3.A Case of 13 Ring Chromosome Syndrome.

We have experienced a case of 13 ring chromosome in a 40-month-old girl who demonstrated psychomotor retardation with delayed speech, growth retardation, hearing loss(left), microcephaly, trigonocephaly with flat occiput, hypertelorism, epicanthal folds, microophthalmia, broad prominamt nasal bridge, high arched palate, micrognathia, large auricles and other anomalies. Cytogenetic studies of peripheral blood lymphocytes with differential staining of chromosomes revealed 46, XX, r13. Her parents' karyotypes were normal. We reported the case with the review of the associated literatures.

4.The Comparison of Tension-Free Vaginal Tape Procedure in the Management of Stress Urinary Incontinence Caused by Anatomical Incontinence and Intrinsic Sphincter Deficiency.

PURPOSE: Recently, tension free vaginal tape (TVT) has been widely used in the management of stress urinary incontinence (SUI), and has been applied to SUI by intrinsic sphincter deficiency (ISD). This study was performed to identify the efficacy of the TVT procedure in SUI patients by ISD. MATERIALS AND METHODS: A total of 196 incontinent women, who underwent TVT procedures were enrolled in this study. The patients were divided into four groups according to their preoperative Valsalva leak point pressure (VLPP) and maximum urethral closure pressure (MUCP): Group I (anatomical incontinence, AI, n=78), Group II (mild ISD: 40cmH2O < or=VLPP<60cmH2O, n=54), Group III (moderate ISD: MUCP< or=20cmH2O or 30cmH2O< or=VLPP<40cmH2O, n=38) and Group IV (severe ISD: MUCP< or= 10cmH2O or VLPP< or=30cmH2O, n=26). One year after the TVT procedure, a symptom questionnaire and the urodynamic parameters were compared with the preoperative data. RESULTS: There were no significant differences in the Q-tip test, peak uroflow (Qmax) and post-voiding residual volume among the four groups preoperatively. There were no significant differences in success rates between ISD and AI. The postoperative cure rates were 82% (64/78) and 78% (92/118) for AI and ISD, respectively. In an urodynamic analysis, Groups III and IV the postoperative Q max were significantly decreased compared with Groups I and II, and the postoperative voiding difficulties were significantly higher in Groups III and IV (p<0.05). CONCLUSIONS: The TVT procedure showed high cure rates of SUI in the mild ISD and AI groups compared with those in the moderate and severe ISD groups. The rates of voiding difficulty were more frequent in moderate and severe ISD groups. According to our results, it can be suggested that TVT can be undertaken for stress incontinent women associated with ISD.

5.Two Cases of Tufted Hair Folliculitis.

Tufted hair follliculitis is a localized, inflammatory and exudative disease of the scalp characterized by a tufted appearance of the scalp hairs emerging from single follicular openings, and may result in permanent and irreversible scarring alopecia. We report two cases of tufted hair folliculitis in a 53-year-old woman and a 47-year-old man. They had several areas of scarring alopecia with multiple bundles of hairs emerging from single follicular orifices. Histopathologic findings were typical for tufted hair folliculitis. The patients were treated successively with oral antibiotics and with topical application of clindamycin.

6.Can Postmortem Fetal MR Imaging Replace Autopsy?.

PURPOSE: The purposes of this study were to compare postmortem fetal MRI findings with autopsy findings and to assess whether postmortem MRI can replace autopsy. MATERIALS AND METHODS: The study group consisted of 13 stillborn fetuses, seven that died immediately after birth, and five terminated because of anomalies seen on prenatal sonograms. A total 17 were male, and eight were female, and their gestational ages were from 20 to 41 (average; 28.2) weeks. Spin-echo T1-and T2-weight-ed axial, sagittal, and coronal MR images were obtained, and autopsy findings were divided into major and mi-nor. A major finding was defined as an anomaly or syndrome which caused fetal death or termination of the pregnancy; minor findings were classified, on the basis of gross inspection, as internal or external. MR images were retrospectively analyzed by two radiologists unaware of the autopsy findings, and by comparison with these, the postmortem MRI detection rates for major and minor findings was then determined. RESULTS: In seven of 25 fetuses, MR imaging revealed major findings, a detection rate of 100%. There were two cases of anencephaly, two of trisomy-18, and one each of hydrops fetalis with large cystic hygroma, diaphrag-matic hernia, and Dandy-Walker malformation. Twenty-three of 60 minor findings (38.3%) were detected by MRI. The detection rates for external and internal findings were 29.6%(8/27) and 45.5%(15/33), respectively. CONCLUSION: Although a limitation of our study is the low detection rate for minor findings, postmortem fetal MRI may help diagnose the major cause of fetal death.

8.Ultrasound Guided Biopsy of Malignant Focal Liver Lesions: Comparison of Automated Gun Biopsy with Fine Needle Aspiration Biopsy.

PURPOSE: To compare the efficacy and safety of ultrasound guided automated gun biopsy with fine needle aspiration biopsy of focal liver lesions. MATERIALS AND METHODS: We performed 32 automated gun(19.5G Autovac Biopsy Needle) biopsies and 27 fine needle aspiration(22G Westcott Style Biopsy Needle) biopsies in 48 patients with malignant focal liver lesion under the guidance of ultrasound. We compared the "positive for malignancy rate" and "pathologic diagnosis rate" of both methods with final diagnosis, and the rate of complications. RESULTS: The "positive for maiignancy rate" of automated gun biopsy and needle aspiration are 84.4%(27/32) and 66.7%(18/27) respectively(p=0.11). The "pathologic diagnosis rate" are 84.4%(27/32) and 55.6%(15/27) with significant statistical difference(p<0.05). And there is no difference of the rates of significant complications. CONCLUSION: When compared with the fine needle aspiration biopsy, ultrasound-guided automated gun biopsy is safe and more effective method for malignant focal liver lesion.

9.Radiographic study of mandibular asymmetry.

The purpose of this study was to perform the radiographic measurements and temporomandibular joint evaluation in mandibular asymmetry. For this study, thirty-two patients who have mandibular asymmetry were selected and submentovertex, panoramic and lateral corrected tomographic radiagraphs were taken. Horizontal and vertical analysis using various landmarks on these radiographs were performed. Also radiographic and clinical evaluation of temporomandibular joint were obtained. Ther results were as follows ; 1. On the submentovertex radiograph, the mean distance of Pogonion to midline was 5.0(+/-)3.8mm. 2. The mean distance of Pogonion to Gonion between the deviated and the contra-lateral side(P<0.001). 3. The distance difference of Pogonion to Gonion between the deviated and the contra-lateral side was significantly realated to the degree of asymmetry(P<0.001). 4. On panoramic radiagraph,the condylar height of the contral-lateral side was significantly longer than the one of the deviated side(P<0.001). 5. On lateral corrected tomogram, bony of temporomandibular joint was observed in 11 condyles of the deviated side and 9 condyles of the contra-lateral side. Erosion and ostephyte were the most common changes in both the deviated and the contra-lateral sides.

10.Clinical Comparison of Neonatal Urinary Tract Infection Caused by Klebsiella pneumoniae Versus Non - klebsiella pneumoniae.

PURPOSE: The purpose of this study was to describe the clinical characteristics of neonatal urinary tract infection (UTI) caused by Klebsiella pneumoniae and non- Klebsiella pneumoniae UTI. METHODS: We compared clinical characteristics of 84 neonatal patients with UTI caused by Klebsiella pneumoniae who were hospitalized at the Department of Pediatricsat Han Dong University, Sunlin Hospital during the period between May, 1994 and August, 1998. The cases were divided into two groups depending upon causative organisms' Klebsiella pneumoniae UTI vs non-Klebsiella pneumoniae UTI, and the clinical characteristics of these groups were compared. RESULTS: Escherichia coli was the most common bacterial pathogen causing neonatal UTI, followed by Klebsiella pneumoniae. There was no significant difference in the sex distribution of Klebsiella pneumoniae UTI, but non-Klebsiella pneumoniae UTI showed male predominence. There were no significant differences in the incidences of hematologic, urologic, radiologic findings and perinatal complications in between these 2 groups. CONCLUSION: Klebsiella pneumoniae is the second most common pathogen causing neonatal UTI. There were no specific differences in the laboratory, symptomatologic, and radiologic findings in these two groups.
