1.EFFECTS OF AGING AND THERMAL CYCLING ON THE BIAXIAL RLEXURE STRENGTH OF VENEERING RESIN COMPOSITES FOR CROWN.

This study was performed to evaluate the effect of aging and thermal cycling on the biaxial flexure strength of four commercially available veneering resin composites for crown(Dentacolor : DC, Artglass : AG, Esternia : ET and Targis : TG). Disc specimens were fabricated in a teflon mold giving 12mm in diameter and 1mm in thickness. All samples were divided into 4groups. Group 1 was dried in a dessicator at 25degrees C for 30 days. Group 2 was immersed in distilled water at 37degrees C for 30 days. Group 3 was immersed in distilled water at 65degrees C for 30 days. Group 4 was subjected to 10,000 thermal cycles between 5degrees C and 55degrees C, and the immersion time in each bath was 15 seconds per cycle. Biaxial flexure test was conducted using the ball-on-three-ball method at the cross head speed of 0.5mm/min and fracture surfaces were observed with scanning electoron microscope. The results obtained were summarized as follows ; 1. Weibull modulus values, except for the AG group, decreased after thermal cycling treatment. 2. Biaxial flexure strength values of aging group at 37degrees C were th lowest in all sample groups. Except for the DC group, strength values were significntly decreased for the drying group. 3. After thermal cycling test, the highest value of biaxial flexure strength of 188.8 MPa was observed in the ET group and the lowest value of 73.2 MPa was observed in the DC group. The strength values showed the significant differences in each group (p<0.05). 4. Observation of surfaces after thermal cycling test revealed the ditching in the part f surrounding large fillers.

3.Isolated Dislocation of Carpal Scaphoid: A Case Report.

Case histories of isolated and complete volar dislocation of the carpal scaphoid have rarely been reported .We report here another such case. On examination, the wrist was moderately swollen and tender over its volar aspect. Radiographs showed isolated, complete volar dislocation of the scaphoid. We used open reduction with K-wire fixation as treatment. After four weeks of operative reduction the patient had undertaken neurolysis due to median nerve compression symptom (carpal tunnel syndrome). At six weeks, the K-wires were removed and the patient was allowed to do a range of motion exercise. Twenty months after injury, the wrist was asymptomatic and had a mid-range of active motion without instability. There was no roentgenographic evidences of scapholunate dissociation or avascular necrosis

4.Effect of the Long-Term Therapy with Felodipine(Munobal(R)) in Patients with Essential Hypertension : With Reference to Persistence of Its Antihypertensive Effect and Safety.

BACKGROUND: Hypertension is a major and growing public health problem and it calls for life-long management. The antihypertensive agents are still playing major roles in the management of hypertension. We studied the effect of the long-term therapy with a new calcium channel blocker, felodipine in patients with essential hypertension. METHODS: We recruited 31 patients with mild to moderate essential hypertention and gave them 5mg (1 tablet) of felodipine(Munobal(R)) once daily for 12 months. We measured the blood pressure and the heart rate every month and tried to detect any untoward symptoms and signs during the whole observation period. Baseline work-up for the hypertensive patients were done as usual before and after the study. RESULTS: The average systolic and diastolic blood pressures were 162.7+/-9.7/99.6+/-7.4mmHg respectively before the administration and they came down to the level of 139.7+/-15.0/87.3+/-8.6mmHg respectively at the end of the 1st month (p<0.05). This trend was maintained during the whole observation period, so they were 141.2+/-11.0/85.6+/-8.2mmHg respectively at the end of the study (p<0.05). Untoward drug reactions such as gastrointestinal troubles, facial edema, leg edema, chest pain and headache were present in 11 patients (35%) but they were not serious. There was no case in which the drug was withdrawn. There was one case in which the level of the serum triglyceride increased after the administration of the drug. CONCLUSION: Felodipine has good and persistent hypotensive effect and it is safe and easy to use.

This study was performed to examine the effect of a 7-week comprehensive health promotion program for improving pain, depression, and disability by employing a quasi-experimental design. The subjects were regular out-patients of a RA aclinic in an University Hospital in Inchon from November 11. 1998 to December 24. 1998. The Experimental group included 18 patients who participated in an arthritis health promotion program, and the control group included 18 patients who did not. The 7-week health promotion program, which had the objective to enhance health promoting skills, was provided to patients. The effects of this program on the patients' pain, depression, and functional disability were examined. According to the study results, a significant group difference was found on these dependent variables (Hotelling's T =.30, F=3.11, p=.04). To examine which dependent variables had significant effects, one-way ANOVAs were performed. There were significant group differences in pain (F=4.35, p=.05) and in depression (F=4.22, p=.05) However, no significant group differences on functional disability (F=.04, p=.84) were found. Conclusively, the arthritis health promotion program, which was designed to enhance 11 health promoting skills, can be evaluated as successfully achieving the ultimate goal of enhancing the patients' quality of life. It can also be contended that the improvement of the patients' quality of life was enabled by relieving pain and reducing depression.

7.Clinical Study on Intrauterine Fetal Growth Restriction.

This study was undertaken at the department of Obstetrics and Gynecology, Chonnam University Medical School, to investigate the association between some of the risk factor and the incidence of intrauterine fetal growth restriction(IUGR). The studied population was selected from patients who admitted at Chonnam University Hospital during January, 1992 through May, 1997, with following criteria, Korean, singletone pregnancy with live birth and known gestational weeks with 28 or more. And then, the risk factors were analyzed in terms of maternal factor, placental factor, and fetal factor. The following results were obtained. 1) The incidence of IUGR was 6.1%. 2) The incidence of IUGR was higher at young aged mother and nullipara. 3) Only 39.1% of etiologic factors for IUGR was found to have known causes. According to the risk factors for IUGR, hypertensive disorder during pregnancy, anemia, cardiac disease, leukemia, and pulmonary tuberculosis were associated with increased incidence of IUGR. 4) The relative risk of IUGR was much higher in neonates born with congenital anomalies. 5) According to the placental causes of IUGR, placenta previa and placenta abruption showed some association with IUGR.

8.Adverse Effect of Human Hydrosalpingeal Fluid on the Development of Mouse Embryo (II).

In our previous study, we observed that hydrosalpingeal fluid (HSF) adversely effect mouswe embryo development and hatching. The aim of this study was to evaluate the effect of HSF as assessed by the blastocyst development rate (BDR) and by cell counting in vitro HSF was collected from nine patients undergoing salpingoneostomy to correct hydrosalpinx. Two-cell embryos were obtained from superovulated ICR mice. T6 medium and T6+/-0.4% bovine serum albumin were used as control media. T6 medium containing 10% or 50% HSF and 100% HSF from each patient were used as test media. Nine to 15 embryos were cultured in microdrops prepared from each of these media. To assess the total cell number within each blastocyst, the blastocysts were fixed and stained with Hoechst 33342 to facilitate cell counting. The mean BDR in two control media were 88.89% and 85.40%. The mean BDR in media containing 10%, 50%, 100% HSF were 85.87%, 89.58% and 75.57%, respectively (*: p<0.05). The overall mean cell count (+/-SEM) in control media were 87.6+/-9.65 and 90.12+/-11.38. The BDR was affected adversely only by 100% HSF and not in media containing 10% or 50% HSF. Mean cell counts were decreased significantly only in blastocysts cultured 100% HSF (63.8+/-13.66; p<0.01) but not in blastocysts cultured in 10% or 50% HSF (91.3+/-12.44 and 82.9+/-18.27, respectively). Thus, it is concluded that HSF has no embyotoxic effect but has a mildly negatively effect on embryonic growth and development.

9.A Clinical Study on the Effect of Bezafibrate in Hyperlipidemia.

The lipid lowering effects of bezafibrate were evaluated in 18 patients with hyperlipidemia, i.e. over 200mg/dl of cholesterol and/or of cholesterol and/or triglyceride before treatment. The materials were 18 patients(male:8, female:10);7 with diabetes mellitus, 8 with congestive cardiomyopathy, 1 with parkinsonism, The serum lipid concents were measured with enzyme method before and 6 weeks after the treatment with daily 600mg of bezafibrate administered per oral and the differences were observed. 1) The level of serum cholesterol was reduced from 262.8mg/dl(+/-58.9SD) before treatment to 191.3gm/dl(+/-31.3SD) after 6 weeks treatment(p<03.005). 2) The level of serum triglyceride was reduced from 231.8mg/dl(+/-104SD) before treatment to 144.4mg/dl(+/-51SD) after 6 weeks treatment(p<0.005). 3) The level of serum HDL increased from 47.2mg/dl(+/-19.7SD) before treatment to 61.3mg/dl(+/-13.9SD) after 6 weeks treatment(p<0.005). 4) The level of serum LDL was reduced from 169.3mg/dl(+/-52.4SD) before treatment to 101.2mg/dl(+/-29.5SD) after 6 weeks treatment(p<0.05). 5) HDL/Cholesterol ratio was increased from 17.8%(+/-5.4SD) before treatment to 32.6%(+/-7.9SD) after 6 weeks treatment(p<0.005). 6) HDL/LDL ratio was increased from 29.2%(+/-10.8SD) before treatment to 67.2%(+/-30.8SD) after 6 weeks treatment(<0.005). The side effects of bezafibrate such as G-I trouble, myositis, hypersensitivity, elevation of BUN and creatinine were not observed.

10.Compliance with Low-Salt Diet and Related Factors in Essential Hypertension Patients.

The purpose of this study was to investigate the compliance with low-salt diet in essential hypertension patients, and to identify the related factors of compliance with low-salt diet. The subjects of this study were 177 hypertensive patients who have been followed at Seoul National University Hospital outpatient clinic. The data was collected from August 20, 1998 to September 22, 1998, through survey using a self-report questionnaires and chart review. 24-hour urine sodium excretion was measured for validation of self-reported low-salt diet compliance from 22 patients who agreed for 24-hour urine collection. The questionnaires consist of general characteristics, disease-related characteristics, diet-related characteristics, and 4 scales: (1) Numeric scale (2) Knowledge of low-salt diet (3) Family support for low-salt diet (4) Low-salt diet compliance. The results were as follows: 1) The mean score of low-salt diet compliance was 38.97 +/- 9.26. The mean salt intake converted from 24-hour urine sodium was 16.81g/day, which was much greater than recommendation. The percentage of patients who were taking salt 6-8g/day was only 13.6%, and 8-10g/day was 13.6%. 2) The mean score of knowledge of low-salt diet was 5.12 +/- 1.81. The mean score of family support for low-salt diet was 30.08 +/- 8.81. The patients received emotional, instrumental, evaluative, and informational aspect of family support in sequence of amount. 3) Several factors were found as significant factors which influence low-salt diet compliance. Those were knowledge(p=.015), family support(p=.000), age(p=.039), diastolic pressure(p=.014), previous dietary habit(p= .000), duration of low-salt diet(p=.000), recognition of importance of low-salt diet on hypertension control(p=.000), and recognition of necessity of low-salt diet while antihypertensive drug therapy(p=.030). 4) Four significant predictive factors of low-salt diet compliance were identified: (1) Family support accounted for 24.8% of low-salt diet compliance (2) Previous dietary habit, 14.4% (3) Recognition of importance of low-salt diet on hypertension control, 3.7% (4) Diastolic pressure, 2.1%. Therefore, these factors accounted for 45.0% of low-salt diet compliance. In conclusion, low-salt diet compliance in hypertensive patients was very poor, which call for nursing intervention for enhancing low-salt diet compliance. It is necessary to provide practical knowledge of low-salt diet for hypertensive patients and family members.
