CT is a highly accurate method of detecting and clarifying the narure of space-occupying lesions within theliver nd pancreatic disease. It was found to be a reliable, non invasive method for detecting the lesion. A normalrange of attenuation values were obtained from 71 CT examinations of the upper abdomen, that is liver, spleen,pancreas, aorta, before and after contrast enhancement in whom no radiologic or laboratory abnormality of theseorgans were detected from March to Sept. 1983. The results were as follows; 1. The age distribution was from 28years to 71 years. The sex ratio was 45 male to 26 female. 2. Mean Hounsfield Units (HU) in liver before contrastenhancement and after contrast enhancement liminute, 3 minutes, 5 minutes, 10 minutes were 56.1±3.85, 75.0±4.77,82.0±3.71, 84.3±2.76 and 88.2±3.95 HU respectively. 3. Mean Hounsfield Units in pancreas before contrastenhancement and after contrast enhancement 1 minute, 3 minutes, 5 minutes, 10 minutes were 43.0±4.30, 66.5±4.27,71.2±2.39, 74.4±3.00 and 79.2±2.(0 HU respectively. 4. Mean Hounsfield Units in spleen before contrastenhancement and after contrast enhancement 1 minute, 3 minutes, 5 minutes 10 minutes were 53.0±4.31, 71.4±2.84,75.3±2.70, 80.5±3.04 and 83.2±2.14 HU respectively. 5. Mean Housfield Units in aorta before contrastenhancement and after contrast enhancement 1 minute, 3 minutes, 5 minutes 10 minutes were 45.1±5.39, 86.7±6.86,92.5±5.33, 106.8±4.12 and 114.1±5.02 HU respectively. 6. The most significant finding was that the livernormaly had the highest attenuation values of any of the organs measured. When another viscera in upper abdomenhad a attenuation value greater than that of the liver, this reflected abnormal lesions.

The purpose of this study was to investigate the displacements of cast framework by torch soldering and electric soldering techniques. Specimen had two cylinders and connecting bar that had sectioned with 0.3mm gap at mid point. 10 of total specimens were divided into two groups. In torch soldered group, soldering investment block was made and conventional torch soldering procedure was carried out. In electric soldered group, electric soldering was carried out on the master cast without soldering investment block by using electric soldering machine(Dentapunkt DP 7, Kulzer, Germany). After soldering procedure, three dimensional coordinates of two centroids of each cylinder were measured by three dimensional coordinate measuring machine. The intercentroidal displacement and global displacement were calculated and then, these values were compared and evaluated. The results were obtained as follows: 1. Intercentroidal distances of specimens decreased after both soldering procedures, and the decrease in intercentroidal distance was greater for torch soldered group than for electric soldered group 2. PURPOSE: We analyzed the clinical outcome of osteosarcoma developed in distal radius and the effect of delayed treatment on prognosis. MATERIALS AND METHODS: Twelve patients with distal radius osteosarcoma were analysed. We categorized patients into two groups of standard treatment or non-standard treatment. The patients of standard treatment group are all stage IIB and non-standard treatment group includes five stage IIB and one stage III. RESULTS: Five-year overall survival and disease-free survival rates of standard treatment group were 100% and 83%. Five-year overall survival rate of non-standard treatment group was 44%. Between two group, there are differences in age, tumor size, surgery type, symptom duration. CONCLUSION: Distal radius osteosarcoma have good prognosis than other extremity osteosarcoma. Survival rate of non-standard treatment group were lower than standard treatment group. Although the prognosis of non standard treatment group is poorer, the duration till death was longer than that of other sites with similar condition. Further multi-institutional study should be needed.

Coronary spasm may be induced by a variety of physiologic and pharmacologic stimuli but specific receptor blockade has not been consistently shown to prevent the attacks. Most patients with variant angina respond well to treatment with calcium antagonists and nitrates. A small proportion of patients are refractory to this therapy. We report a case of the patient with a 9-year-history of variant angina who has been refractory to high doses of calcium antagonists and nitrates. The repeated addition of clonidine was consistently effective in abolishing both symptoms and objective evidence of myocardial ischemia in this particular patient.

BACKGROUND: Because systolic and diastolic dysfunction frequently coexist in acute myocardial infarction(AMI), we hypothesize that a combined measure of ventricular performance using Doppler echocardiography may be more sensitive and time-saving diagnostic tool for the evaluation of patients presenting with cardiogenic chest pain. METHOD AND RESULTS: Seventy-one patients with AMI (47 male, 59+/-11 years) and 45 patients with normal coronary artery (29 male, 52+/-11 years) were included in the study for measurement of cardiac performance index and established parameters of ventricular function using conventional echo-Doppler methods. a new derived index of cardiac performance: (ICT+IRT)/ET, was obtained by subtracting ejection time(ET) from the interval between cessation and onset of the mitral inflow velocity to give the sum of isovolumic contraction time(ICT) and isovolumic relaxation time(IRT). The mean value of the index was significantly different between normal and AMI(p<0.01). The degree of inter-group overlap was smaller for the index compared to other parameters. within functional groups, the value of the index did not appear to be related to heart rate, mean arterial pressure and the degree of mitral regurgitation. CONCLUSION: cardiac performance index is a conceptually new, simple and reproducible Doppler index of combined systolic and diastolic myocardial performance, and it is useful as screening test for patients with cardiac dysfunction due to AMI.

Prostate intraepithelial neoplasia (PIN) is a putative premalignant change in the human prostate, which is an intraluminal proliferation of the secretory cells of the prostatic duct-acinar system that is enveloped by a basal cell layer and displays a spectrum of dysplastic cytologic features ranging from minimal atypia (low grade PIN) to those which are ultimately indistinguishable from carcinoma cells (high grade PIN). To evaluate the clinical significance of the PIN in prostatic tumor and BPH, we reviewed the serum prostate specific antigen (PSA), prostate specific antigen density (PSAD), and pathologic findings in the specimen of 21 BPH and 11 Prostate cancers, which were pathologically confirmed. The distributions of PIN are 7/21 (33%) in BPH and 8/ 11 (73%) in prostatic ca (P<0.05). The mean value (+/-SD) of PSA and PSAD in BPH patient were 8.42+/-5.57 ng/ml, 0.16+/-0.09 for PIN(-), 10.13+/-5.97 ng/ml, 0.17+/-0.09 for PIN(+), and in prostatic cancer patient were 60.53+/-1.83 ng/ml, 1.42+/-0.25 for PIN(-), 54.15+/-34.61 ng/ml, 1.28+/-0.84 for PIN(+), respectively. The mean value (+/-SD) of PSA & PSAD according to histologic types of BPH were 9.04+/-3.88 ng/ml, 0.17+/-0.06 for glandular type, 5. 57+/-1.31 ng/ml, 0.10+/-0.03 for stromal type, and 11.18+/-8.93 ng/ml, 0.19+/-0.13 for mixed type. The distributions of PIN according to histologic types of BPH were 30% (3/10) for glandular type, 40% (2/5) for stromal type, and 33% (2/6) for mixed type. All 7 PIN(+) BPH were low grade, while, of the 8 PIN(+) prostatic Ca, 1 was low grade and 7 were high grade. From these results, the frequent of PIN was higher in prostatic cancer than BPH (P<0.05). PIN had no significant influence on PSA elevation in prostatic cancer and BPH. There was no correction between PSA, PSAD and histologic types of BPH (P>0.05). There was no significant difference in the distribution of PIN according to histologic types of BPH. And high grade PIN was observed only in prostatic cancer. Therefore, if high grade PIN is observed in pathologic specimens, undetected prostatic cancer should be found.

Paragonimiasis is prevalent in Far East and a kind of endemic Korean disases. The primary site of humanparagonimiasis is the lung, but hte ectopic infection of lung fluke has been reported by many authors. Weexperienced one case of abdominal paragonimiasis in a 44 year old male with a complaint of left lowerquadrantpain. Physical examination, barium enema, and other data suggested the possibility of tumor originating from thewall of descending colon. (intramural tumor). Postoperative specimen taken from the solid tumorous lesion showedparasitic granuloma, characteristic of Paragonimus Westermani. Here, we report a case of very peculiar ectopicparagonimaisis involved descending colon wall simulating neoplastic tumor, and also review the literaturesbriefly.

The goal of fracture treatment is to restore the patient to pre-injury function level. But the outcome assessment after orthopedic interventions has usually focused on physician-defined parameters of technical success, such as fracture union, alignment and range of motion rather than patient function and quality of life. However, the correlation among improvements in these parameters and functional status, psychosocial well being is inconsistent and weak in the elderly. Therefore authors assessed the functional status after surgical intervention of hip fractures in the elderly with functional recovery score(FRS). Independence in basic activities of daily living, in instrumental activities of daily living, in mobility, freedom from pain and intact memory were assessed in 133 hip fractures in the elderly. The hip fractures in the elderly resulted in 24.8% loss of function after the first year. The older the age, the more the loss of function after surgery of hip fractures. The more functional loss was observed in female patients and in patients with poor postoperative radiological results, but statistically insignificant (P>0.05). The more functional loss was observed in patients with underlying medical disease preoperatively and it was statistically significant (P<0.05). We recommend the evaluation of functional status in the elderly after hip fractures.

Coronary artery steonosis can be effectively treated by non-operative percutaneous tansluminal coronaryangioplasty(PTCA). We performed PTCA in three patients with coronary artery stenosis, who were referred to thedepartment of radiology, from January to Dec. in 1985. The results were as follows; 1. The locations of coronaryartery stenosis were the proximal anterior descending branch of left coronary artery. 2. The number of stenoticlesions was single without calcium plaque in all three cases. 3. The extent of coronary artery disease is focal,under 2cm in length. 4. PTCA was performed successfully with satisfactory post-dilatation results in all cases. 5.Severe complications such as arterial intimal dissection, acute myocardial infarction, did not occurred. PTCA hasmany advantages over vascular surgery on surgical, economical, and psychological aspects.
