Delayed reattachment of the muscle to the sclera may be desirable to correct the angle of deviation developed after the surgery of strabismus. In order to know the possible time for the delayed adjustment, the authors tried Viscoat(R). Interceed(TC7) and polyglactin 910 mesh(Vicryl mesh(R)) as physical barriers for preventing the formation of postoperative adhesions. These materials were placed around between the detached end of the muscle and the sclera in 29 rabbits and adjustment was performed thereafter. Viscoat(R) was effective in delaying the adjustment up to 6 days after surgery. Interceed(TC7) and polyglactin 910 mesh were effective in delaying the adjustment up to 1 week after surgery.

Lebers hereditary optic neuropathy(LHON)is caused by a single nucleotide change in the mitochondrial deoxynucleic acid(mtDNA)and accounts for 30% of bilateral optic atrophy of unknown etiology. In order to define the clinical features of LHON associated with the 11778 mtDNA mutation, clinical and historical data were collected from 60 visually symptomatic patients with the molecularly confirmed mtDNA 11778 mutation. Forty-nine(82%)of the 60 patients were male. Onset of visual loss was between 6 and 63 years of age. Worst visual acuity ranged from light perception to 0.8. In most patients, visual acuities deteriorated to worse than 0.1 and remained at that level. Final visual acuities ranged from light perception to 1.2.Five(8.3%)out of 60 patients showed improvement of visual acuity up to 0.5 or better in at least one eye. Ishihara test revealed severe color vision impairment in most patients. Optic atrophy was the most common ophthalmoscopic finding.However, increased C/D ratio or normal disc was also observed. Central scotoma was the most frequent visual field defect, but normal visual field was also found. LHON should be considered in the differential diagnosis of optic neuropathy in a young man with visual and color deterioration as well as with a central scotoma. However, various phenotypes also exist in an atypical LHON case.

Polytetrafluoroethylene(PTFE) was placed around the cut end of the muscle as a physical barrier between the free muscle end and the sclera in 16 rabbits in order to prevent the postoperative adhesions. PTFE was effective in delaying the adjustment for up to 4 weeks after surgery in 4 out of 10 eyes. After removal of PTFE, adjustment was possible up to 12 weeks in 24 out of 25 eyes because there was no adhesion between the muscle and PTFE. In conclusion, PTFE may be clinically helpful for delaying the timing of adjustment.

In order to prevent the formation of postoperative adhesions and delay the time of adjustment, the authors tried 5-fluorouracil and physical barriers of polytetrafluoroethylene, Interceed, Viscoat in rabbits. Fluorouracil delayed the adjustment for up to one week after surgery in three out of four eyes. Combined use of Interceed and 5-fluorouracil of addition of Viscoat could delay the adjustment for up to two weeks after surgery I one out of four eyes. Combined use of pophtetrafluoroethylene and 5-fluorouracil of addition of Viscoat could delay the adjustment for up to four weeks after surgery in four out of five eyes. In conclusion, combined use of polytetrafluoroethylene, 5-fluorouracil and Viscoat could be more helpful for delayine adjustment in rabbits.

The inferior mesenteric artery is a rare source of postpartum hemorrhage. We report two cases of primary postpartum hemorrhage that originated from the inferior mesenteric artery after vaginal delivery. Both patients showed signs of hypovolemic shock, and disseminated intravascular coagulation was suspected. The bleeding continued even after embolization of the uterine artery, a typical source of postpartum hemorrhage. Inferior mesenteric arteriography confirmed contrast extravasation from the superior rectal artery, and selective embolization was performed using N-butyl cyanoacrylate. This report highlights that the inferior mesenteric artery can be a source of bleeding in patients with intractable and persistent postpartum hemorrhage due to birth canal injury.

Spontaneous bleeding due to vascular involvement of neurofibromatosis type 1 is rare but potentially fatal. Herein, we report a case of a lethal spontaneous hemomediastinum in a patient with neurofibromatosis type 1. The bleeding was caused by rupture of an aberrant bronchial artery arising from the ipsilateral subclavian artery, which was successfully treated using transarterial embolization with coils and N-butyl-2-cyanoacrylate.
