3.Acetabular Revision by Using Femoral Head Allograft and Uncemented Cup.

Acetabular bone deficiencies encountered during the revision hip arthroplasties should be recon- structed to provide the implant stability and to restore the normal center of rotation of hip and the leg length. We revised the loosened acetabular cup by grafting fresh-frozen bulk femoral head and inserting uncemented cup in 17 hips of 15 patients. The average follow-up period was 2 years and 3 months. The acetabular bone deficiencies were type 2A in 6 hips, type 2B in 8, type 3A in 1 and type 3B in 2 by Paprosky's classification. Three blocks of femoral head were grafted in 3 type 3 deficiencies, but only one in type 2 deficiencies. The cup-host bone contact was 41% on the average. However, the cup-host bone contact in the zone I was present only in 12 out of 17 hips and its average was 14%. Incorporation of the allograft into the host bone occurred between 5 months and 1 year and 7 months (average, 8,6 months) after revision surgery. Significant radiographic loosening sign was noted only in 2 hips which had not only type 3B bone deficiencies reconstructed with 3 blocks of femoral head allograft but also no cup-host bone contact in zone I . The bulk allograft of fresh-frozen femoral head demonstrated acceptable results in type 2 acetabular bone deficiencies, although the follow-up period was relatively short. Reconstruction of type 3B acetabular bone deficiencies by using multiple blocks of femoral head allograft had been failed. We presumed that the lack of the graft stability and the intimate contact between the grafts and host bone was the cause of failure.

4.Difference in Injury of the Corticospinal Tract and Spinothalamic Tract in Patients with Putaminal Hemorrhage

PURPOSE:
We investigated the difference in injury of the corticospinal tract (CST) and the spinothalamic tract (STT) in patients with putaminal hemorrhage, using diffusion tensor tractography (DTT).
METHODS:
Thirty one consecutive patients with PH and 34 control subjects were recruited for this study. DTT scanning was performed at early stage of PH (7-63 days), and the CST and STT were reconstructed using the Functional Magnetic Resonance Imaging of Brain (FMRIB) Software Library program. Injury of the CST and STT was defined in terms of the configuration or abnormal DTT parameters was more than 2 standard deviations lower than that of normal control subjects.
RESULTS:
Among 31 patients, all 31 patients (100%) had injury of the CTS, whereas 25 patients (80.6%) had injury of the STT: the incidence of CST injury was significantly higher than that of STT (p<0.05). In detail, 20 (64.5%) of 31 patients showed a discontinuation of the CST in the affected hemisphere; in contrast, 14 patients (45.2%) of 31 patients showed a discontinuation of the STT in the affected hemisphere. Regarding the FA value, 6 (19.4%) of 31 patients and 2 (6.4%) of 31 patients were found to have injury in the CST and STT, respectively. In terms of the fiber number, the same injury incidence was observed in 11 patients (35.5%) in both the CST and STT.
CONCLUSION
The greater vulnerability of the CST appears to be ascribed to the anatomical characteristics; the CST is located anteriorly to the center of the putamen compared with the STT.
5.Visual recovery demonstrated by functional MRI and diffusion tensor tractography in bilateral occipital lobe infarction.

We report on a patient who showed visual recovery following bilateral occipital lobe infarct, as evaluated by follow up functional magnetic resonance imaging (fMRI) and diffusion tensor tractography (DTT). A 56-year-old female patient exhibited severe visual impairment since onset of the cerebral infarct in the bilateral occipital lobes. The patient complained that she could not see anything, although the central part of the visual field remained dimly at 1 week after onset. However, her visual function has shown improvement with time. As a result, at 5 weeks after onset, she notified that her visual field and visual acuity had improved. fMRI and DTT were acquired at 1 week and 4 weeks after onset, using a 1.5-T Philips Gyroscan Intera. The fiber number of left optic radiation (OR) increased from 257 (1-week) to 353 (4-week), although the fiber numbers for right OR were similar. No activation in the occipital lobe was observed on 1-week fMRI. By contrast, activation of the visual cortex, including the bilateral primary visual cortex, was observed on 4-week fMRI. We demonstrated visual recovery in this patient in terms of the changes observed on DTT and fMRI. It appears that the recovery of the left OR was attributed more to resolution of local factors, such as peri-infarct edema, than brain plasticity.

6.Doppler Echocardiographic Findings of Mitral Valve Prolapse : Usefulness of the Apical Rotation Method of a Transducer for Assessment of Site of Prolapse.

BACKGROUND: Color Doppler echocardiography is sensitive in detecting mitral regurgitation and useful in quantitating its severity. The presence of an eccentric regurgitant jet suggests that regurgitation is caused by prolapsing or flail leaflet of mitral valve. Until recently the direction of regurgitant jet in mitral valve prolapse has been examined in a single(parasternal short axis view) or orthogonal plane using color Doppler echocardiography, and few in the apical rotation method of a transducer. The purpose of this study was to clarify the usefullness of the apical rotation method of a transducer in detection of the direction of mitral regurgitant jet and diagnosis of the sites of mitral valve prolapse. METHODS: Twenty four patients(8 men and 16 women, mean age:47.3+/-18.8 years) with mitral valve prolapse with eccentric regurgitant jet were examined by two-dimensional and color Doppler echocardiograply using conventional parasternal long and short axis views, and four apical longitudinal planes(four chamber, vertical, two chamber and transverse views) obtained by the apical retation method of a transducer. RESULTS: Thirty one regurgitant jets were detected in twenty four patients, eighteen patients had anterior, nine patints posterior, and three patients bi-leaflet(anterior and posterior) prolapse. In eighteen patients with anterior leaflet prolapse, ten had medial, eight had middle, three had lateral, and three had two portions(two, medial and middle; one, middle and lateral) prolapse. In nine patients with posterior leaflet prolapse, five had medial, three had middle, two had lateral, and one had two(medial and middle) scallop prolapse. CONCLUSION: Color Doppler echocardiography by the apical rotation method of transducer is useful in assessment of the site of prolapse in patients with mitral valve prolapse with eccentric regurgitation.

7.Comparison between Planar View and SPECT View on the Dipyridamole Thallium 201 Myocardial Perfusion Scintigraphy.

Exercise testing with Thallium imaging is widely used for the noninvasive evaluation of patients suspected of having coronary artery disease. However, many patients referred for stress testing connot exercise adequately for either physical or psychological reasons, and as a result may have nondiagnostic or suboptimal test results. Intravenous dipyridamole in conjunction with Thallium imaging is as effective alternative method without exercise. But, myocardial imaging using the standard scintillation camera technique(planner view) is hampered by superposition of proximal & distal cardial walls and by the segmental nature of myocardial ischemia. For this reason, Single Photon Emission Computed Tomography(SPECT) reslut in high specificity & sensitivity rates for the detection of coronar artery disease compared with conventional technique. So we performed dipyridamole Th-201 myocardial scintigraphy on 25 subjects who have suspicious angina or myocardial infarction instead of exercise Th-201 myocardial scintigraphy, and compared SPECT view with conventional plannar view. The results obtained are as follows : 1) T1-201 scintigraphic findings in 17 patients with suspicious angina were as follows ; redistribution defect was seen in 4 cases in plannar view and 13 cases in SPECT view. 2) T1-201 scintigraphic findings in 8 patients with myocardial infarction were as follows ; in planner view, perfusion defect was seen in all cases and 1 cases of them, redistribution defect was accompained, and in SPECT view, perfusion defect was seen in all cases and 6 cases of them, redistribution defect was accompained. 3) During dipyridamole infusion, the mean systolic & diatolic pressure decreased from 133+/-22.7/86+/-13.5 to 121+/-23.9/78+/-13.1mmHg and the heart rate increased from 68+/-12.4 to 84+/-12.4beats/min. 4) Adverse effects of dipyridamole were noted in 14(56%) of the subjects, but in 12 of these, the symptoms were mild in severity and subsided spontanously. To summarize, Dipyridamole-201 myocardial imaging is a useful and test for coronary artery disease, and the new tomographic technique, SPECT view, is more useful than the conventional plannar view.

8.Characteristics of Corticospinal Tract Area According to Pontine Level.

Diffusion tensor imaging (DTI) allows to isolate the corticospinal tract (CST) area from adjacent structures. Using DTI, we investigated the characteristics of the CST areas according to the pontine level in the normal human brain. We recruited 33 healthy subjects and DTIs were acquired using a sensitivity-encoding head coil on a 1.5-T Philips Gyroscan Intera. We measured the size and fractional anisotropy (FA) value of the CST area at the upper, middle, and lower pons. The size of the CST area in the lower pons was smaller than those of the mid-pons and upper pons, and the size of the CST area in the mid-pons was smallerthan that of the upper pons (p<0.05). FA values of the lower pons were larger than those of the mid-pons and upper pons, and the FA value of the mid-pons was also larger than that of the upper pons (p<0.05). In summary, we found a smaller size and higher FA value of the CST area from rostral to caudal direction in the pons. These results suggest a more compact neural structure of CST areas from rostral to caudal direction in the pons.

9.The Effects of Horse-riding Simulator Training with Dual-task on Pulmonary Function and Flexibility in Healthy Adults

Purpose:
The purpose of the present study was to investigate the effects of a simultaneous dual-task and horse-riding simulator (HRS) training regime on pulmonary function and flexibility.
Methods:
Sixteen subjects were recruited and randomly allocated to two groups: a dual-task (DT) (n=8) or a single-task (ST) (n=8) training group. Flexibility and pulmonary function were assessed before and after HRS training. Both groups underwent HRS training for 4 weeks, 3 times/week in 15-minute training sessions. The ST group underwent HRS training and the DT group underwent dual-task HRS training, which consisted of throwing and catching a ball and ring catching while HRS training.
Results:
Training significantly increased flexibility and FVC (forced vital capacity) and FEV1 (forced expiratory volume in 1 second) in both groups (p<0.05), but FEV1/FVC and PEF (peak expiratory flow) were not significantly different after training in both groups (p>0.05). After the training, flexibility and FVC in the DT group were significantly greater than in the ST group (p<0.05), but FEV1, FEV1/FVC, and PEF were not significantly different (p>0.05).
Conclusion
Simultaneous dual-task and HRS motor training improved flexibility, FVC, and FEV1, and our comparative analysis suggests that dual-task HRS training improved flexibility and FVC more than single-task training.
10.The Nigrostriatal Tract between the Substantia Nigra and Striatum in the Human Brain: A Diffusion Tensor Tractography Study

Objectives:
The nigrostriatal tract (NST) connect from the substantia nigra pars compacta to the striatum. A few previous studies have reported on the NST in the Parkinson’s disease using a proboblistic tractography method. However, no study has been conducted for identification of the NST using streamline DTT technique. In the current study, we used streamline DTI technique to investigate the reconstruction method and characteristics of the NST in normal subjects.
Methods:
Eleven healthy subjects were recruited in this study. The NST from the substantia nigra of the midbrain and the striatum of basal ganglia was reconstructed using DTI data. Fractional anisotropy, apparent diffusion coefficient (ADC) values and fiber numbers of the NST were measured.
Results:
In all subjects, the NST between the substantia nigra of the midbrain and the striatum. Mean values for FA, ADC, and tract volume were 0.460, 0.818, and 154.3 in the right NST, and 0.485, 0.818, and 176.3 in the left NST respectively.
Conclusions
we reconstructed the NRT from the substantia nigra of the midbrain and the striatum of the basal ganglia using streamline tractography method. We believe that the findings and the proposed streamline reconstruction method of this study would be useful in future researches on the NST of the human brain.
