1.A Study of Relation Between Number of Daily Food Intake and Nutritional Balance.

During the period of May 10 to June 6, 1987, we have conducted the survey about the number of the food daily taken and the in take of nutrition of the 83 pregnant women who have registered at Sudong Myun and Mikum-myun, Yangchu-gun which are demonstration are of the public health and medicine of the College of Medicine, Ewha Women's University, and of the 100 pregnant women who have been to the University Hospital attached to the above School. The result of the survey is as follows: 1) Among the above pregnant women, primigravida constituted 91 women and multigravida 92 women. As for the month of pregnancy, those who have been pregnant for ten months numbered 43 women (23.5%), which was the highest number. As for age, those who were 25 to 29 years old constituted 63.9%, which was the highest in percentage. 2) As for the level of education, those who graduated from high school constituted 49.2%, which was the highest in percentage. As for occupation, those who were entirely in charge of household affairs as housewives were 89.6%, which comprised most of the jobs held by the above women. 3) As for the composition of a family in environmental food and nutrition, one generation was 68.3%, and two member family was 42.6% which was the highest. As for those taking care of cooking and serving meals, housewives constituted 95.1%. 4) As for the economic situation of dietary life, those who were in the highest level (A level), were 47.0% in an urban area and 41.0% in rural area, which were the highest in percentage. 5) The food which the above people selected after being pregnant was fruit not only in an urban area, but also in a rural area. And in terms of the percentage of fruit, and urban area and a rural area showed 44.0% and 54.2% respectively. As for milk, and urban area and a rural area showed 28.0% and 8.4% respectively, which disclosed that the rate of a rural area was much lower than that of an urban area. 6) As for the number of food intake, 10-14 was 54.0% in an urban area, which was the highest. And 5-9 was 73.5% in a rural area, which was the highest. 7) As for the composition of food taken according to the number of food taken, meat showed the lowest rate in 5-9 group. (P<0.05) As the number of food taken increased, the rate of food taken(Points) increased (2.4). Milk and milk products showed al low rate in 5-9 group and 15-19 group (P<0.05). Fruit intake showed a similar rate in all groups. As for vegetables, the rate of intake was high, as the number of food taken increased. Cereals showed a similar rate in all troups. And the points of intake was high. As for fat, hte rate of intake was high, as the number of food taken increased. 8) In 5-9 group in the number of food intake, energy was 79.9% of RDA, protein was 64.9%, fat was 90.0%, carbohydrate was 81.6%, iron was 83.0%, calcium was 71.1%, vitamin A was 94.3%, vitamin B1 was 77.8%, vitamin B2 was 79.2%, and vitamin C was 88.2%, so that it showed the lowest sufficiency rate of RDA. 15-19 group maintained the balance in energy and all nutrients nearly. In 20-24 group, fat was 142.4%, vitamin B2 was 105.6% and vitamin C was 102.2%, so that it exceeded RDA.

3.The clinical significance of serum CA125 and CA19-9 levels in endometriosis.

OBJECTIVE: Because endometriosis is difficult to diagnose and has a high recurrence rate after treatment, a reliable serum marker of endometriosis is necessary. Therefore, the aim of this study is to measure the serum levels of CA125 and CA19-9 in patients with endometriosis before and after treatment and during recurrence, and to assess the usefulness of these levels in the diagnosis, clinical follow up and prediction of recurrence in endometriosis. METHODS: Eighty-eight patients who visited the department of Obstetrics and Gynecology of Ewha Mokdong Hospital from January 1994 to December 1998 and were diagnosed as endometriosis by laparoscopy or explo-laparotomy were enrolled as subjects. A retrospective analysis of serum CA125 and CA19-9 levels at 1 month before and 3 to 6 months after initiation of treatment was done. RESULTS: The serum CA125 and CA19-9 levels of endometriosis group(81.0+/-252.5, 36.6+/-53.4 ; mean+/-2SD, U/ml) before treatment was significantly higher than control group(11.6+/-12.8, 9.4+/-8.6)(p<0.05). Overall sensitivity rate for CA125, CA19-9 levels and both was 53.4%, 42.9% and 64.3% respectively. The sensitivity rate for endometriosis, stage 3 and 4(85.4%, 55.0%) was significantly higher than that, stage 1 and 2(p<0.05). The serum CA125 level in endometriosis group showed a significant increment according to stages(p<0.05) while the serum CA19-9 level showed an increasing trend(p=0.055) and both levels decreased significantly after treatment(p<0.05). The serum CA125 level was also higher at recurrence after treatment(p<0.05). CONCLUSIONS: The serum CA125 and CA19-9 levels are a useful marker for diagnosing severity of disease, monitoring efficacy of treatment and predicting recurrence in endometriosis.

4.Effects of Sanhujori and Menopausal Adaptation on Health-related QOL in Middle-aged Women.

PURPOSE: This study investigated the effects of menopausal adaptation and Sanhujori (Korean traditional postnatal care) on the Health-related quality of life in middle-aged women. METHODS: Women aged from 45 to 60 completed questionnaires consisting of SF-36 ver. 2 and menopausal adaptation scale between July to Aug, 2013. Two hundred and eighteen participants' data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and multiple regression. RESULTS: Physical health (69.5+/-15.1) was better than mental health (68.8+/-17.1), and bodily pain and social functioning were the same results as 2009 general U.S. population, with the sub-variables of quality of life being lower. Menopausal adaptation was at moderate level, and correlation of its sub-variables with quality of life were significant: fatigue (r=.60, p<.001), accomplishing feeling (r=.55, p<.001), confidence (r=.54, p<.001), dryness (r=.51, p<.001), flush (r=.38, p<.001), dominance (r=-.36, p<.001), stability (r=.26, p<.001). Fatigue, confidence, stability, period of sanhujori, and dryness revealed as influencing quality of life, while the total variation explained by these components was at 52%. CONCLUSION: To improve quality of life of middle aged women, health care providers need to provide nursing intervention to relieve fatigue and dryness; and address psychosocial concerns with developing nursing strategies to improve stability and confidence. For post-partum women, enough periods of sanhujori needs to be recommended.

6.Health Behavior and Influencing Factors in Patients with Coronary Artery Disease Admitted to Hospital.

PURPOSE: The purpose of this study was to identify the impact that health motivation, stage of change and cardiac risk factors have on health behaviors in Korean patients with coronary artery disease(CAD) admitted to hospital. METHOD: The participants in this study were 127 patients with CAD hospitalized between May 2008 and July 2009. A structured questionnaire with personal interviews and chart reviews were used to collect data. Data were analyzed using the SPSS. RESULTS: Of the relatively significant factors, including a stage of change, self efficacy, modifiable risk factor score, and perceived barriers, stage of change was found to be the most significant predictor of health behavior in patients with CAD. These variables accounted for 48.2% of the variance in health behavior. CONCLUSION: The results of the study indicate the necessity of developing a cardiac rehabilitation program for use in their daily lives after discharge from the hospital. Stage of change should be assessed for all patients with CAD being discharged from acute care hospitals.

The purpose of this study is to develop the standards of care in rehabilitation nurse specialist. This study was a descriptive survey. The data were collected 143 nurses who were worked in rehabilitation unit at 4 university hospital and 3 rehabilitation center in Korea from Aug. to Nov. 2000. The questionnaire was consisted of 78 items developed by the standards of care in rehabilitation nurse specialist in A.R.N. and the practice contents of care at rehabilitation unit in Korea. Collected data were analyzed by frequency, percentage, mean, S.D. ANOVA The results are as follows 1. For the adequency of the practice contents of rehabilitation nursing, area of skin care is the highest score, and safety, elimination, emotion, respiration, was ordered. 2. The adequency of the practice contents of rehabilitation nursing according to age, educational level. position. clinical experience, clinical experience in rehabilitation unit were significant difference.

8.A Clinical Study on the Cardiac Toxicity of Adriamycin.

To clarify the exact role of endomyocardial biopsy in the diagnosis and monitoring of adriamycin-induced cardiotoxicity and to observe the actual relationship between pathologic changes and cardiac dysfunction, a cross-sectional clinical study was conducted. Echocardiography was used to evaluate cardiac dysfunction in 18 patients who had received chemotherapy including adriamycin(mean dose : 410mg/m2 of B.S.A.) without clinical evidence of congestive heart failure, and in 19 normal controls. Six patients receiving adriamycin underwent 7 transfemoral endomyocardial biopsy procedures, and the specimens were evaluated by light and electron microscopy for evidence of drug-related cardiotoxicity. Indexes of cardiac systolic function obtained by M-mode echocardiography(left ventricular dimension, excursion of interventricular septum and left ventricular posterior wall, shortening fraction and ejection fracton) did not show any statistically significant difference between patients who received adriamycin and normal controls. In transmitral flow-velocity curves recorded by Doppler echocardiography with a 2.25MHz probe, the patients showed less E peak velocity and decreased E/A ratio compared with normal controls, which suggests left ventricular diastolic dysfunction in the patients who received adriamycin. All the specimens of the endomyocardial biopsy showed significant pathologic changes of adriamycin indnced cardiotoxicity which was characterized by myofibrillar loss and vacuolization of the cytoplasm. In 2 specimens, pathologic grade was II, while 5 specimens showed pathologic changes of grade III and further chemotherapy with adriamycine was not done in thse 5 cases. From these results it is suggested that pathologic changes precede the clinical onset of congestive cardiomyopathy in the patients receiving adriamycin and left ventricular diastolic dysfunction occurrs before ejection fraction falls to subnormal levels. We conclude that sequential endomyocardial biopsy is absolutely indicated for exact diagnosis and monitoring of adrinamycin-induced cardiotoxicity to prevent the development of irreversible and often fatal cardiomyopathy.

9.Eccrine spiradenoma: A report of two cases.

Eccrine spiradenomas are clinically characterized by a solitary, tender mass and they are situated on the upper parts of the body, predominantly on the ventral aspect. We herein report two cases of eccrine spiradenoma in 35-year-old man and 53-year-old woman, which located on upper extremity and inguinal region. The masses are round, well circumscribed and measuring 0.7x0.5x0.5 cm, 5x4.5x3 cm in size, respectively. Histopathologically, the tumor consists of masses of two types of cells, intensely staining cells and pale staining cells, usually arranged in chains, cords and pseudoglands. Immunostainings for low molecular cytokeratin, high molecular cytokeratin, carcinoembryonic antigen, and S-100 protein show positivity in neoplastic cells. Electron microscopically, the tumor was composed of three types of cells, i. e. 1) round or ovoid tumor cells in shape with scanty cytoplasm and poorly developed intracytoplasmic orgenelles, 2) spindle shaped dark cells interconnected by desmosomes, 3) large epithelial cells with abundant cytoplasm and cytoplasmic intermediate filaments which formed glandular structures. The large epithelial cells joint each other by desmosomal attachments and luminal cells featured small numbers of microvilli, but either secretory granules nor ductal type granules were noticed.

10.Lethal pulmonary thromboembolism misdiagnosed as pneumonia: A case report.

Although pulmonary thromboembolism (PTE) is not rare, unfortunately for anesthesiologists, the signs and symptoms of PTE are unreliable and nonspecific. PTE is a potentially lethal condition without an accurate diagnosis and prompt treatment. We report a case of PTE misdiagnosed as simple pneumonia. A 60-year-old female, not receiving prophylactic anticoagulant therapy, underwent elective surgery for a left proximal tibial fracture. During induction of general anesthesia, a temporary bronchospasm occurred and subsided quickly. Because vital signs were stable and patient monitoring was normal, we did not identify a serious hypercarbic condition. After surgery, a massive hemoptysis occurred and the patient expired due to cardiopulmonary collapse. According to autopsy, the cause of death was a PTE originating in deep vein thrombosis.
