1.THE STUDY ON THE PHYSICAL PROPERTY OF THE PERMANENT SOFT DENTURE LINERS.

This study was performed to evaluate the tensile bond strength and modulus of elasticity of three permanent soft denture liners (Molloplast B , Ufi Gel C , Tokuyama ) before and after thermocycling. And their water sorption were also evaluated. Each soft denture liner was bonded to PMMA denture base resin blocks and the tensile bond strength and modulus of elasticity were measured by using universal testing machine. For the water sorption, weight measured after immersion of soft denture liners in 37+/-1degrees C water bath for 4 weeks. The results were as follows : 1. Molloplast B had the highest tensile bond strength, while Tokuyama had the lowest tensile bond strength, There was no significant difference between Tokuyama and Molloplast B in the both nonthermocycling and thermocycling. There was significant difference in tensile strength of Tokuyama before and after thermocycling(p<0.05). 2. For the modulus of elasticity, there was no significant difference between Ufi Gel C and Tokuyama in the both nonthermocycling and thermocycling. There was significant difference in modulus of elasticity of Tokuyama before and after thermocycling(p<0.05). 3. The failure modes of Molloplast B and Ufi Gel C were nainley adhesive type and that of Tokuyama was mainly mixed type in case of nonthermocycling and cohesive type after thermocycling. 4. The water sorption of each soft liners was within+/-2% in times (p<0.05) but, there was no significant difference among the soft liners in times.

3.Law and Ethics in Emergency Medicine.

Ethical problems in an emergency department (ED) are much more common than is usually recognized. But these difficult ethical dilemmas have not been dealt with by general medical ethicists. Most medical ethics guidelines tend to concentrate on chronic or at least relatively stable situations rather than on the acute, episodic cases that are typical in the ED. most ethical problems such as abortion, euthanasia, and professionalism can be solved after reflection and deliberation, and after a process of communication that reveals the values and interests of the patient or the patient's family. In contrast, when health care professionals in the ED recognize ethical problems, they often don't have enough time for an ethical consultation such as a Hospital Ethics Committee. Ethical principles such as autonomy, beneficence, nonmaleficence, and justice need to be applied to the unique setting of emergency medicine. Hence, it is necessary to develop ethics guidelines in emergency medicine and ethics education for health care professionals in emergency departments. At first, we collected cases involving ethical problems and reviewed the ethical and legal aspects of those cases. In this article, we summarize the ethical issues in emergency medicine, deal with actions in emergency medical services, and also consider the relationships between ethical issues and act on emergency medical services. We want to present the important factors that should be considered in ethical decision making within an emergency medicine department including patient decision making capacity, legal custody, and ethical principles.

5.ACCURACY OF FOUR ELASTOMERIC INTEROCCLUSAL RECORDING MATERIALS.

The purpose of this study is to evaluate the accuracy of four elastomeric interocclusal recording materials and the weight change according to different storage period (24 hours, 48hours, 7days) and different conditions (dry, wet). The techniques for establishing the intercuspal position were centic occlusion interocclusal record with four elastomeric interocclusal recording materials (Ramitec, Stat-BR, Blu-Mousse, Regisil) (experimental groups) and location of the cast in a position of maximum interdigitation using tactile and visual method(control group). To evaluate the accuracy of four elastomeric interocclusal recording materials, the quality of contacts between the mounted casts was compared with the results of an occlusal examination of the same subjects. The results were as follows : 1. The most accurate method of articulating study casts at the intercuspal position was by hand articulation and no significant differences in accuracy were observerd among the experimental groups. 2. In case of interocclusal records storaged in dry sealed plastic box, no significant differences in accuracy were observed among the experimental groups. 3. In case of interocclusal records storaged in water, there was significant inaccuracy in Ramitec group. 4. The biggest weight change was observed in Ramitec group in all storage conditions

6.A Case of Glassy Cell Carcinoma of the Uterine Cervix.

Glassy cell carcinoma (GCC) of the uterine cervix is a rare and highly malignant tumor, accounting for only 1%~2% of all cervical carcinomas. It is typically composed of malignant cells having a moderate amount of cytoplasm with "ground glass" appearance, distinct cell membranes that stain with eosin or periodic acid-Schiff, and large nuclei with prominent nucleoli. Since its original description in 1956 by Glucletmann and Cherry, 200 - 250 cases of GCC of the uterine cervix have been listed in the literature. We report here the clinicopathological study of one case of glassy cell carcinoma with brief review of the literature.

7.AN EVALUATION OF PRECISION FIT OF IMPLANT-SUPPORTED PROSTHESIS USING THE PERIOTEST.

In this study, the Periotest value was measured with Periotest to evaluate precision fit of the 2-unit and 3-unit implant-supported prosthesis by modifying the size and location of ill-fitted conditions. The 2-unit prosthesis was fabricated with the right implant fitted incorrectly and the 3-unit prosthesis with the right and center implant fitted incorrectly. To evaluate the effects of the ill-fitted sizes, 4 groups were divided.:The control group being the accurately fabricated sample group fitted properly. Group 1 was constructed with 40micrometer ill-fitted conditions, group 2 with 70micrometer and group 3 with 100micrometer ill-fitted conditions. The Periotest value was measured at each implant site after tightening 10Ncm. The result was follows: 1. The PTV on the ill-fitted area in the 2-unit implant-supported prosthesis increased as the ill-fitted conditions increased. There was a statistically significant difference among groups(p<0.05). In the same ill-fitted sample, the PTV depending on the measured location demonstrated a statistically significant difference (p<0.05). 2. The PTV on the ill-fitted area of the 3-unit implant-supported with an ill-fitted condition in the right implant increased as the ill-fitted conditions increased. There was a significant difference among groups (p<0.05). In the same ill-fitted sample, the PTV depending on the measured location demonstrated a statistically significant difference (p<0.05). 3. In the 3-unit implant-supported prosthesis with ill fitting conditions in the center implant, the PTV on the ill-fitted area demonstrated a statistically significant difference between the control group, group 1 and group 2 (p<0.05). In the same ill-fitted sample, the PTV depending on the measured location demonstrated significant difference between the gap side and the adjacent side with over 70micrometer ill-fitted conditions (p<0.05). The results suggest that Periotest is a valuable objective method for evaluating the precision fit of an implant superstructure.

8.Two Cases of Endometriosis in the Extraperitoneal Portion of the Uterine Round Ligament.

Extraperitoneal inguinal endometriosis often presents as an inguinal mass in women of reproductive age, Approximately 40 cases of inguinal endometriosis have been documented in the sutgical and gynecological literature. The overall incidence is in the range of 0.4% of patients with endometriosis. The right side is much more commonly involved than the left (almost 90% of the time in one series of 23 patients), and rarely is the condition bilateral. The most frequent complaint of patients with inguinal endometriosis is an inguinal mass, followed by pain and enlargement of the mass near menstruation. Authors have experienced two cases of endometriosis of the extraperitoneal portion of the round ligament of uterus and report with a brief literature review.

9.A Case of Primary Leiomyosarcoma of the Uterine Cervix.

Primary sarcomas of the uteriae cerrix are exceedingly rare neoplasms. They comprise less than 1 percent of all cancers, with an incidence of 0.2 to 0.4 percent of those originating in the cervix. Indeed, only 19 cases of primary cervical leiomyosarcomas have been reported in the English literature. We have recently experienced one case of leiomyosarcoma arising from the cervix in a 40-year-old woman. Therefore, we the case with a brief review of the literature.

10.The risk factors of residual disease after conization.

OBJECTIVE: Conization is used for diagnosis and treatment of cervical neoplasia. Our purpose of this investigation is to determine the efficacy of loop conization for the treatment of cervical dysplasia and the significance of the clinical and histological factors used to predict residual dysplasia after loop conization. METHODS: We reviewed the charts of patients who were received conization and subsequently total hysterectomy at Kangnam St Mary Hospital during 1989 and 2000. Logistic regression and Chi-square test were used for analysis. RESULTS: Total 257 patients were included. The mean age of the patients was 44.7 years. Of these patients, 87 (33.8%) had residual disease in the hysterectomy specimens. Age, the involvement of cut surface of endocervix and exocervix, severity and extent of the lesion, and scattering lesion were associated with the residual disease. But preoperative HPV infection, which is known as the cause of cervical neoplasia was not associated with the residual disease. CONCLUSION: When the high risk factors are present, the validity of conservative treatment should be considered and more careful follow up with pap smear, HPV test and colposcopy is necessary. Because about one thirds of patient has residual disease after conization, LEEP conization should be used for diagnosis of cervical neoplasia rather than treatment.
