2.The Relationships among Emotional Intelligence, Interpersonal Relationship, and Job Satisfaction of Clinical Nurses.

PURPOSE: The purpose of this study was to investigate the relations among emotional intelligence, interpersonal relationship, and job satisfaction among clinical nurses. METHODS: Data were collected from 315 nurses who had worked for more than 6 months at five general hospitals by a self-reported questionnaire. The collected data were analyzed with descriptive statistics, t-test, ANOVA, Scheffe-test, Pearson's correlation, and hierarchial regression using SPSS 18.0 program. RESULTS: The mean score of emotional intelligence was 3.42 and the score of self emotional appraisal was the highest. The mean score of interpersonal relationship was 3.44 and the score of intimacy was the highest. The mean score of job satisfaction was 3.04. Emotional intelligence and interpersonal relationship were positively correlated with the job satisfaction. Hierarchial multiple regression analysis showed that use of emotion was the main factor of affecting job satisfaction, which explained 30.8% of the variance for the nurses' job satisfaction together with intimacy, regulation of emotion, position, and monthly salary. CONCLUSION: These findings indicated that emotional intelligence and interpersonal relationship, especially use and control of emotion and intimacy contributed to nurses' job satisfaction. It is necessary to develop and implement the program for increasing emotional intelligence and interpersonal relationship for improving job satisfaction.

3.IRB review points for studies utilizing paraffin blocks archived in the pathology laboratory

In the personalized medicine era, utilizing paraffin blocks in pathology archives for investigating human diseases has come into the limelight. This archived material with clinical data will reduce the research time and could prevent new patient recruitment to obtain tissue for research. However, the clause indicating the necessity of consent from human material providers in the Korean Bioethics and Safety Act has made the Institutional Review Board (IRB) deny permission to use paraffin blocks for research without consent, and alternatively to get the same before starting an experiment. Written consent may be waived off in studies using paraffin blocks with anonymous status or conditions not linked to personal information by applying the paragraph 3, article 16 of the current Bioethics and Safety Act. Also, the IRB should recommend researchers to preserve the blocks as medical records of patients in long-term archives.

5.The effect of LH Receptor in the Pregnancy of Poor Responders.

OBJECTIVES: To investigate the effect of LH receptor in folliculogenesis, we confirm the expression level of LH receptor (LH-R) mRNA in human granulosa cells (GCs) and its expression levels were analyzed by comparison to embryo developmental rate and pregnancy rate. MATERIALS AND METHODS: GCs were obtained at the time of oocyte retrieval from the patients undergoing IVF-ET program. The patients were divided into two groups: Group I (n=20) is poor responder (retrieved oocyte(s)< or =3ea), Group II (n=80) is normal responder (retrieved oocytes>3ea). After the extraction of total RNA, semiquantitative RT-PCR was performed and the expression level of LH-R mRNA was normalized by beta-actin. Statistical analysis was performed by using Chi(2) test, Student's t-test and Pearson correlation. RESULTS: In Group II, the relative values of LH-R mRNA (0.680 vs. 0.463, p<0.005) and pregnancy rate (54.7% vs. 23.1%, p<0.05) were significantly higher than in Group I. Number of retrieved oocyte(s) was gradually increased when the expression of LH-R mRNA was increased (p<0.05). But the quality of retrieved oocyte and transferred embryo were not related with the expression of LH-R mRNA. When the pregnancy rate was compared with FSH only group and FSH combined with hMG group in the ovarian stimulation protocol, FSH combined with hMG group was significantly higher than FSH only group in Group I (37.5% vs. 0%), and the expression of LH-R mRNA was significantly higher in hMG combined group than FSH only group (p<0.05). CONCLUSION: Expression level of LH-R mRNA has important role in ovarian function related with the response to gonadotrophin in human folliculogenesis. Furthermore these data might provide the evidence that additional use of hMG is helpful to poor responders.

Hemangiomas are the most common benign tumors in infancy, occurring most often on cutaneous and mucosal surfaces. Although less than 1% of hemangiomas occur in skeletal muscle, 15% of intramuscular hemangiomas arise in the head and neck musculature. Less than 10 cases of the sternocleidomastoid muscle have been reported in the English literatures. The masseter muscle is most commonly involved in the head and neck. Clinically, these tumors are present as distinct, localized, rubbery swelling. Neck computed tomography scan with enhancement may suggest a vascular mass within the muscle while angiography may detect feeding arteries in large intramuscular hemangiomas. The treatment of the hemangiomas is based on location, accessibility, depth of invasion, age, and cosmetic considerations. The optimal treatment is complete wide resection including the cuff of surrounding muscle. We report a case of hemangioma that occurred in the sternocleidomastoid muscle along with a with literature review.

8.A Clinical Analysis of 475 Cases of Colorectal Cancer.

Colorectal cancer is the third most common malignant neoplasm worldwide and the fourth most common cancer in Korea. Recently, incidence of colorectal cancer is increasing steadily. Because of advances in diagnostic tools, such as colonofiberoscopy, early detection of colorectal cancers is available. The survival rates of colorectal cancer is increasing due to early detection and advances in adjuvant therapy, such as chemotherapy and radiotherapy. The clinical analysis was made on 475 patients with colorectal cancer who have been admitted and operated at the Department of Surgery, Chonnam University Hospital, from January, 1987 to December, 1996. The results were as follows: 1) The peak age group was in the sixth(29.7%) and seventh decade(29.7%) followed by the fifth decade(17.3%) of life. The male: female ratio was 1.1:1. 2) The annual incidence was increased steadily, but since 1994, plateau in incidence has occurred. 3) The most common site was the rectum(62.7%). 4) Common symptoms and signs were anal bleeding(40.1%) and abdominal pain (28.1%). According to site, anal bleeding was the most common complaint in the rectum and left sided colon and abdominal pain was in other colons and cecum. 5) The most common stage by TNM staging system was stage II (38.7%). 6) Most cases of colorectal cancer were adenocarcinoma(97.7%), well differentiated lobular adenocarcinoma(55.6%) in which was the most common histopathologic type. 7) Preoperative circulating CEA(carcinoembryonic antigen) level was checked in 396 patients(83.4%). CEA was less than 10.0 ng/ml in 295 cases(74.5%) and more than 10.0 ng/ml in 101 cases(25.6%). 8) DNA ploidy was checked in 197 cases(41.5%). There ware aneuploid in 102 cases, diploid in 95 cases. 9) Curative surgery were performed in most cases. The most common surgical procedure was low anterior resection(35.4%). 10) Postoperative complications were developed in 63 patients(13.3%). The most common complication was wound infection in 12 cases(19.0%). 11) The overall 5-year survival rate for patients with colorectal cancer was 55.9%. The difference of 5-year survival rate between diploidy and aneuploidy of DNA content was not significant statically(p=0.97). But, patients of preoperative serum CEA level over 10 ng/ml had a low survival rate compared with those under 10 ng/ml(p=0.0002). And also those of tumor size over 3 cm had a low survival rate compared with those under 3 cm(p=0.002).

9.Recurrent Rhabdomyolysis after Cerivastatin-Gemfibrozil Combination Therapy.

A 64-year-old woman with ischemic heart disease was admitted to our hospital because of both leg pain and difficulty to walk for 5 days. She had taken cerivastatin and gemfibrozil for atherosclerosis and ischemic heart disease. One year ago, she had been admitted to our hospital because of acute renal failure due to rhabdomyolysis of unknown origin and was improved after conservative therapy. Laboratory studies revealed serum creatinine 1.2 mg/dL, creatine kinase 23,700 IU/L, serum myoglobin >500 ng/mL, urine myoglobin >3,000 ng/mL. (99m)Tc-HDP whole body Bone scan showed multiple increased uptakes of the soft tissue, especially both calf. With supportive care, she was recovered and discharged with normal creatinine(0.8 mg/dL) and creatine kinase(260 IU/L).

10.Combined Chemoradiotherapy vs Radiotherapy Alone for LocallyAdvanced Squamous Cell Carcinoma of the Head and Neck.

PURPOSE: The traditional approach with surgery and/or radiotherapy(RT) for advanced head and neck cancer provides anticipated cure rates of 10-65% depending on stages and sites. Recently, combined modality with chemotherapy have been extensively investigated in attempts to improve survival and local control. We retrospectively analysed our experience of 31 patients with advanced head and neck cancer METHODS AND MATERIALS: From November 1983 to October 1994, 31 patients with Stage II and IV squamous cell head and neck cancer were treated with RT. Sixteen patients were treated with RT alone. and 15 patients were treated with combined RT plus chemotherapy. All patients were treated with 4-MV LINAC and radiation dose ranged from 5000 cGy to 7760 cGy(median 7010 cGy). In combined group. 7 patients were treated with cis-platin plus 5-FU, 2 patients were treated with methotrexate plus leucovorin plus 5-FU plus cisplatin or carboplatin, and 6 patients were treated with cisplatin as a radiosensitizer. RESULTS: Median follow up period was 16 months (range 4-134 months). The major responses (CR+PR) were noted in 10 patient (66.6%) of the RT alone group and 14 patient (93.3%) of the chemoradiation group. There was no statistical difference in CR rate between the two groups. The overall survival rates at 5 years were 23.4% in the radiation alone group. 23.5% in the chemoradiation group. Disease-free survival rates at 3 year were 44.5% in the radiation alone group. 40% in the chemoradiation group. There was no statistical differences in overall survival rates and disease free survival rates between the two groups.Local recurrences occurred in 71.5% of the radiation alone group. 72.7% of the chemoradiation group and distant metastasis occurred in 14.4% of radiation alone group. 9.1% of the chemoradiation group. The frequencies of complications were comparable in both groups except hematologic toxicity. CONCLUSION: Total response rates in the combined chemotherapy and radiotherapy was relatively higher than radiotherapy alone. But our result failed to show any survival benefit of the combined chemotherapy and radiotherapy. The accrual of large number of patients and long term follow-up may be necessary to confirm the present result of combined chemotherapy and radiotherapy.
