. The subjects were medical record administrators working at medical record department in general hospitals. RESULTS: The response rate was 59.8%(55/92). The functions which more than ninety percent of subjects responded as necessary right now and/or in near future related to 'drug alert', 'clinical guideline', 'chronic disease management', 'automated real-time surveillance', 'coded data', 'result reporting', 'de-identifying data', 'disease registry', and 'provider-provider communication and connectivity'. CONCLUSION: The results showed the high prioritized functions were decision support and health information/data management.

7.Health Behavior and Influencing Factors in Patients with Coronary Artery Disease Admitted to Hospital.

PURPOSE: The purpose of this study was to identify the impact that health motivation, stage of change and cardiac risk factors have on health behaviors in Korean patients with coronary artery disease(CAD) admitted to hospital. METHOD: The participants in this study were 127 patients with CAD hospitalized between May 2008 and July 2009. A structured questionnaire with personal interviews and chart reviews were used to collect data. Data were analyzed using the SPSS. RESULTS: Of the relatively significant factors, including a stage of change, self efficacy, modifiable risk factor score, and perceived barriers, stage of change was found to be the most significant predictor of health behavior in patients with CAD. These variables accounted for 48.2% of the variance in health behavior. CONCLUSION: The results of the study indicate the necessity of developing a cardiac rehabilitation program for use in their daily lives after discharge from the hospital. Stage of change should be assessed for all patients with CAD being discharged from acute care hospitals.

8.Double chambered right ventricle

Fourteen cases of double chambered right ventricle were diagnosed angiographically and of these nine caseswere confirmed after operation and autopsy at Seoul National University Hospital in recent four years since 1979.The clinical and radiological findings with the emphasis on the cinecardiographic findings were analysed. Thesummaries of the analysis are as follows; 1. Among 14 cases, 6 cases were male and 8 cases were female. Agedistribution was from 4 years to 36 years. 2. In chest X-ray findings, pulmonary vascularity was increased in 8cases, decreased in 4 cases, and nomral in 2 cases. Cardiomegaly was observed in 8 cases and other showed normalheart size. 3. In cinecardiography, 11 cases had interventricular septal defect. Among these 11 cases, VSD locatedin proximal high pressusre chamber was in 2 cases and located in distal low presssure chamber was in 9 cases. 4.The location of aberrant muscle bundle in sinus portion of right ventricule was in 8 cases. In the rest cases, andaberrant muscle bundle was located below the infundibulum of right ventricle. 5. For accurate diagnosis anddifferential diagnosis with other congenital cardiac anomalies such as Tetralogy of Fallot or isolated pulmonicstenosis, biplane cineangiography and catheterization is an essential procedure.

9.A radiologic analysis and comparative evaluation of hysterosalpingographic and laparoscopic findings ininfertile patients

Hysterosalpingograms of 375 patients with infertility were reviewed with reference to radiological features of each pathology and a comparative study of hysterosalpingography and laparoscopy in 36 patients was presented. The results were as follows; 1. In 375 cases infertility, 191 cases (50.9%) were primary infertility and 181 cases(49.1%) were secondary infertility. 2. The peak age distribution was 26 to 30 year old group in 211 cases (56.3%),and the peak duration of infertility was less than 3 years in 256 cases (68.3%). 3. Abnormal uterine findings were 115 cases (30.7%) and there were 73 cases of filling defects, 13 cases of irregularity , 12 cases of uterineflexion, 11 cases of congenital anomaly, and 9 cases of hypoplasia among them. 4. Abnormal tubal findings were 196 cases (52.3%) and there were 103 cases of simple obstruction, 69 cases of hydrosalpinx, and 50 cases of peritubal adhesion among them. 5. Intravasation of contrast media was noted in 42 cases (11.2%). 6. In 36 cases of laparoscopy, 23 cases (63.7%) was agreed to hysterosalpingography. 7. Aothough laparoscopy was better than hysterosalpinography in external anatomy of salpinx and relationship to overay, hysterosalpingography should be important for fundamental study of infertile women because of lower failure rate, lower risk of complications,therapeutic effect, and lower cost.

10.Factors affecting Retention Intention of Male Nurses Working Health Care Institution in Korea.

PURPOSE: The study was done to investigate male nurses' gender discrimination, person-organization fit, organization leader-member exchange, career plateau and retention intention and to identify factors affecting nurses' retention intention for these nurses. METHODS: A cross-sectional survey was conducted using self-report questionnaires. The participants were 144 male nurses working in hospitals and other health care facilities in Korea. Data were analyzed using hierarchial regression. RESULTS: The mean score was 5.40±1.61 (out of 8) for retention intention. Factors influencing retention intention were ‘married’ (β=.27, p<.001) and ‘employment career (3~5 yr)’ (β=-.24, p=.003) in model 1, ‘person-organization fit’ (β=.42, p<.001) in model 2, ‘content plateau’ (β=-.19, p=.020) in model 3. Person-organization fit was the most significant factor followed by content plateau, employment career and married state in that order. These factors explained 33.2% of the variance in retention intention (F=17.23, p<.001). CONCLUSION: Findings indicate that it is necessary to confirm that the male nurses are suitable for their organization and improve career development at 3~5 years of work experience as a retention strategy of male nurses.
