1.Relationship between Pulmonary Function and Apnea-Hypopnea Index in Asthmatic Children: The Preliminary Study.

OBJECTIVE: It has been reported that the sleep apnea syndrome in the asthmatic patients is prevalent, however, the systematic study in this field using polysomnography has rarely been performed. The aim of this study is to investigate the relationship between the apnea-hypopnea index (AHI) and the pulmonary function in asthmatic children. METHODS: This study enrolled 19 male and 12 female asthmatic children aged 6-13 years (average 8.2+/-1.7 years old). Complete overnight polysomnography and pulmonary function test were performed for the participants. RESULTS: Of the 31 asthmatic children, 21 (67.7%) met the diagnostic criteria of the pediatric sleep apnea and the average AHI was 1.7+/-1.5/h. The children with higher AHI showed poorer pulmonary function (FEV1/FVC ratio : p=0.002, FEV1%pred : p=0.047). CONCLUSION: These results suggest that the prevalence of the pediatric sleep apnea could be very high among the asthmatic children and the severity of the sleep apnea correlates with the pulmonary function. However, the case-control study to compare the AHI between the asthma and control groups is absolutely necessary because few normative data are available for the children.

2.Local Complications Associated with Vascular and Endovascular Surgery.

Vascular surgeons encounter a myriad of complications during the management of patients. The complications that occur associated with vascular surgery are often life threatening. Vascular surgeons have played a pioneering role in the development and introduction of new vascular and endovascular techniques, and some of the techniques changed the therapeutic options available for patients with vascular disease. Especially, with the development of endovascular surgery, there has been substantial change in the management of most vascular diseases and their complications. The best way to manage complications is to prevent them from occurring by careful planning, appropriate patient selection, extensive operator experience, and the correct use of appropriate devices. Successful operation outcomes depend on avoiding complications, as well as recognizing and promptly managing complications when they occur. This article reviews various local complications and their management in the field of vascular and endovascular surgery.

4.Sacral Dermatomal Somatosensory Evoked Potentials Its Techniques and Clinical Significance.

Pudendal nerve somatosensory evoked potential (PSEP) study has been utilized for the evaluation of neurogenic dysfunctions of bowel, bladder and sex. However, the reluctance of sexual organ exposure during the study can be a serions limiting factor. Sacral dermatomal somatosensory evoked potential (SDSEP) study stimulating sacral dermatome can be an alternative or a supportive method for the PSEP study. The purpose of this study is to present the techniques, normal values and clinical significance of SDSEP study in spinal cord injured patients. Thirty control subjects and thirty-five spinal cord injured patients were enrolled for the study. Using ring electrodes, S3 dermatome was stimulated by Nicolet Viking IV EMG/EP system. Evoked responses were recorded at the cortex(Cz'-Fz) by the needle electrodes. The latencies and amplitudes of SDSEP and PSEP responses were obtained and analyzed. In control subjects, the P1 latencies of SDSEP were 34.72+/-2.68 msec for the right and 33.54+/-1.95 msec for the left. The N1 latencies were 43.06+/-2.31 msec for the right and 42.14+/-2.29 msec for the left, respectively. The P1N1 amplitudes of control subjects were 0.73+/-0.40 V for the right and 0.69+/-0.22 V for the left. The coincidence of SDSEP and PSEP was 86.7% of the spinal cord injured patients. In conclusion, SDSEP study could be used for the evaluation of neurogenic dysfunctions of bladder, bowel, and sex in conjunction with the PSEP study.

5.Treatment of Peripheral Artery Pseudoaneurysm: Three Case Reports.

The pseudoaneurysms are resulted from complication of vascular catheterization, trauma, etc. and recently, the reports on pseudoaneurysms have been increasing. Successful treatment of pseudoaneurysms have been described usingdirect compression guided by color doppler ultrasound, instead of invasive surgical treatment. Authors experienced three cases of pseudoaneurysm ; two resulted from post-catheterization and one from trauma. We successfully treated two post-catheterization cases using C-clamp compression under the color doppler US-guidance. The traumatic case was treated by embolization instead of US-guided compression. We suggest that the compression of pseudoaneurysm using C-clamp under the US-guidance is more useful than hand or probe compression method.

6.A study on Evaluating Solute Excretion in the Normal Neonate.

Fine needle aspiration biopsy cytology (FNA) is an important diagnostic tool in the management of thyroid nodule. Especially, papillary carcinoma of the thyroid has distinct morphologic features that allow a definite cytologic diagnosis with high degree of accuracy. We examined the characteristic cytologic features of 57 cases of papillary carcinoma of the thyroid, and their frequency and diagnostic significance were evaluated. The results obtained are summerized as follows : 1. In pattern of cellular arrangement, papillary structure with or without stroma is predominant feature (96%). 2. In individual cell morphology, grooved nuclei (95%), intranuclear cytoplasmic invagination (89%) and nuclear lobulation(74%) are most frequent and important cytologic findings. 3. Chromatin pattern is usually fine. Coarse chromatin is infrequent finding (37%). Nucleoli are inconspicuous. Cytoplasm is plump and distinctly eosinophilic. 4. Psammoma bodies are identified only in 4 cases (7%), but they are considered as helpful diagnostic features. 5. There are other associated findings including multinucleated giant cells (51%), macrophages (37%) and cystic degeneration(16%).

7.A Case of McCune-Albright Syndrome with Vitamin D Resistant Rickets.

We present the cytologic features of small cell neuroendocrine carcinoma of the liver metastasized from the uterine cervix. Cytologically, tumor cells were arranged in a pat- tern of solid sheet in necrotic background. The tumor cells were characterized by uniform, small cells, round hyperchromatic nuclei, and high nuclear cytoplasmic ratio. The smears showed frequent mitotic figures and rosette formation, These findings were identified with the previous histologic sections of uterine cervix. To make a diagnosis of metastatic small cell neuroendocrine carcinoma on the Papanicolaou smear, a high index of suspicion and careful review of clinical history are needed

10.Korean Urologist's View of Practice Patterns in Diagnosis and Management of Benign Prostatic Hyperplasia: A Nationwide Survey.

PURPOSE: In Korea, there was no specific guidelines for the management of benign prostatic hyperplasia (BPH). We reviewed the practice patterns of Korean urologists in the management of BPH and aimed to describe the need to develop specific guidelines. MATERIALS AND METHODS: A probability sample was taken from the Korean Urological Association Registry of Physicians, and a structured questionnaire, that explored practice patterns in the management of BPH, was mailed to a random sample of 251 Korean urologists. RESULTS: For the initial evaluation of BPH, most urologists routinely performed prostatic specific antigen (PSA) (96.4%), digital rectal exam (94.4%), international prostate symptom score (IPSS) (83.2%) and transrectal ultrasound (79.2%). Symptom assessment (36.4%) followed by transrectal ultrasound of prostate (TRUS) (20.0%) was considered as the most important diagnostic examination affecting the decision about individual treatment options. Almost all urologists (92.2%) chose medical treatment as the first-line treatment option for uncomplicated BPH with moderate symptoms. Of the respondents, 57.2% had prescribed alpha blocker and 41.6% alpha blocker plus 5-alpha reductase inhibitors as the medical treatment option for BPH. The prescription of 5-ARIs was dependent on the size of the prostate and the severity of symptoms. CONCLUSION: The results of our current survey provide useful insight into variations in the clinical practice of Korean urologists. They also indicate the need to develop further practical guidelines based on solid clinical data and to ensure that these guidelines are widely promoted and accepted by the urological community.
