2.The Relationship Between Types of Smartphone Use, Digital Literacy, and Smartphone Addiction in the Elderly

Objective:
There is limited understanding of which specific factors relate to smartphone addiction in the elderly. The aim of this study was to examine the relationship between types of smartphone use, digital literacy, and smartphone addiction in the elderly.
Methods:
It used data from a survey on smartphone overdependence by the South Korea National Information Society Agency in 2019. Participants were 3,121 older people (1,627 males, 1,494 females; mean age=63.83 [standard deviation=2.88 years]). We conducted hierarchical multiple regression analysis.
Results:
The results of a hierarchical multiple regression analysis indicated that smartphone usage time was positively related to smartphone addiction. Entertainment-seeking and life service types of smartphone use, and digital literacy were positively associated with smartphone addiction. Conversely, smartphone use for information-seeking and communication-seeking was negatively related to smartphone addiction.
Conclusion
This study suggests that it is necessary to understand different types of smartphone uses in the elderly and to educate the elderly on digital literacy to prevent smartphone addiction. In future studies, it will be necessary to re-confirm the relationship between smartphone usage type, digital literacy, and smartphone addiction in the elderly (including participants in their 70s or older).
3.The Relationship Between Perceived Stress and Smartphone Addiction: The Mediating Effect of Rumination and The Mediated Moderating Effect of Mindfulness

Objective:
This study was to examine the mediated moderation effect of mindfulness through rumination on the relationship between perceived stress and smartphone addiction. In particular, this study examined the moderating effect of mindfulness in detail by dividing it into five sub-factors.
Methods:
An online self-report questionnaires were conducted on 697 participants aged 20 to 59. Finally, 681 participants (male=356, female=325) were included final analysis. Moderating effect, mediated moderating effect were verified using PROCESS macro for SPSS v3.5.
Results:
First, perceived stress was positively related to smartphone addiction. Second, rumination mediated the relationship between perceived stress and smartphone addiction. Third, acting with awareness and nonjudging of experience, which are a sub-factor of mindfulness, moderated the relationship between perceived stress and smartphone addiction. Fourth, mindfulness facets (acting with awareness and nonjudging of experience) moderated the relationship between rumination and smartphone addiction. Finally, there was a mediated moderating effect of mindfulness facets (acting with awareness and nonjudging of experience) on the relationship between perceived stress and smartphone addiction through rumination.
Conclusion
This research suggests the useful and specific therapeutic interventions that help lower the level of the adults’ addiction on smartphones.
4.Multiple Surgical Treatments for Recurrent Retroperitoneal Hemorrhage in a COVID-19 Patient with Respiratory Failure on Extracorporeal Membrane Oxygenation

Extracorporeal membrane oxygenation (ECMO) may be required in patients with corona virus disease-19 (COVID-19) and respiratory failure. Anticoagulation is the standard treatment to prevent complications of ECMO and COVID-19 coagulopathy, however, there is a risk of bleeding. Some patients with retroperitoneal hemorrhage (RPH) have been treated with angiography-embolization. We report on a patient with COVID-19 on ECMO who underwent multiple operations (×5) for recurrent RPH. A 46-year-old man was admitted with COVID-19 pneumonia. ECMO with anticoagulation therapy was initiated. The patient developed RPH, caused by external compression of the inferior vena cava interrupting the ECMO inflow, and surgical hematoma evacuation was performed, with no obvious bleeding focus during the multiple surgeries. Following the patient’s recovery, a follow-up computed tomography scan showed the hematoma had been resolved, but there was a dilemma regarding anticoagulation. Lowering the threshold for surgical treatment, enabled treatment of a patient with serious RPH.
5.Treatment of Acute Traumatic Patella Dislocation.

PURPOSE: To analyze clinical results of the cases diagnosed and treated as acute patella dislocation. MATERIALS AND METHODS: Thirteen cases diagnosed as acute patella dislocation between March 2000 and May 2006, were enrolled for this study. The mean follow-up period was 28 months. Preoperatively plain radiographs and MRIs were taken, and operative treatment was performed in 11 cases with osteochondral fragments or patella malalignment after reduction. Clinical results were assessed by the range of motion and the Lysholm score. For radiologic assessment, patella tilt by lateral patellofemoral angle and patella subluxation by congruence angle were observed. RESULTS: In all cases(100%), bone contusion was observed on MRIs. Osteochodral injury was found in six cases (46.1%). The postoperative mean range of motion was from 0 degree to 130 degrees, and the mean Lysholm score was 86 points. The congruence angle was recovered to -10.9 degrees from 1.7 degrees in 12 cases except one case. No case presented lateral patella tilt when the lateral patellofemoral angle was measured. CONCLUSION: Bone contusion on MRIs was helpful in diagnosing acute traumatic patella dislocation. Operative treatment is considered necessary for the cases with remained patellofemoral malalignment or osteochondral fragments after reduction.

6.Prevalence of PER-1 Extended-Spectrum beta-Lactamase-Producing Clinical Isolates of Acinetobacter baumannii in a University Hospital, Busan, Korea.

BACKGROUND: In recent years, Acinetobacter baumannii isolates acquired resistance to cefepime have increased significantly. The aim of this study was to survey the prevalence of PER-1 extendedspectrum beta -lactamase (ESBL)-producing A. baumannii isolates in a University Hospital, Busan, Korea. METHODS: Antimicrobial susceptibilities were tested by the disk diffusion method, and double disk synergy test was performed for screening of ESBL-production. MICs were determined by agar dilution method. The isoelectric points of beta -lactamases were determined by isoelectric focusing. Transferability of cefepime-resistance were tested by conjugation. blaPER-1 and blaPER-2 alleles were detected by PCR, and the DNA sequences of amplified products were determined by using the dideoxy-chain termination method. RESULTS: Among 51 clinical isolates of A. baumannii intermediate or resistant to cefepime, 10 isolates (19.6%) showed positive results in double disk synergy test. PCR-based experiments detected blaPER-1 gene in all the 10 isolates. All the isolates contained three beta -lactamase bands: pI 5.3, 7.9, and 9.4. MICs of ampicillin, piperacillin, cephalothin, cefoxitin, cefoperazone, ceftazidime, cefotaxime, cefepime, and aztreonam were >256 mg/L, respectively, and them of imipenem were 8-16 mg/L. CONCLUSION: The prevalence of PER-1-producing A. baumannii strains in Busan was less than that of in Seoul. But an outbreak of infection caused by this strain in an intensive care unit shows that spread of PER-1-producing A. baumannii strains can be anticipated in a near future. Prevention of hospital infection by these resistant microorganisms are needed.

A Case of primary malignant lymphoma in the uterine cervix of a 58-year-old multigravidwoman is reported. Primary malignant lymphoma localized in the uterine cervix is a rare condition.Previously, treatment of this disease has been radiation therapy, surgery or chemotherapyeither alone or in combination.We experienced one case of primary malignant lymphoma of the uterine cervix, stageI b according to FIGO, which was treated with radiation therapy and CHOP chemotherapy.The patient first underwent radiation therapy. This was followed by an chemotherapyconsisting of cyclophosphamide, adriamycin, vincristine, and prednisone.No evidence of recurrent lymphoma has been observed in 3 year after the treatment.

8.Thoracoscopic Sympathectomy for Essential Hyperhidrosis.

Essential hyperhidrosis is a condition with excessive sweating, which may be localized in any part of the body. Excessive sweating has a strong negative impact on the quality of life for many persons. From June 1992 to May 1996, 211 cases of thoracoscopic thoracic sympathectomy were performed in the Department of Thoracic Surgery, Yongdong Severance Hospital, Seoul, Korea. Among the 211 cases, 192 patients had palmar hyperhidrosis, and 19 cases had facial hyperhidrosis. There were 121 males and 90 females, and the ages ranged from 10 to 67 years(average: 24.82 years old). The average operation time and the average postoperative hospital stay were 91.94 minutes and 4.31 days, respectively. Perioperative courses were uneventful, and all the patients had immediate and complete relief of symptoms with mild compensatory sweating on the chest wall and the back. Even though a thoracoscopy has the possibility of emergency conversion to a thoracotomy and technical difficulties still exist, especially in patients with facial hyperhidrosis, our experience indicates that video-assisted thoracoscopic thoracic sympathectomy is a very safe and useful procedure for hyperhidrosis.

9.Percutaneous Balloon Valvuloplasty of Pulmonary Valvular Stenosis in Adult.

Percutaneous balloon valvuloplasty (PBV) was done in 7 adults of pulmonary valvular stenosis, which was diagnosed with catheterization and cine-angiography of rigtht heart, in Seoul National University Hospital from Jan. 1986 to Dec. 1986. The results were as follows : 1) Among the seven cases six were male and the range of age was from 19 to 40 years. Atrial septal defects associated in 2 cases were not significant clinically. 2) The peak systolic gradient from right ventricle to pulmonary artery before the procedure was below 50 mmhg in 1 cases, 50 to 100 mmHg in 1 case, and above 100 mmHg in 5 cases. 3) Before PBV the mean systolic pressure of right ventricle was 121+/-42 mmHg and the peak systolic gradient was 106+/-41 mmHg. After PBV those were 58+/-30 mmHg and 40+/-31 mmHg, revealing significant improvement. 4) Balloon catheter (Medi-tech, 9F) was used in all cases. single balloon of 20 mm outer diameter was applied in 2 cases and double balloons of various sizes were applied in 5 cases in which single balloon showed inadequate dilatation. It is suggested that percutaneous balloon valvuloplasty is safe and effective alternative to surgical valvotomy of pulmonary valvular stenosis in adults.

10.Theory of Mind Deficits in Schizophrenia Patients and Their First-Degree Relatives.

OBJECTIVES : Theory of mind (ToM) is the ability to conceptualize other people's mental states in order to explain their behavior. This study compared the ToM ability of schizophrenia patients, their first-degree biological relatives, and healthy controls. METHODS : ToM animation test was administered to schizophrenia patients (N=28), their healthy first-degree relatives (N=26), and healthy controls (N=28). The Korean version of schizotypal personality questionnaire (SPQ) was used to assess the schizotypal personality trait of all three groups. RESULTS : Schizophrenia patients and their first-degree relatives showed decreased accuracy on ToM animation test compared to healthy controls [F(2,78)=29.75, p<.001]. Additionally, first-degree relatives performed worse than the control group and better than schizophrenia patients. First-degree relatives had similar SPQ scores compared to healthy controls but showed decreased ToM accuracy compared to healthy controls. CONCLUSION : ToM deficits in schizophrenia patients are apparent. The ToM deficit in first-degree relatives of schizophrenia patients raises the possibility that ToM deficit may be a endophenotype for schizophrenia pathology.
