1.Value of the Left Portal Vein Angle (LPVA) on CT for the Diagnosis of Liver Cirrhosis: Comparison with the Caudateto Right Lobe (C/RL) Ratio.

PURPOSE: To verify the usefulness of left portal vein angle (LPVA) on CT scan in the diagnosis of liver cirrhosis and to compare its diagnostic value with that of caudate to right lobe ratio (C/RL ratio). MATERIALS AND METHODS: LPVA, an angle formed by a vertical line and a line connecting the center of the vertebral body to the umbilical point of the left portal vein, and C/RL ratio were measured on CT scans of 100 cirrhotic and 100 normal livers. Diagnostic values of LPVA and C/RL ratio were compared statistically. RESULTS: The mean of LPVA was 18.9 degrees(SD; 7.6) for normal livers and 25.8 degreesSD; 8.4) for cirrhotic livers (P<0.001). The mean of C/RL ratio was 0.47(SD; 1.10) for normal livers and 0.58(SD;0.14) for cirrhotic livers (P<0.001). When LPVA was greater than 30 degreesliver cirrhosis was diagnosed with 36% sensitivity and 92% specificity. When C/RL ratio was greater than 0.60, the diagnose of liver cirrhosis was with 41% sensitivity and 90% specificity. There was no significant difference of the diagnostic accuracy between LPVA and C/R L ratio in ROC analysis. CONCLUSION: Both LPVA and C/RL ratio are useful diagnostic indices of liver cirrhosis on CTscan. LPVA is more convenient to measure than C/RL ratio.

2.A Case of Renal Parenchymal Hypertension in Child.

About 10-15% of hypertension is secondary to renal disease. Cause of renal hypertension is renal parenchymal disease or renovascular problem. Hypertension in children which estimate at 1-2 % of all hypertension is very rare. We experienced a ease of renal parenchymal hypertension due to unilateral chronic reflux pyelonephritis in 10 year old boy, treated by nephrectomy.

3.Characteristic Changes of Cough Urethral Pressure Profile in Stress Urinary Incontinence.

Urodynamic study developed because physicians recognized that history, physical examination and previous testing modality were inadequate to differentiate between those patients with and those without stress urinary incontinence( SUI). The purpose of this study is to evaluate the practicability and usefulness of the test cough urethral pressure profiler in the evaluation of SUI with objective data. Between November 1991 and October 1993, 45 patients with symptoms of urinary incontinence were studied to determine the value of this test by using microtip transducers of Urocompact 8000 which is a computerized urodynamic system. Dynamic pressure profiles were obtained during cough by measuring the pressure at the proximal, middle and distal urethra. At rest, functional urethral length and maximal urethral closure pressure were 3.01+/-0.58cm, 52.0+/-17.8cmH2O respectively (control 3.66+/-0.35cm, 80.25+/-16.95cmH2Orespectively, p <0.05, p<0.05). During the cough, urethral closure pressure decreased to negative pressure in 41 of 42(98% ) patients who were available for analysis. Three of the 45 patients were not available due to the lack of experience of one. Six months after bladder neck suspension was performed, the urethral pressure profile of four patients were checked again and the results showed that the functional urethral length and maximal urethral closure pressure at rest were greater than those of the preoperative urethral pressure profile, but statistically not significant. In 3 patients who underwent successful treatment, the urethral closure pressure at cough did not decrease to a negative pressure. As a result, the cough urethral pressure profile is a useful and objective diagnostic method of SUI and evaluation of the postoperative state.

4.Correlation Between Histological Changes of Human Testis after and Fertility after Subsequent Vasectomy Reversal Vasectomy.

The aim of this study is to determine the correlation between any histological changes in the human testis after vasectomy and fertility after subsequent vasectomy reversal. A sample of 21 previous fertile, vasectomized, healthy volunteers with a mean age 35 years( range, 29 to 47) underwent open, unilateral testicular biopsy at the time of vasectomy reversal. The mean duration of obstruction was 3.8 years(range, 1 to 12). The vasovasostomy was performed by means of same technique. The qualitative and morphometric quantitative analysis were performed for the data obtained. Vasal patency was determined by the presence of sperm in the postoperative semen analysis in 18 patients. Pregnancy occurred in 9 of 18 couples( 50%). Statistical analysis was performed to compare the morphological findings in both fertile and infertile patients. No significant correlation was observed between the histologic changes and fertility in two groups. Similarily there was no significant correlation between the obstructive interval. We concluded that morphologic changes occurred in the human testis after vasectomy does not affect the fertility rate after successful vasectomy reversal.

6.Double chambered right ventricle

Fourteen cases of double chambered right ventricle were diagnosed angiographically and of these nine caseswere confirmed after operation and autopsy at Seoul National University Hospital in recent four years since 1979.The clinical and radiological findings with the emphasis on the cinecardiographic findings were analysed. Thesummaries of the analysis are as follows; 1. Among 14 cases, 6 cases were male and 8 cases were female. Agedistribution was from 4 years to 36 years. 2. In chest X-ray findings, pulmonary vascularity was increased in 8cases, decreased in 4 cases, and nomral in 2 cases. Cardiomegaly was observed in 8 cases and other showed normalheart size. 3. In cinecardiography, 11 cases had interventricular septal defect. Among these 11 cases, VSD locatedin proximal high pressusre chamber was in 2 cases and located in distal low presssure chamber was in 9 cases. 4.The location of aberrant muscle bundle in sinus portion of right ventricule was in 8 cases. In the rest cases, andaberrant muscle bundle was located below the infundibulum of right ventricle. 5. For accurate diagnosis anddifferential diagnosis with other congenital cardiac anomalies such as Tetralogy of Fallot or isolated pulmonicstenosis, biplane cineangiography and catheterization is an essential procedure.

7.A case of metastatic retroperitoneal fibrosis from the gastric adenocarcinoma.

We report a case of 45-year-old woman of histologically proven malignant retroperitoneal fibrosis from signet ring cell type adenocarcinoma of the stomach. She had suffered from flank colicky pain for a duration of a month. We underwent ureterolysis with omental wrappine under the impression of retroperitoneal fibrosis due to miss-diagnosed chronic inflammation of periureteral tissue by frozen biopsy at the time of the operation. As urine leaks continued at the drain site for 22 days despite of nephrostomy tube placement, we underwent nephrectomy which revealed signet ring cell type adenocarcinoma. Gastrofiberscopic biopsy resulted in the same adenocarcinoma.

OBJECTIVE: This study was aimed to investigate of newborn babies sex ratio in Seoul Seong Ae hospital and review of future problem. METHODS: We reviewed retrospectively the 21,332 cases of deliveries in women visiting at Seoul Seong Ae hospital from Jannuary 1994 to December 2003 (10 year's period). We investigated 1st baby and 2nd baby sex ratio and in case of 3rd baby, subdividing then into tree groups (first and second are female, first and second are male, first and second are male and female). We arranged the data at the base of childbirth registers and patients' obligation records. RESULTS: The results were as follows, 1. The total sex ratio in last 10 years was 111.2:100 (male:female). 2. 1st baby sex ratio was 109.2:100. 3. 2nd baby sex ratio was 110.6:100. 4. 3rd baby sex ratio was 133:100. CONCLUSION: These findings suggest that unbalance of sex ratio in 3rd baby is extremely serious trend in this country and is expected a tremendous social problems in early 21th century.

10.A Case of Double Pylorus.

The double pylorus is a rare condition consisting of a double communication between gastric antrum and duodenal bulb. Some investigators postulate that the doubling of the pyloric channel is a congenital phenornenon, but others believe that it is an acquired lesion. A 72 year-old-man was admitted to this hospital because of epigastric pain for 1 month. Upper G-I series revealed thickened rnucosal folds of pylorus and duodenal bulb and dilated, deformed duodenal bulb filled with barium materials. Endoscopic findings also showed two ovoid large openings of the pyloric channel divided by smooth thickened septum and multiple gastroduodenal ulcers. We thought that this case was an acquired lesion. The relevant literatures on the subject were reviewed.
