2.Difference between before and after Compensating for Cyclotorsion by Optical Coherence Tomography Guided Progression Analysis

Purpose:
This study evaluated the difference in parameters before and after the application of optical coherence tomography (OCT) Guided Progression Analysis (GPA) to compensate for cyclotorsion during image acquisition.
Methods:
Among glaucomatous eyes with ≥4 OCT tests, eyes with correction for cyclotorsion in GPA printouts defined as rotated OCT fundus images were included. The degree of cyclotorsion (i.e., the degree of fundus image rotation), direction of cyclotorsion (incyclotorsion or excyclotorsion), difference in optic nerve head (ONH) parameters, and retinal nerve fiber layer (RNFL) thickness before and after correction were analyzed.
Results:
Correction for cyclotorsion was seen in 33 of the 401 eyes (12.2%). Incyclotorsion was observed in 16 eyes (48.5%) and excyclotorsion in 17 (51.5%). The mean degree of cyclotorsion was 5.5 ± 2.3 degrees. The ONH parameters were unchanged after correcting for cyclotorsion. The mean overall, inferior, and superior quadrant RNFL thicknesses were 73.6 ± 14.8, 79.3 ± 26.3, and 90.1 ± 24.7 μm before correction and 73.6 ± 14.9, 79.6 ± 26.6, 89.7 ± 24.8 μm afterwards (p > 0.05). The difference in RNFL thickness after correction ranged from 0 to 6 μm.
Conclusions
The difference in RNFL thickness before and after cyclotorsion correction by GPA was not large. GPA did not correct the ONH parameters induced by cyclotorsion. Regarding this, GPA algorithm improvements may be needed.

4.Effects of Daily Chlorhexidine Bathing on the Acquisition of Multidrug-resistant Organisms and Healthcare-associated Infection in an Intensive Care Unit

PURPOSE: The aim of this study was to verify the effects of daily 2% chlorhexidine gluconate (CHG) bathing on the acquisition of multidrug-resistant organisms (MDRO) and healthcare-associated infection (HAI) in a medical intensive care unit (MICU). METHODS: The study was a randomized controlled group posttest only design, involving 91 patients in MICU at a tertiary hospital (47 patients in the experimental group and 44 patients in the control group). The 2% CHG bathing was performed daily according to bathing protocol to the patients in the experimental group, and traditional bath was performed every three days to those in the control group. Fisher's exact test and χ² test were used to analyze the data. RESULTS: MDRO were found in 6 patients of the experimental group and in 15 patients of the control group. The difference was statistically significant (p=.016). HAI occurred in 2 patients of the experimental group and in 7 patients of the control group. The difference was not statistically significant (p=.084). CONCLUSION: The results confirmed that daily bathing with CHG was effective in reducing the incidence of MDRO acquisition. Therefore, it is expected that daily bathing with CHG will be used as an effective nursing intervention to reduce the incidence of MDRO acquisition.

5.Progression of Glaucoma in Highly Myopic Eyes with Paravascular Inner Retinal Defects

Purpose:
To report a case of glaucoma progression in highly myopic eyes with paravascular inner retinal defects.Case summary: Retinal nerve fiber layer (RNFL) defects with cleavage-like irregular margins and internal areas were found around the retinal vessels in highly myopic eyes of a 17-year-old female. On suspicion of paravascular inner retinal defect (PIRD) instead of glaucomatous RNFL defect, observation without treatment was recommended. After 10 years of follow-up, RNFL defects and neuroretinal rim thinning were found around the pre-existing PIRD on fundus photographs. Optical coherence tomography also revealed decreased RNFL thickness.
Conclusions
Glaucomatous changes progressed around the pre-existing PIRD area over the 10-year follow-up. This indicates the need for caution when assessing glaucoma in highly myopic eyes with PIRD.
9.Human Cellular Immune Responses to the Aqueous Fraction of the TSP Antigen of Mycobacterium tuberculosis H37Rv.

Phase-partitioning with Triton X-114 (TX114) was applied to the TSP antigen, which may be preferentially associated with the cell wall of M. tuberculosis. The hydrophilic protein components of the TSP antigen were successfully separated from integral hydrophobic macromolecules. To further characterize and examine the cellular immune response of the aqueous fraction of the TSP antigen (TSPa), the in vitro properties of the antigen were measured by lymphoproliferation; surface expression of IL-2 Ra on T lymphocytes was analyzed by flow cytometry; and the cytokine mRNA expression pattern was determined by RT-PCR. Significant lymphoproliferative responses to the TSPa antigen were observed in healthy tuberculin reactive donors after a 5 day in vitro stimulation. TSPa treatment of PBMCs from healthy tuberculin positive subjects for 5 days resulted in progressive augmentation of IFN-r, II 2, and IL-2Ra mRNA expression, as measured by RT-PCR, but considerably reduced IL-4 mRNA expression. In addition, the TSPa antigen stimulated more IL-12 p40 mRNA production than did the PPD antigen, and graduaBy suppressed IL- 10 mRNA expression. Moreover, the CD3' T cells of tuberculin positive subjects displayed a profound increase in their expression of the II 2Ru protein (39.0%) in response to the TSPa antigen. Proliferation was correlated with IL-2 and IL-2Ra mRNAs, but not correlated with distinct IFN-r or IL-12 p40 mRNA production. These findings strongly suggest that the TSPa antigen preferentially evokes the generation of a Thl-like immune response in healthy tuberculin reactors.

10.Rupture and Spontaneous Sealing of a Coronary Aneurysm After Deployment of Drug-Eluting Stent.

Here, we report a case of a 78-year-old man who developed a rupture, and spontaneous sealing of the CAA occurred after stent implantation, as shown by computed tomography coronary angiography.
