1.Misdiagnosed Breast Cancer on Mammography Retrospective Analysis in 17 Cases.

PURPOSE: Misdiagnosed breast cancer on screening mammography was retrospectively reviewed in an attempt to analyze the cause for the undiscovery and to determine findings that might have been suggested on the mammographic examination. MATERIALS AND METHODS: Of the 173 breast cancers that were pathologically proven, 17 breast cancers misdiagnosed on mammogram was retrospectively analyzed for the causes of the misinterpretation and predominant secondary findings. RESULTS: Three of the 17 were negative on mammography except for dense breast. Two patients had localized asymmetricity without visible mass. Ten patients had benign-looking mass which was well-defined (n=5), partially defined(n=2), multiple (n=l), or included benign-looking calcifications(n=2). Remaining two cases were retrospectively diagnosed as misinterpreted breast cancers. Various secondary findings such as were useful in the diagnosis of breast cancer. CONCLUSION: Meticulous observation could improve the diagnostic yield of breast cancer in mammography.

2.Association of Adiponectin and Hepatic Steatosis in Adults with Normal Transaminase Levels.

BACKGROUND: Nonalcoholic fatty liver disease (NAFLD) is associated with metabolic syndrome including insulin resistance, hypertension, dyslipidemia, central obesity, type 2 diabetes, and cardiovascular disease. In NAFLD, insulin resistance plays an important role in the progression of liver damage. In this study, we evaluated insulin resistance, plasma adiponectin, hepatic steatosis, and their association in adults with normal liver transaminase concentrations. METHODS: We analyzed 111 subjects, aged over 20 years old, who visited the Health Management Center at Chung-Ang University Hospital between May 2006 and August 2006. They had neither history nor clinical evidence of diabetes, cardiovascular or liver disease. They were divided into three groups by the degree of hepatic steatosis based on ultrasound findings. Anthropometric parameters were measured and blood samples were drawn after eight hours of fasting. RESULTS: Hepatic steatosis had a positive correlation with body mass index, waist circumference, blood pressure, triglycerides, HOMA-IR, and QUICKI. Moreover, it had a negative correlation with high-density lipoprotein cholesterol and adiponectin. Triglycerides, Adiponectin, and QUICKI were independent variables in predicting the degree of hepatic steatosis. CONCLUSION: This finding suggests that triglycerides and plasma adiponectin are independent predictors of hepatic steatosis in adults with normal liver transaminase concentrations.

5.A Case of Kimura's Disease Presenting as a Rhinophyma-like Configuration.

Kimura's disease is a benign, uncommon, chronic inflammatory condition that usually presents with painless subcutaneous nodules or plaques. Head and neck are the most frequently involved sites in Kimura's disease. Mandible is the most commonly involved, followed by neck, cheek, scalp and forehead. Other possible sites are oral cavity, inguinal area and extremities, but there have been no reports involving the nose, especially the one that looks like a rhinophyma. We describe a case of Kimura's disease presenting like a rhinophyma.

6.Two Case of Twin Pregnancy with a Single Anomalous Fetus.

Twin pregnancy is at increased risk for congenital anomalies. Although twins were relatively infrequent, they accaunted for a disproportionately large portion of adverse pregnancy outcome, primarily as a consequence of preterm delivery, gestational diabetes, and pregnancy induced hypertension. In case of twin pregnancy with a single anomalous fetus, the clinicians are faced with difficult decision-making processes regarding the route of delivery, when to deliver, how aggressively to attempt to prevent delivery. Twin pregnancy discordant for a fetal abnormality can be managed expectantly or by selective fetocide of abnormal twin. Recently we experienced two cases of twin pregnancy with a single anornalous fetus. We reported cases with concerned literatures.

7.Comparison of Orbital Anatomy in Korean and Caucasian Patients Using Computed Tomography.

PURPOSE: In this study we analyzed and compared the orbital anatomy in Korean and Caucasian subjects using computed tomography (CT) measurements. METHODS: Two observers performed a cross-sectional, retrospective analysis of 44 CT scans of subjects (22 Koreans, 22 Caucasians) with no appreciable orbital or globe disease. Ten length measurements and 3 angle measurements of various orbital aspects were obtained. Analysis was performed to determine if changes in these parameters were associated with race. RESULTS: Anterior medial interorbital length was 24.05 +/- 2.00 mm in Korean and 21.96 +/- 1.96 mm in Caucasian subjects. Anterior vertical orbital length was 34.19 +/- 1.67 mm in Korean and 35.03 +/- 1.18 mm in Caucasian subjects. The anterior medial interorbital length and anterior vertical orbital length (p < 0.05) were significantly different. Interorbital angle was 47.69degrees +/- 1.49degrees in Korean and 46.15degrees +/- 2.19degrees in Caucasian subjects; the difference was statistically significant (p < 0.05). CONCLUSIONS: Compared with Caucasians, the orbit of Korean subjects has a narrower orbital opening and longer interorbital distance. The present study regarding the anatomy of Korean and Caucasian orbits will assist physicians with the evaluation and treatment of orbital diseases.

8.SINGLE TOOTH IMPLANT RESTORATION USING COMBINATION IMPLANT CROWN : A CASE REPORT.

The purpose of this article is to present the clinical and laboratory procedures for single tooth restoration using "Combination Implant Crown". It is cemented on implant abutment and that abutment is screw-retained over implant body. This type of implant restorations has the advantages of cement-retained restoration while being antirotational and retrievable. And, more esthetic and functional result can be achieved by minimizing the size of access hole. The results were as follow : 1. Preparation of abutment below the cuffline should be avoided 2. Axial reduction of implant abutment should not be excessive because it may weaken the abutment 3. More esthetical and functional occlusal surface was achieved with a minimal access hole which is slightly larger than the diameter of hex driver to enable future total retrievability. 4. Combination Implant Crown has the advantages of both the cement-retained and screw-retained type implant restoration. 5. Cementation between implant crown and abutment reduces screw loosening through even force distribution.

9.A Case of Relapsing Polychondritis.

Relapsing polychondritis is a rare inflammatory disorder affecting the cartilaginous structures throughout the body. Although there is no pathognomonic laboratory tests, clinical features, when coupled with histopathologic findings, enable one to make a diagnosis of relapsing polychondritis. A 26-year-old man presented with purplish, swollen, and tender auricles and injected eyes. His symptoms had developed eight months ago and fluctuated irrespective of various medications such as corticosteroids and antibiotics. Histopathologic examination revealed loss of basophilic staining of the cartilage with some dissolution of its structures. Treatment was initiated with dapsone, However, the remission was too immediate to be considered as a effect of dapsone.

10.Quantitative Analysis of p53 Expression in Carcinoma of the Uterine Cervix.

This studly was perormed 10 define the role of compulerized quantitative analysis in ewalualion for overexpression of p53 by immunohistochemistry. Total and ladeled cells wers counted automatically using commercially available software for color-image analysis. In 16 uterine cerwical carcinomas, the p53 ladeling index calculted bycomputerized5quantitatiwe analysis was 9.22%+/-8.70% and by visual analysis 5.90+/-6.51%. The present results suggest that the computerized quantitative analysis may be valuadle in objective interetation of immunohistochemical expression of p53 and reliable than conventional ways of visual analysis.
