2.Length determination of long bone by CT scanogram

The CT digital radiography is a new accurate technique for measuring the long bone length. Authors performed acomparative study on accuracy, time and film consumption and radiation dose between conventional spot scanogramand CT scanogram. We used two femur and two tibia specimen for materials, the Somatom II(Siemens) for CTscanogram, and Rad Check(Voctoreen) for radiation dosimetery. There was no significant difference in accuracybetween direct measurement, conventional scanogram and CT scanogram(lesser than 1% difference). The examinationtime of conventional scanogram was about 35 minutes, CT was 15 minutes, and the film consumption of spot scanogramwas 3 of 14x17'' size, but the CT scanogram need only one 8x10'' film for completion of study. The radiation doseof hip, knee and ankle joint were 220 mRad, 365 mRad, respectively, in spot scanogram, but it was lesser than 5mRad in all joints in CT scanogram. The advantages of the CT scanogram are simple, rapid and reduced radiation.

3.Nutritional Support for Successful Ventilator Weaning in Patients with Respiratory Insufficiency.

Consideration of the nutritional status of patients with acute respiratory failure and mechanical ventilation is important for effective patient assessment and management. Patients with acute respiratory failure are at a high risk for developing malnutrition. High glucose intakes given during the administration of total parenteral nutrition (TPN) have been demonstrated to increase CO2 production. The workload imposed by the high CO2 production may precipitate respiratory distress in patients experiencing compromised pulmonary function. The authors report a case of successful weaning from mechanical ventilation in patient with post-operative ARDS by proper nutritional support and by changing the proportion of calories derived from carbohydrates and fats. Substitution of fat calories for carbohydrate in TNA solutions can reduce CO2 production and help wean patients from mechanical ventilation. Conclusively, the TNA (total nutrients admixture) system is more beneficial to patients with acute respiratory failure due to less CO2 production and surfactant production than TPN. For patients with hypercapnia, providing 25% to 30% of calories as carbohydrate and 50% to 55% of calories from fat may be beneficial.

4.Meaning of TCO2 in the Diabetic and Non-diabetic Patients Undergoing Long-term Hemodialysis.

OBJECTIVE: Various degrees of metabolic acidosis are accompanied by the decrease in the kidney function. It is known that dialysis patients' long-term convalescences and complications are related to metabolic acidosis. It is generally known that the extreme acidosis of the dialysis patients should be corrected but on the contrary, there are reports on hemodialysis patients with slight acidosis have better nutritional condition. However, the research results are rare compared to the ratio of diabetic patients among dialysis patients. Therefore, in this paper we tried to reach a conclusion by comparing and analyzing the total carbon dioxide of the serum and other nutrition parameters of the diabetic patients among long-term hemodialysis patients with the non-diabetic patients. METHODS: We divided 50 patients, who have been hemodialysis for longer than three months, into 21 patients with diabetes and 29 patients with non-diabetes. And we compared and analyzed the TCO2 in addition to measuring other parameters based on the blood tests carried out on the beginning of every month from January of 1999 to December of 2000. RESULTS: TCO2 showed a inverse correlation with nPCR, serum albumin, blood urea nitrogen, and creatinine on the correlation analysis of the total target patients. And blood urea nitrogen and age were the independent factors in the independent factor analysis using multiple regression analysis. When we divided and compared the total target patients into diabetic patients and non-diabetic patients, there were no significant differences between various kinds of nutrition parameters and dialysis parameters. And TCO2 showed a inverse correlation with nPCR, nPCR being the independent factor in the diabetic patients. In addition, TCO2 showed a inverse correlation with serum creatinine, serum creatinine being the independent factor in the non-diabetic patients. CONCLUSION: Increase of ingestion of protein was one of the important factors in inducing metabolic acidosis, but the TCO2 and improved nutritional condition resulting from ingesting sufficient protein could be preserved in the diabetic patients. Therefore we think that the diabetic patients' nutritional condition and supervision of their ingestion of protein is more needed as sufficient ingestion of protein is a major factor in balancing acid-base for diabetic patients than non-diabetic patients.

5.Clinicopathological Features and Survival of Patients with Gastric Cancer with a Family History: a Large Analysis of 2,736 Patients with Gastric Cancer.

PURPOSE: Previous studies indicated conflicting results regarding the prognosis of gastric cancer with a family history (FHX). This study aimed to determine the clinicopathological features and survival of patients with gastric cancer with a FHX. MATERIALS AND METHODS: We reviewed 2,736 patients with gastric cancer who underwent surgery between 2003 and 2009. The prognostic value of a FHX was determined in the multivariate model after adjusting for variables in the Asian and internationally validated prognostic models. RESULTS: Of the patients, 413 (15.1%) had a FHX of gastric cancer. The patients with a FHX were younger (58.1 vs. 60.4 years; P<0.001) than the patients without a FHX. There were no significant differences in the histopathological characteristics between the 2 groups. A FHX was associated with a better overall survival (OS) rate only in the stage I group (5-year survival rate, 95% vs. 92%; P=0.006). However, the disease-specific survival (DSS) rate was not significantly different between the 2 groups in all stages. The multivariate model adjusted for the variables in the Asian and internationally validated prognostic models revealed that FHX has no significant prognostic value for OS and DSS. CONCLUSIONS: The clinicopathological features and survival of the patients with gastric cancer with a FHX did not significantly differ from those of the patients without a FHX.

6.Fracture toughness of self-curing denture base resins with different polymerizing conditions.

PURPOSE: The intent of this study was to evaluate the effects of curing conditions on selfcuring denture base resins to find out proper condition in self-curing resin polymerization. MATERIALS AND METHODS: In this study, 3 commercial self-curing denture base resins are used: Vertex SC, Tokuso Rebase and Jet Denture Repair Acrylic. After mixing the self curing resin, it was placed in a stainless steel mold(3x6x60mm). The mold containing the resin was placed under the following conditions- in air at 23 degrees C, or in water at 23degrees C; or in water at 23degrees C under pressure(20psi); or in water at 37degrees C under pressure(20psi); or in water at 50degrees C under pressure(20psi); or in water at 65degrees C under pressure(20psi), respectively. Also heatcuring denture base resin is polymerized according to manufacturers' instructions as control. Fracture toughness was measured by a single edge notched beam(SENB) method. Notch about 3mm deep was carved at the center of the long axis of the specimen using a dental diamond disk driven by a dental micro engine. The flexural test was carried out at a crosshead speed 0.5mm/min and fracture surface were observed under measuring microscope. RESULTS AND CONCLUSION: The results obtained were summarized as follows : 1. The fracture toughness value of self-curing denture base resins were relatively lower than that of heat-curing denture base resin. 2. In Vertex SC and Jet Denture Repair Acrylic, higher fracture toughness value was observed in the curing environment with pressure but in Tokuso Rebase, low fracture toughness value was observed but there was no statistical difference. 3. Higher fracture toughness value was observed in the curing environment with water than air but there was no statistical difference. 4. Raising the temperature in water showed the increase of fracture toughness.

7.Spermatic Cord Leiomyoma.

A spermatic cord leiomyoma is an uncommon disease. A leiomyoma can originate anywhere, including the genitourinary system, where smooth muscle is present. 70 and 30% of spermatic cord leiomyomas are benign and malignant, respectively. The preoperative diagnosis of a leiomyoma is difficult; the only definite diagnostic method is histological confirmation. A retroperitoneal lipoma probably constitutes the largest group of solid abdominal tumors. However, their etiology is unknown and the symptoms insidious, and recurrences and sarcomatous changes frequently occur. The clinical diagnostic methods are the clinical symptoms, a physical examination and radiological evaluations, such as intravenous urography, computed tomography and angiography, and so on, but an abdominal CT scan provides the most useful information. A fifty-six-old man presented with a five-month history of a suddenly enlarging right inguinal mass. He had undergone a right inguinal hernia operation, with medication for hypertension. Herein, a rare case of a huge retroperitoneal lipoma, with a spermatic cord leiomyoma, is reported.

8.Laparoscopic Total Gastrectomy in Elderly Patients (> or =70 Years) with Gastric Carcinoma: A Retrospective Study.

PURPOSE: To compare the surgical outcomes of laparoscopic total gastrectomy between elderly and non-elderly patients. MATERIALS AND METHODS: Between 2008 and 2015, a total of 273 patients undergoing laparoscopic total gastrectomy for gastric carcinoma were divided into two age groups: elderly (> or =70 years, n=71) vs. non-elderly (<70 years, n=172). Postoperative outcomes, including length of hospital stay, morbidity, and mortality were compared between the groups. RESULTS: The elderly group showed a significantly higher rate of comorbidities and American Society of Anesthesiologists scores than those in the non-elderly group. No significant differences were found with respect to lymphadenectomy or combined organ resection between the groups. After surgery, the elderly group showed a significantly higher incidence of grade III and above complications than the non-elderly group (15.5% vs. 4.1%, P=0.003). Among the complications, anastomosis leakage was significantly more common in the elderly group (9.9% vs. 2.9%, P=0.044). Univariate and multivariate analyses showed that old age (> or =70 years) was an independent risk factor (odds ratio=4.42, 95% confidence interval=1.50~13.01) for postoperative complications of grade III and above. CONCLUSIONS: Elderly patients are more vulnerable to grade III and above complications after laparoscopic total gastrectomy than non-elderly patients. Great care should be taken to prevent and monitor the development of anastomosis leakage in elderly patients after laparoscopic total gastrectomy.

9.Real-Time Video-Assisted Retrieval of Airway Foreign Body in Very Young Pediatric Patients.

OBJECTIVES: Advancements in medical endoscopy and techniques of rigid bronchoscopy for foreign body removal have enabled higher diagnostic accuracy, reduced morbidity and precise manipulation. However, in pediatric patients, endoscope-combined forceps may be too big to fit into the small sized airway. Here we present our method of endoscope assisted rigid bronchoscopy in pediatric patients and compare the clinical benefits with conventional naked-eye rigid bronchoscopy. METHODS: We used a 2.7 mm, 0degrees straight endoscope and small caliber grasping forceps with 3.0 to 4.5 mm sized rigid bronchoscopy for very young (<3 years of age) patients of foreign body aspiration. As an assistant held the rigid bronchoscope in situ, the operator could manipulate the endoscope and forceps bimanually. With endoscopic guidance, the foreign body retrieval was performed carefully. The clinical advantages were compared between our endoscope-assisted method (n=29) and the conventional bronchoscopy method (n=33) in terms of operation time and recovery (hospital stay). RESULTS: Bimanual endoscope-assisted rigid bronchoscopy method was technically feasible and safe. The operation time was less, compared to the conventional technique and the patients recovered more quickly. In all cases, our method completely removed the foreign body without need of a second bronchoscopy procedure. CONCLUSION: Bimanual endoscope-assisted retrieval of airway foreign body in very young age pediatric patients was superior to the conventional naked-eye method concerning accurate manipulation and safety.

10.Study of endodontic working length of Korean posterior teeth.

OBJECTIVES: The aim of this study was to investigate average working lengths of Korean posterior teeth and evaluate validity of endodontic file length. MATERIALS AND METHODS: The endodontic working length of the posterior teeth of 670 Korean patients were measured than each mean value and standard deviation were investigated than the frequency deviation and standard deviation per each length were calculated. RESULTS: Among the canals of premolar, 66.5% of canal length was marked under 20 mm by endodontic working length and 95.4% could be measured under 22 mm and Among the canals of molars, 95.5% of canal length was marked under 20 mm endodontic working length. CONCLUSIONS: With the result of measurement of endodontic working length of premolars of Korean, it suggested that 23 mm endodontic file is more proper than the 21 mm and 25 mm file on the market.
