2.A Case of Krukenberg Tumor associated with Ovarian Dermoid Cyst.

Krukenberg tumor, which can account for 30-40% of metastatic cancers to the ovaries, arises in the ovarian stroma and is usually metastatic from the gastrointestinal tract, especially from the stomach. The pathognomonic feature is the presence of signet ring cells, which may be arranged in acini or appear as individual cells. We experienced a Krukenberg tumor which was bilateral and associated with left ovarian dermoid cyst. This patient was treated for a primary gastric carcinoma(Stage II) about 3 years ago. After bilateral salpingoophorectomy, she received adjuvant chemotherapy. But, she died about fow months after operation.

3.Clinical Observation of Cotreal

Various kinds of spinal instrumentation have been developed for treatment of spinal disorders which are associated with instability. Recently, newly designed devices using pedicle screw were developed with advantages of short segment fixation and firm internal fixation. This is a retrospective clinical and roentgenographic study to evaluate the effectiveness of Cotrel 1. Of the 27 cases, spinal stenosis were 11, spondylolisthesis were 8, spondylolysis were 4, burst fracture were 2, metastatic bone tumor was 1, and failed back syndrome was 1 case. 2. Result of clinical evaluation by Hanley's criteria at last follow up were excellent in 7(27%), Good in 15(58% ), fair in 4(15%). (1 case of metastatic bone tumor was excluded). 3. In the cases of spondylolisthesis, we tried to reduce the displacement in 2 cases of Meyerding Grade II, and 1 case was reduced and 1 case was not reduced. In the cases of of Meyerding Grade I, no further displacement was developed. 4. The complications were observed in 10(37%)cases. A) Generalized complications were 2 cases of hematoma, 3 cases of superficial infection of wound, and 1 case of nerve root irritation. B) Failure in instrmentation were 1 case of screw breakage, 1 case of complete loss of fixation between screw and rod, and 2 cases of partial loss of fixation. 5. To prevent above mentioned complications in fixation of the instrument, the following factors are recommended: 1) Selection of the adequate length of rod. 2) Fixation of the additional implant such as security bolt when instability between screw and rod is predictable. 3) Selection of the closed head screw in the fixation of proximally sided screw if possible. 6. Cotrel-Dubousset instrument has many advantages such as rigid internal fixation, anatomical reduction, and good maintenance and also effective in reduction and maintenance of spondylolisthesis and in wide posterior decompression of spinal stenosis, but for prevention of some complications, precise use of instrument and good application of surgical technique will be needed.

5.Morphometric Study of the Pedicles of Thoracic and Lumbar Vertebrae in Korean

Total 7,446 pedicle measurements were made from T1 to L6. Direct dimensional measurements were obtained from dried adult human spinal columns. Parsmeters considered were the pedicle isthmus diameter in the coronal and sagittal planes, pedicle angles in the transverse and sagittal planes, and the depth to the anterior cortex in a line parallel to the midline of the vertetral body and along the pedicle axis. The results were as follows. 1. The widest pedicle transverse diameters were seen at L6, the narrowest at T4. 2. The widest pedicle superoinferior diameters were seen at T12, the narrowes at T1. 3. In the transverse plane, the posterolateral to anteromedial pedicle axis orientations were seen at all levels except at T12 and T12. 4. In the sagittal plane, the pedicles angled caudally at L5 and cephaladly from L3 to T1. 5. In the thoracolumbar junction, the percentage less than 6mm in the transverse diameters of pedicles is most high at L1(27%) and followed by at the level L2, T11 and T12. 6. The use of 6mm transpedicular screws can be expected to violate the cortex of the pedicles in a significant number in the upper lumbar spine. 7. The 5mm transpedicular screws can be used safely below the T11 level. 8. It is advisable to use roentgen image intensifier during insertion of the transpedicular screws.

We report a rescently experienced case with atypical radiologic manifestation of solid and papillary epithelial neoplasm of the pancreas in a 37 years old female patient. The tumor had heavy calcification on its capsule wall and septa. Instead of the usual encapsulated lesion with partly solid and partly cystic-hemorrhagic components, the lesion consisted of numerous fine cavitations containing air without fluid component except focal abscess fluid.

7.The Clinical Values of Metaplasia, p 53, c - erbB2 and CEA Expression in Gallbladder Carcinoma.

PURPOSE: We evaluated the correlation between the carcinogenesis of gallbladder and the expression of lysozyme, p53, c-erbB2 and CEA in gallbladder lesions. MATERIALS AND METHODS: Thirty cases of gallbladder lesions (containing 17 cases of GB carcinoma) were examined. We analyzed the clinicopathologic findings of the early (stage I & II) and advanced carcinoma (stage III, IV & V) and those of carcinoma with or without metaplasia in the tumor. We performed p53, c-erbB2 and CEA immunohistochemical staining and compared their findings with those of normal mucosa and preneoplastic lesions. We also performed lysozyme immunohistochemical staining and compared its finding with metaplastic and non-metaplastic lesions. RESULTS: There are two distinct genetic pathways in gallbladder cacinogenesis and metaplastic carcinoma was more frequent than non-metaplastic carcinoma. Metaplasia of gallbladder did not reveal any difference of the clinicopathologic findings and depth of invasion (Nevin stage). Lysozyme expression was found in all metaplastic lesions but non-expression did not indicate non-metaplastic lesions. p53 mutations and c-erbB2 alterations may have a role in the carcinogenesis of gallbladder carcinomas, especially, in a late event, and in an early and late events, respectively. The correlation of p53 and c-erbB2 expressions was found but which did not indicate that the co-expression was needed in the carcinogenesis. CEA immunohistochemical staining may be helpful in the differential diagnosis of benign lesions and precancerous and cancerous lesions of the gallbladder. CONCLUSION: These results suggest that p53 mutations and c-erbB2 alterations may have a role in the carcinogenesis of gallbladder carcinomas, especially, in a late event, and in an early and late events, respectively.

10.Latent Mean Analysis of Health Behavior between Adolescents with a Health Problem and Those without: Using the 2009 Korean Youth Health Behavior Survey.

PURPOSE: The purpose of this study was to identify the construct equivalence of the general five factors of health behavior and to compare the latent means between adolescents with a health problem and those without in Korea. METHODS: The 2009 KYRBS (Korean Youth Risk Behavior Survey) data were used for the analysis. Multi-group confirmatory factor analysis was performed to test whether the scale had configural, metric, and scalar invariances across the existence of health problems in adolescents. RESULTS: Configural, metric, and factor invariances were satisfied for the latent mean analysis (LMA) between adolescents with health problem and those without. Adolescents with health problem and those without were not different in the LMA of all factors. CONCLUSION: Health providers should give more interest to the group of adolescents with health problems and consider prudential school life to the same group.
