1.Microplate Identification System of Enterobacteriaceae.

BACKGROUND: To access the accuracy and clinical usefulness of microplate identification (ID) system for the identification of Enterobacteriaceae, we compared microplate ID system with API 20E(bioMerieux, Etoile, France). METHODS: Ninety-two cultures of Enterobacteriaceae and one isolate of Aeromonas species were simultaneously identified by microplate ID system and the API 20E. Twenty biochemical tests used in microplate ID system were lactose, sucrose, and H2S in Kligler's iron agar media; indole, sucrose, raffinose, arabinose, trehalose, adonitol, dulcitol, sorbitol, cellibiose, methy-red, phenylalanine deaminase, ornithine decarboxylase, lysine decarboxylase, arginine dihydrolase, urease, and citrate in microplate; and oxidase test. The identification was obtained by considering percent likelihood(% ID), modal frequency and ID score method. RESULTS: Among the 92 cultures of Enterobacteriaceae and one isolate of Aeromonas species, agreement rate of identification according to the % ID between microplate ID system and API 20E were 90.3% to the species level and 97.8% to the genus level. CONCLUSIONS: For the identification of clinical Enterobacteriaceae isolates, the microplate ID system compares favorably with API 20E in identification accuracy and have the advantage of costsaving and easy to use.

2.Arterial Priapism: Superselective Embolization using Autologous Blood Clot.

A case of arterial priapism was treated by superselective transcatheteric autologous clot embolization of cavernosal artery 25 days after blunt periheal trauma. No immediate improvement was noted but the priapism subsided on 48th day after the procedure. Normal physiclogic erectlie function was achieved on 53rd day after the procedure, and the period of postprocedure impotence was shortened, as compared with previously reported cases. Autologous blood clot embolization may be an effective treatment method even in delayed priapism, and superselective technique is thought to be more effective to shorten the postprocedure erectlie dysfunction

3.A Case of Spontaneous Retroperitoneal Hemorrhage due to Iliopsoas Muscle Hematoma in Patient with Myocardial Infarction Receiving Intravenous Heparin.

The most common adverse effect of intravenous heparin is hemorrhage. Of the these, retroperitoneal hemorrhage and femoral neuropathy secondary to heparin anticoagulation has reported in 1966 for the first time by DeBolt and Jordan. We experienced a case of spontaneous retroperitoneal hemorrhage due to iliopsoas muscle hematoma in patient with myocardial infarction receiving intravenous heparin for a 3 days in therapeutic doses. The pathophysiology of iliopsoas muscle hematoma has not yet been cleared. But because of the possibility of large amount in volume, retroperitoneal hemorrhage has been known as a serious adverse effect that leads to the hypovolemic shock and death. In this article, we described the clinical manifestation, the importance of the early diagnosis and diagnostic clues and associated factors of the retroperitoneal hemorrhage in patient receiving intravenous heparin.

4.Osteochondral Lesion at the Proximal Phalangeal Base of the Hallux: A Case Report

Although osteochondral lesions are well-described in previous literature, lesions located at the proximal phalangeal base of the hallux are extremely rare. Here, the authors report the case of a 21-year-old male who presented with chronic right great toe pain, which was eventually diagnosed as an osteochondral lesion at the proximal phalangeal base of the hallux. The diagnosis presented herein should be kept in mind and included in the differential diagnosis when patients complain of chronic pain in the great toe.
5.Natural transition of endemicity of Malayan flariasis in inland Korea Pattern of change in microfilaria rate among inhabitants of Yongpung (former Yongju) area during the period of the last seven years.

With a purpose to find out natural transition of endemicity of Malayan filariasis in inland Korea, a survey was conducted in June 1980 in Isan-Myeon of Yongpung-Gun (former Yongju-Gun) where an epidemiological investigation had been carried out in 1973 without any control activities such as chemotherapy. Five sample villages were surveyed for microfilaremia by 20 microliter night blood examination among inhabitants and the results of the surveys conducted in 1973 and 1980 were compared to determine natural transition of the endemicity of malayan filariasis during the period of the last 7 years. The current microfilaria rate among inhabitants in the 5 villages was 2.2 percent on the average (male: 1.6 percent, female: 2.8 percent) from 370 persons examined. By village, the rates were 5.9 percent (number of persons examined: 34) in Baranggol, 0 percent (30) in Guitonggl, 4.2 percent (72) in Alseonggol, 0 percent(65) in Jangjagol and 1.8 percent (169) in Saehae. Extremely low microfilaria rate was noted in young age groups. By age group, no positive case was found in those age groups below 30-39 years except 10-14 age group in which 2 positives (4.4 percent) were found. Two positives each were found in the respective age groups of 40-49(3.2 percent), 50-59 (4.4 percent) and over 60 (3.4 percent). In evaluation of the natural transition of the endemicity during the period of the last 7 years, the microfilaria rate turned out from 13.1 percent in 1973 to 2.2 percent in 1980. The difference in the microfilaria rate was 10.9 percent and the natural reduction rate per year was 1.6 percent on the average. From the examination of 35 cases which had revealed microfilaremia 7 years ago, 85.7 percent(30) of them were found to have converted to microfilaria negatives. On the other hand, from the 151 cases which had revealed no microfilaria in 1973, only 0.7 percent (1) of them was found to have converted to microfilaria positive. In the intensity of microfilaremia, the number of microfilaria/20 microliter blood per positive case was 11.0 in 1973 and 9.1 in 1980. The number of microfilaria/20 microliter per examinee was 1.4 in 1973 and 0.2 in 1980, thus reduced to 1/7 during the period of the 7 years. The retarding endemicity of malayan filariasis in inland Korea was considered to be resulted in by the gradual increase of environmental factors in relation to ecology of vector mosquitoes, which adversely affect to the transmission of malayan filariasis. Followings are suggested to be the factors which control the transmission of the malayan filariasis in this area: Inhabitants are the only natural final host of Brugia malayi infection in this area, gradual elevation of living standards of the inhabitants, gradual awakening of consciousness and behavior among inhabitants to protect themselves from mosquito biting using such as mosquito nets and insecticide sprays, preference of animal bait of vector mosquitoes, Anopheles sinensis, increase in number of domestic animals and fowls being raised in the village areas which play a major part of blood donors to vector mosquitoes, and relatively short (3-4 months) period of mosquito season in a year.

6.Squamous Cell Carcinoma of the Larynx Arising in the Intracordal Cyst

More than half of patients presenting with hoarseness show benign vocal cord changes. However, in rare cases with benign mucosal lesions, it can be diagnosed as a malignant disease after histopathological examination. A 53-year-old man with a 30-pack-year smoking history was admitted for the evaluation of hoarseness, and using a laryngoscope, an enclosed, sac-like cystic lesion was detected on the midpoint of the right true vocal cord. The cystic lesion was deemed to be an intracordal cyst and treated with laryngeal microsurgery. However, pathological findings showed squamous cell carcinoma of the larynx arising in the intracordal cyst, which is exceptionally rare. Therefore, even if a benign lesion is initially suspected, a biopsy must be performed on a patient with smoking history to confirm the diagnosis. In conclusion, we report a case of squamous cell carcinoma of the larynx arising in the intracordal cyst.

8.Video Analysis on the Injury Mechanism of the World Taekwondo Championship Athletes

Purpose:
This study aimed to analyze the profiles and mechanisms of injuries using online injury surveillance system (OISS) with match video files at 2017 World Taekowndo Championship (WTC) athletes.
Methods:
This study design was a prospective epidemiological study with video analysis. All injuries were recorded during the 2017 WTC using the OISS developed by the International Olympic Committee. A total of 971 athletes who participated in the 2017 WTC were enrolled. Each injury was recorded retrospectively by three cameras surrounding each court. Injury profiles and mechanisms were calculated as; percent injury rate, injury rate/100 athlete-exposures, injury location, type, and mechanism via the injury surveillance system and video analysis form.
Results:
There were 74 injuries, which corresponded to an overall incidence of 13.5 injuries (95% confidence interval, 11.2–15.8) per 100 athletes. The face (27.0%), hand/finger (23.0%), knee (14.9%), and ankle (10.8%) were most frequently injured parts. Contusions (37.8%), fracture and ligament sprains (20.3%) were the most frequently injury types. The main mechanism for contact injury is while attacking with the roundhouse kick (66.2%), or not used block (81.1%). There were also noncontact injuries (24.3%).
Conclusion
The incidence of injuries to the face, hand/fingers, knee, and ankle was relatively high. To prevent contact injuries, protective headgear and better shin guards and hand protectors need to be developed. As for the noncontact injuries, we recommend hamstring strengthening exercises, and neuromuscular training to strengthen the tendons and ligaments protecting the ankle and knee joints.
9.A comparative clinical study on the accuracy and efficacy of Accumate(R) 1100, an infusion pump for patient-controlled analgesia.

BACKGROUND: The accuracy and efficacy of patient-controlled analgesia (PCA) devices are crucial because malfunction of the devices may lead to fatal outcomes. The aim of this study is to evaluate the accuracy and efficacy of Accumate(R) 1100, an electronic PCA pump, in comparison with GemStar(R) infusion system. METHODS: Fifty patients scheduled for gastrectomy or colon resection were assigned to the Accumate or GemStar group (n = 25 in each group). Intraoperatively, backflow, siphonage, autoclamp function, and lockout time were tested. Postoperatively, PCA using either Accumate(R) 1100 or GemStar(R) infusion system according to the assigned group was maintained for 48 hours. For PCA, fentanyl 1,500 ug and ketorolac 180 mg in 100 ml were used (demand bolus 1 ml, background infusion 1 ml/h, lockout time 15 minutes). The accuracy of the total infused dose and the level of satisfaction of the patients and users were assessed. The results were compared between the two groups. RESULTS: Although backflow and auto-clamp backflow were less with GemStar(R) infusion system, their extent was negligible in both groups. Siphonage was not different between both pumps. Autoclamp and lockout time worked without abnormalities. The relative error of total infused volume over 48 hours was -0.26 +/- 1.20% and -0.34 +/- 0.48% in the Accumate group and GemStar group, respectively. The Accumate group showed a higher level of satisfaction in terms of the ease of priming (P < 0.05). CONCLUSIONS: The accuracy and clinical efficacy of Accumate(R) 1100 were comparable to those of GemStar(R) infusion system.

10.Effect of Outflow Resistance on Airway Pressure during High Frequency Jet Ventilation.

High frequency ventilation considerably reduces the risk of barotrauma due to low peak airway pressure compared to conventional mechanical ventilation. This risk, however, is also preaent with high frequency jet ventilation (HFJV) if excessive driving preasure are used and, above all if expiration is impeded. We investigated the effects of outflow resistance, which was varied by connecting different size of tube (ID 8.0, 7.5, 7.0, 6.5, 5.5, 5.0, 4.5, 4.0 mm), which was cut in 10 cm length, to the proximal site of endotracheal tube (ID 8.0 mm), which was inserted into the trachea of anesthetized dogs with a attached airway pressure monitoring catheter externally, in different driving pressure (2 kg/cm2, 1 kg/cm2) and frequency (100beats/min, 200beats/min) on the intra-airway preesure during HFJV. HFJV was performed with a catheter (diameter 2.5 mm) which was inaerted through endotracheal tube and located 1 cm proximal to the tip of endotracheal tube. Intra-airway pressure was acutely increased with the tube size of smaller than 5.5 mm in driving pressure 2 kg/cm2 and 5.0 mm in driving pressure 1 kg/cm2 compared to previous size of tube. 2 kg/cm2 of driving pressure showed significant higher airway pressure compared to 1 kg/cm in any size of tube. There was no difference in airway pressure by varing of frequency with same driving preasure. In summary, pulmonary barotrauma due to higher airway pressure may be occur if HFJV catheter occupied more than 25% of outflow tract area especially in higher driving pressure.
