2.Comparison of usefulness in the variable standards of waist circumference measurement.

BACKGROUND: Waist circumferences are widely used to diagnosis and assessment of obesity but various standards of the measuring waist circumference originate the confusion on diagnosis and assessment of obesity. According to this, this study is willing to light up standards of measuring waist circumference what is intimate with body mass index(BMI) and have a precision of intra observer and inter observers in repeated measurements of the waist. METHODS: Measuring of waist circumference by the 4 different standards(anterior superior iliac crest(ASIC), umbilicus, above 3 cm to ASIC, the thinnest area in the waist by the range of seeing) were performed on 102 people visiting Health Promotion Center of one medical collage hospital by two observers in each 2 times, total 16 times. Each data were analyzed by the relativity between the BMI and the each means of measuring waist circumference and by ANOVA test in intra observer and inter observer bias. RESULTS: The relation between BMI and data in measuring standard of ASIC of the waist circumference is the highest. (r=0.900) and the following umbilicus(r=0.896), above 3 cm to the anterior superior iliac crest(r=0.888), the thinnest area in the waist by the range of seeing(r=0.877), It is the lowest in intra observer bias that the thinnest area in the waist by the range of seeing, and ASIC in inter observer. CONCLUSION: It is desirable that ASIC is the standard of measuring waist circumference in the diagnosis of obesity and in measuring that by different two person, because of the highest relativity in BMI and the lowest inter observers bias. But the thinnest area in the waist by the range of seeing is desirable in measuring by same person because of intra observer bias.

3.Echocardiographic Differences between Hemodialysis and Essential Hypertension Patients and the Correlations with Factors Affecting the Differences.

To compare the differences between hemodialysis and essential hypertension patients and its affecting factors of left ventricular hypertrophy and left ventricular systolic dysfucntion in patients with hemodialysis, M-mode and two dimensional echocardiography were performed in 77 essential hypertension without azotemia and 78 chronic renal failure patients receiving maintenance hemodialysis. M-mode measurement including LV mass (192.56+/-63.6g vs 300.01+/-95.99g, P=0.000), r/th (radius/LV thickness, 4.41+/-0.97 vs 4.74+/-1.0, P=0.039), LV dimemsion and fractional shortening (4.68+/-0.6 vs 5.63+/-0.97, P=0.000, 30.0+/-19.7% vs 36.6+/-97%, P=0.000 respectively) showed more severe eccentric LV hypertrophy and LV dysfunction in patients with hemodialysis than those of essential hypertension. Using Pearson correlation in hemodialysis patients, Interdialytic weight gain was positively correlated with LVEDD (r=0.318, P=0.005). In addition to the determinant, serum PTH level was negatively (r=-0.344, P=0.002) and Kt/V (r= 0.0487, P=0.003) was positively correalated with systolic function. The hypertension and dialysis duration, patient's age, had no relationship with LV function and mass in this study. In Conclusion, LV hypertrophy and LV systolic dysfunction occur more frequently in hemodialysis patients than in essential hypertension patients. And the LV systolic dysfunction, which is acutally related with the patient's quality of life, was partially explained by serum parathyroid level and Kt/V. But additional laboratory and prospective clinical studies are needed to further elucidate the mechanisms involved in the development of LVH and LV impairment in hemodialysis patients.

4.Characteristics of 18F fluorodeoxyglucose Uptake in Human Colon Cancer Cells.

Cancer tissues are characterized by increased glucose uptake. 18F-fluorodeoxyglucose(FDG), a glucose analogue is used for the diagnosis of cancer in PET studies. This study was aimed to compare the glucose uptake and glucose transporter l(GLUT1) expression in various human colon cancer cells. We measured FDG uptake by cell retention study and expression of GLUTI using Western blotting. Human colon cancer cells, SNU-C2A, SNU-C4 and SNU-C5, were used. The cells were incubated with 1micro Ci/ml of FDG in HEPES-buffered saline for one hour. The FDG uptake of SNU-C2A,SNU-C4 and SNU-C5 were 16.8+/-1.36, 12.3+/-5.55 and 61.0+/-2.17cpm/microgram of protein, respectively. Dose-response and time-course studies represent that FDG uptake of cancer cells were dose dependent and time dependent. The rate of FDG uptake of SNU-C2A, SNU-C4 and SNU-C5 were 0.29+/-0.03, 0.21+/-0.09 and 1.07+/-0.07cpm/min/microgram of protein, respectively. Western blot analysis showed that the GLUT1 expression of SNU-C5 was significantly higher than those of SNU-C2A and SNU-C4. These results represent that FDG uptake into human colon cancer cells are different from each other. In addition, FDG uptake and expression of CLUT1 are closely related in human colon cancer cells.

6.The Therapeutic Effects of Adenosine Triphosphate(ATP) on Paroxysmal Supraventricular Tachycardia : Based on Analysis of Electrocardiograms.

BACKGROUND: ATP is an endogenous nucleotide that has potential electrophysiologic effects. The effects of ATP are largely mediated by its metabolite, adenosine, which acts on A1 receptor in myocardium and mediates negative chronotropic, dromotropic, and inotropic action. The purpose of this study is to determine the efficacy and side effects of ATP in the treatment of paroxymal supraventricular tachycardia(PSVT). METHODS: We analyzed 28 episodes of PSVT in 22 patients(7 men and 15 women, mean age 467.+/-15.9 years), who were selected from Jan. 1992 to Sep. 1992. After measuring blood pressure and obtaining a standard 12 leads electrocardiogram(ECG), we rapidly injected ATP intravenously with dose of 10, 15, 20mg every 1 min, until the termination of PSVT. We recorded peri-injection and post-injection ECG, interval between ATP injection and the termination of PSVT, the pause after termination, side effects and blood pressure immediately after termination. RESULTS: 1) Among 28 episodes of PSVE, 26 episodes(92.9%) were terminated successfully : 21 episodes(75%) with 10mg, 3 episodes(10.7%) with 15mg, and 2 episodes(7.2%) with 20mg of ATP. 2 episodes(7.1%) were failed. 2) The PSVT ended 21.2+/-9.2 seconds after injection of ATP(maximally 40 seconds). 3) The mechanisms of PSVT in 9 patients were atrioventricular nodal reentrant tachycardia(AVNRT), those in 13 patients were atrioventricular reciprocating tachycardia(AVRT). 4) The side effects of ATP included 13 episodes of chest pain(46.6%), 9 episodes of ventricular arrhythmia(32.1%), 6 episodes of sinus pause(21.4%) and 5 episodes of atrioventricular block(17.9%), and these were uncomplicated and short-lasting. CONCLUSION: The ARP is a safe and effective therapy for the conversion of PSVT to sinus rhythm and perhaps more importantly, a valuable new approach to the management and diagnosis of wide-complex tachycardia.

8.The Protective Effect of Calcium Antagonist on Myocardium in Coronary Reperfusion Following Experimental Myocardial Infarction.

Although it has been suggested that the calcium antagonist verapamil has beneficial effects on ischemic myocardium, its effect during coronary reperfusion has not been studied in detail. The purpose of this study was to investigate the inhibitory effect of verapamil on myocardial damage quantitatively using 111 In-anticardiac myosin antibody (ACM Ab) and qualitatively using electronmicroscopic method. Anesthetized open-chest dogs were subjected to 1 hour of occlusion of the left anterior descending coronary artery (LAD) followed by 90 minutes of reperfusion. Regional myocardial blood flow was determined by injecting 85Sr-microsphere prior to LAD reperfusion, and regional myocardial damage was measured by injecting 111In-ACm Ab at 30 minutes after LAD reperfusion. Six dogs were randomly selected as saline control and verapamil-treated (0.6 mg/kg. hr) groups each. Saline or verapamil was infused at 40 minutes after LAD occlusion and continued through the experiment. 1) Verapamil produced significant (P<0.05 by Wilcoxon rank sum test) decrease in heart rate, mean arterial blood pressure and double product. There was no significant change in pulmonary hemodynamics or cardiac output. 2) Stroke volume was reduced significantly (P<0.05 by Wilcoxon rank sum test) after 30 minutes of LAD reperfusion in the control group, but it was preserved in the verapamil-treated group. 3) There was an inverse exponential relationship between 111In-ACm Ab localization and regional blood flow in both control (r=-0.86) and verapamil treated (r=-0.71) groups. Significant difference between the two groups was found in exponential curve (p[t]<0.05). 4) A lesser uptake of 111in-ACM Ab was observed in the verapamil treated group compared with that in the control group in the region where the regional blood flow was lower than 30+/- of normal. 5) In the control group, the myocardium showed swelling, contraction bands, and electron dense granules in the mitochondria which were proven to be calcium aggregates. In the verapamiltreated grooup, the myocardium showed fewer electro dense granules and mild degree of contraction bands. This study supports the concept that verapamil reduces the myocardial damage following coronary reperfusion in myocardial infarction and may reduce contraction band necrosis.

9.A Case of Spontaneous Hemothorax Due to Rupture of Pseudoaneurysm in Type 1 Neurofibromatosis.

A non-traumatic, spontaneous hemothorax is rare. The most common causes are coagulopathy, due to anticoagulation treatment, and cancers with a metastasis to the pleural surface. Other unusual causes include thoracic endometriosis, ruptured aortic aneurysm, pulmonary arterio-venous malformation, coagulopathy, Osler-Rendeu-Weber syndrome, Ehlers-Danlos syndrome et cetera. A type 1 neurofibromatosis(Von Recklinghausen's disease) is an autosomal dominant disease that is characterized by multiple skin tumors(neurofibroma) and abnormal skin pigmentation(cafe-au-lait spots). Some are accompanied by vasculopathy, and are present with a spontaneous hemothorax. Such cases are unusual but fatal. We have recently experienced a case where a young male patient with neurofibromatosis initially presented with hypovolemic shock due to a spontaneous hemothorax. Later, aortography revealed that the cause of the hemothorax was a rupture of a pseudoaneurysm of the right internal mammary artery and as a result, an embolization was performed. Here we report this case with a review of the appropriate literature.

10.Cytologic Evaluation of CellPrep(R) Liquid-based Cytology in Cervicovaginal, Body Fluid, and Urine Specimens: Comparison with ThinPrep(R).

This study purposed to evaluate a CellPrep(R) (CP) of liquid-based cytology (LBC) to search for a less expensive and automated alternative cytologic preparation technique applicable to usually encountered cytologic specimens. Cervicovaginal direct-to-vial split samples from 457 gynecologic patients, 40 body fluid samples, and 34 urine samples were processed with the CP technique and the results were compared with those of currently used ThinPrep(R) (TP) method. Both CP and TP methods provide evenly distributed thin layers of cells with little cellular overlaps or significant obscuring elements in most of cases. Staining quality of both preparations showed a little difference due to the difference of fixative solutions without significant distractions in cytologic interpretation. On the supposition that TP was a gold standard, sensitivity, specificity, positive predictive value, and negative predictive value of CP cytology were 89%, 98%, 86%, and 99% in the cervicovaginal smear, 89%, 82%, 80%, and 90% in body fluid, and all of these values were 100% in urine samples. To testify the availability of immunohistochemistry on CP preparations, cytokeratin, vimentin, and Ki-67 were applied on body fluid specimens, and all of these antibodies were specifically stained on targeted cells. Conclusively, the CP method gave comparable results to those of TP in terms of smear quality and cytologic diagnostic evaluation, and was available on immunohistochemistry. The CP method could offer a cost-effective and automated alternative to the current expensive techniques of liquid- based cytology on popular cytologic materials including cervicovaginal, body fluid, and urine specimens.
