1.Complete Versus Partial-Thickness Tears of the Posterior Cruciate Ligament: Differential Features at MR Imaging.

PURPOSE: To evaluate the differential features of complete and partial- thickness tears of the posterior cruciate ligament(PCL) and to describe associated knee injuries at magnetic resonance imaging(MRI). MATERIALS AND METHODS: Twenty-four subjects were studied, including 15 with complete PCL tears and 9 with partial PCL tears. The PCL status was determined at arthroscopy in all cases. Knee MRI were perfomed at 1.0 Tesla using a dedicated knee surface coil. Two radiologists retrospectively reviewed MR images and correlated MRI findings to results of arthroscopy. RESULTS: There was no statistically significant difference between complete tears and partial tears with regard to thickness, margination, and signal intensity of the PCL. However, complete tears were more likely to show focal areas of ligamentous discontinuity(13/15:3/9, p=0.0073). Associated knee injuries were seen in 22 (92%) patients and were seen more frequently in patients with complete PCL tears, which were bony injuries (n=16, 67%), tears of the medial collateral ligament(n=12, 50%) and menisci(n--11, 46%), and stage III joint effusion(n=17, 71%). CONCLUSION: The most useful MRI criteria for distinguishing complete from partial PCL tear is focal area of ligamentous discontinuity, that is, complete tears are more likely to show focal areas of discontinuity, whereas partial tears are more likely to show at least some intact fibers.

Acute changes of the bilateral basal ganglia shown in the brain MRI a diabetic uremic patient have been rarely reported. We report a 52-year-old diabetic uremic patient who had neurological disturbances. At admission, axial diffusion-weighted images and T2-weighted images showed high signal intensities in the bilateral basal ganglia, and axial T1-weighted images visualized low signal intensities in same area. The underlying mechanism may be associated with metabolic as well as vascular factors.

3.A study of Cytokine in Peritoneal Fluid of Infertile Patients with Endometriosis.

OBJECTIVE: The presence of the various cytokines in human peritoneal fluid has been evaluated incompletely. Changes in cytokine lefels may be related to activation of peritoneal macrophage and T-lymphocyte, development of endometriosis, and infertility. This study assesses peritoneal fluid levels of interleukin-6 (IL-6), interleukin-8 (IL-8), interleukin-10 (IL-10) and tumor necrosis factor-alpha (TNF-alpha) in infertile women with endometriosis and normal women without endometriosis. Design : Prospective and case-control study in university hospital. MATERIALS AND METHODS: Cytokine levels in peritoneal fluid obtained during laparotomy or laparoscopy from 21 patients in infertile patients in infertile patients with endometriosis and 24 controls undergoing laparotomy or laparoscopy with no evidence of pelvic endometriosis were determined by enzymelinked immunosorbent assay. RESULTS: The mean levels of interleukin-6 in infertile patients with endometriosis and controls were 72.7+/-23.7 pg/ml and 18.5+/-9.7 pg/ml respectively (p=0.02). Similarly, the mean levels of interleukin-8 in infertile patients with endometriosis was significantly higher than that of controls (445.0+/-89.6, vs 45.1+/-48.4, p=0.04). The mean concentration of interleukin-10 in infertile patients with endometriosis was significantly lower than that of controls (1.09+/-0.04 vs 2.19+/-0.03, p=0.03). The level of tumor necrosis factor-alpha was not significantly different between the two study groups. CONCLUSIONS: Increased IL-6 and IL-8 and decreased IL-10 levels in the peritoneal fluid may be related to pathogenesis in the endometriosis and infertility, suggesting that partially contribute to the disturbed immune regulation observed in infertili women with endometriosis.

4.Relevance Epistasis Network of Gastritis for Intra-chromosomes in the Korea Associated Resource (KARE) Cohort Study.

Gastritis is a common but a serious disease with a potential risk of developing carcinoma. Helicobacter pylori infection is reported as the most common cause of gastritis, but other genetic and genomic factors exist, especially single-nucleotide polymorphisms (SNPs). Association studies between SNPs and gastritis disease are important, but results on epistatic interactions from multiple SNPs are rarely found in previous genome-wide association (GWA) studies. In this study, we performed computational GWA case-control studies for gastritis in Korea Associated Resource (KARE) data. By transforming the resulting SNP epistasis network into a gene-gene epistasis network, we also identified potential gene-gene interaction factors that affect the susceptibility to gastritis.

5.Surgical Outcome of Minimal Resection with Full Thickness Rotating Suture Technique for Lower Lid Epiblepharon.

PURPOSE: To introduce the minimal skin and orbicularis oculi muscle resection with the full thickness rotating suture technique for lower lid epiblepharon and to assess its surgical outcome. METHODS: A retrospective review of medical records was performed on lower lid epiblepharon patients who were followed for more than 6 months after surgical correction performed between January 2004 and December 2015. All surgeries were performed by one surgeon using minimal skin and orbicularis oculi muscle resection and the full thickness rotating suture technique for lower lid epiblepharon correction. RESULTS: A total of 943 lower lid epiblepharon patients (403 male, 540 female) were included in the analysis. The mean patient age was 6.7 ± 2.4 years, and the mean postoperative follow-up was 12.9 ± 7.2 months. Eyelid shape and function were well maintained in 904 patients (95.9%), with no recurrence during follow-up. Among the recurrent cases (39 patients [4.1%]), 19 patients (2%) underwent a second correction surgery. CONCLUSIONS: Minimal skin and orbicularis oculi muscle resection with the full thickness rotating suture technique for lower lid epiblepharon showed good surgical outcome with few complications and high success rate.

6.Perceptions on Pharmacy Students on Consumers’ Demand for Transparent Dispensing Room in Community Pharmacies

Background:
Dispensing of prescription drugs is the most important task for community pharmacists. However, the public’s satisfaction with pharmacist services is not high, and distrust due to dispensing by unqualified person and/or under unsanitary conditions led to demands for transparent dispensing room (TDR) in pharmacies.
Objective:
This study was conducted to investigate how pharmacy students perceive pharmacists’ credibility and professional ethics in conjunction with the TDR issue.
Methods:
A survey using 20-items questionnaire was conducted from July 12 to 20, 2019 in pharmacy students nationwide.
Results:
Among 218 respondents, 84.1% attended pharmacy school located in the metropolitan area, 61.1% were the 3rd or 4th graders, and 81.2% had no practical training in community pharmacy. 56.2% were unaware of the TDR issue, and 66.8% agreed on the public’s demand for TDR. 68.8% disagreed that TDR was the best way to solve the problem of unqualified dispensing, while 51.9% agreed that TDR was the best way in solving unsanitary dispensing problem. Publics’ confidence level of community pharmacists was mean 5.84 (range 1-10). In a question asking how expertise and professional ethics affect pharmacist confidence, 50% said expertise was more important, 31.7% the same, and 18.3% said professional ethics was more important.
Conclusions
To overcome the demand for TDR which began with public distrust in pharmacists, it is necessary to seek measures to increase the credibility of pharmacists, and as part of this, pharmacist ethics should be systematically educated at pharmacy school.
7.Role of Shear Wave Elastography in Evaluating the Risk of Hepatocellular Carcinoma in Patients with Chronic Hepatitis B.

BACKGROUND/AIMS: To investigate the use of measurements of liver stiffness (LS) by two-dimensional real-time shear wave elastography (SWE) for predicting the development of hepatocellular carcinoma (HCC) in patients with chronic hepatitis B (CHB). METHODS: We retrospectively collected data on 291 enrolled patients with CHB whose LS had been measured using SWE. RESULTS: The mean age of the patients was 46.8 years; males predominated (67%), and 40 of the patients (14%) had clinical cirrhosis. Among the patients, 165 (56.7%) received antiviral treatment. The median LS value was 7.4 kPa, and the median follow-up period was 35.8 months (range, 3.0 to 52.8 months). During follow-up, HCC developed in 13 patients (4.5%), and the cumulative incidence rates of HCC at 1, 2, and 4 years were 1.1%, 3.6%, and 8.4%, respectively. Based on a multivariate analysis, older age (≥50 years) and higher LS value (≥10 kPa) were independently associated with the risk of developing HCC (hazard ratio [HR], 4.53, p=0.023; and HR, 4.08, p=0.022). The cumulative incidence rate of HCC was significantly higher in patients with higher LS values (≥10 kPa) than in those with lower LS values ( < 10 kPa) (p=0.001). CONCLUSIONS: Increased LS measured by SWE at any time point regardless of antiviral treatment is associated with an increased risk of HCC in patients with CHB.

8.Determination of Therapeutic Dose of I-131 for First High Dose Radioiodine Therapy in Patients with Differentiated Thyroid Cancer: Comparison of Usefulness between Pathological Staging, Serum Thyroglobulin Level and Finding of I-123 Whole Body Scan.

PURPOSE: Recently, a number of patients needed total thyroidectomy and high dose radioiodine therapy (HD-RAI) get increased more. The aim of this study is to evaluate whether pathological staging (PS) and serum thyroglobulin (sTG) level could replace the diagnostic I-123 scan for the determination of therapeutic dose of HD-RAI in patients with differentiated thyroid cancer. Materials and METHODS: Fifty eight patients (M:F=13;45, age 44.5+/-11.5 yrs) who underwent total thyroidectomy and central or regional lymph node dissection due to differentiated thyroid cancer were enrolled. Diagnostic scan of I-123 and sTG assay were also performed on off state of thyroid hormone. The therapeutic doses of I-131 (TD) were determined by the extent of uptakes on diagnostic I-123 scan as a gold standard. PS was graded by the criteria recommended in 6th edition of AJCC cancer staging manual except consideration of age. For comparison of the determination of therapeutic doses, PS and sTG were compared with the results of I-123 scan. RESULTS: All patients were underwent HD-RAI. Among them, five patients (8.6%) were treated with 100 mCi of I-131, fourty three (74.1%) with 150 mCi, six (10.3%) with 180 mCi, three (5.2%) with 200 mCi, and one (1.7%) with 250 mCi, respectively. On the assessment of PS, average TDs were 154+/-25 mCi in stage I (n=9), 175+/-50 mCi in stage II (n=4), 149+/-21 mCi in stage III (n=38), and 161+/-20 mCi in stage IV (n=7). The statistical significance was not shown between PS and TD (p=0.169). Among fifty two patients who had available sTG, 25 patients (48.1%) having below 2 ng/mL of sTG were treated with 149+/-26 mCi of I-131, 9 patients (17.3%) having 2< or =sTG <5 ng/mL with 156+/-17 mCi, 5 patients (9.6%) having 5< or =sTG<10 ng/mL with 156+/-13 mCi, 7 patients (13.5%) having 10< or =sTG<50 ng/mL with 147+/-24 mCi, and 6 patients (11.5%) having above 50 ng/mL with 175+/-42 mCi. The statistical significance between sTG level and TD (p=0.252) was not shown. CONCLUSION: In conclusion, PS and sTG could not replace the determination of TD using I-123 scan for first HD-RAI in patients with differentiated thyroid cancer.

9.Quantitative Analysis of Bulbocavernosus Reflex Activity in Patients with Spinal Cord Injury.

OBJECTIVE: To quantify activities of bulbocavernosus reflex (BCR) using root mean square (RMS) value of motor unit action potentials in power spectrum analysis. METHOD: Twenty-six male patients with spinal cord injury were studied. The BCRs were evaluated by one examiner. The patients were assigned into absent, hypoactive and normal reflex groups according to the degree of BCR activity. The electrophysiological activities of the BCRs were evaluated by the RMS value of the motor unit action potentials in power spectrum analysis recorded from the bulbocavernosus muscle using concentric needle electrode. RESULTS: The BCR by clinical examination was absent in 6 patients, hypoactive in 10 patients and normal in 10 patients. RMS values of absent, hypoactive and normal BCR groups were 15.1+/-3.6 uV, 26.7+/-13.2 uV and 36.3+/-4.8 uV, respectively. RMS values of three groups were significant different (p=0.028) and showed significant correlation (r=0.714, p= 0.005). CONCLUSION: There was strong correlation between RMS value of motor unit action potentials of bulbocavernosus muscle and degree of BCR activities estimated by clinical examination in patients with spinal cord injury.

10.Impact of Delirium on Clinical Outcomes in Intensive Care Unit Patients: An Observational Study in a Korean General Hospital.

OBJECTIVES: The purpose of this study is to evaluate the impact of delirium on clinical outcomes in intensive care unit (ICU) patients in a Korean general hospital. METHODS: All patients admitted to ICU from March 1, 2013 to October 31, 2013 were assessed by Confusion Assessment Method adapted for use in the ICU for delirium daily, and consistently comatose patients were excluded for analysis. Differences in clinical outcomes (mortality, length of hospital stay, length of ICU stay) were analyzed between delirious patients and non-delirious patients. Subsequently, the impact of delirium on clinical outcomes was analyzed with adjusting for covariates including surgery, age, emergent admission, presence of surgery, and severity of illness. RESULTS: The analysis included 129 delirious patients and 469 non-delirious patients. As primary outcome, mortality, length of stay (hospital day, ICU-stay) were significantly high in the delirious group. The association remained the same after adjusting for the covariates. CONCLUSION: The results correspond with those of previous research studies conducted in foreign ICU. Based on this observation that delirium also has an impact on clinical outcomes in Korean ICU, integrative and in-depth investigation on ICU delirium will be needed.
