2.Combination Therapy with Intralesional Interferon α-2b and Pulsed Dye Laser for the Treatment of Periungual Warts.

BACKGROUND: Periungual warts are a therapeutic challenge. Many studies have revealed that intralesional interferon α-2b therapy and pulsed dye laser therapy have numerous advantages over other modalities of theratment. OBJECTIVE: The purpose of this study was to determine if combination therapy with intralesional interferon α-2b and pulsed dye laser might offer an effective treatment for periungual warts. METHODS: Thirty-three patients were randomly assigned to one of three study groups. In group A, the patients received both intralesional interferon α-2b treatment and pulsed dye laser(PDL) therapy (n=13). Group B patients were treated wit intralesional interferon α-2b alone (n=10) and group C was done with PDL therapy only (n=10). RESULTS: The clearance rate was 92.3% for the patients treated with the combination therapy of intralesional interferon α-2b treatment and pulsed dye laser (PDL) therapy. It was 50% for the patients with the intralesional interferon α-2b treatment alone, and 0% for the group with the PDL therpy only. No significant side effects were observed. At 6 months after cessation of the therapy, total 2 cases (one from group A and the other from group B) were recurred. CONCLUSION: The combination therapy with intralesional interferon α-2b treatment and pulsed dye laser therapy was highly effective for the treatment of recalcitrant periungual warts.

3.Screening of Urine Culture Specimens by Gram Stain, Urinalysis and Urine Microscopic Examinations.

BACKGROUND: The purpose of this study was to discover ways to screen urine culture specimens through Gram stains, urine stick analyses and microscopic examinations for the laboratory cost saving. METHODS: One hundred and fifty-eight urine specimens for culture were included. Fifty uL of urine were inoculated onto one well each of 10-well slide, dried on the hot plate, and Gram-stained. The results combined with routine urinalyses including urine nitrite and leukocyte esterase, and pyuria, were compared with the routine culture results. RESULTS: The screening of bacteriuria by Gram stains, urinalyses and microscopic examinations revealed the high sensitivity (91.9%) and negative predictive value(95.5%) with cost saving of 41.8% of inoculating media. Not considering the Gram stains, the screening revealed 83.8% sensitivity and 92.5% negative predictive value, even if the cost saving of inoculating media were as high as 50.1%. CONCLUSION: It was demonstrated that it was sensitive and economic and produced rapid preliminary results to screen bacteriuria by the Gram stains combined with urinalyses and microscopic examinations.

5.Clinical Study After Reconstruction of the Posterior Cruciate Ligament: Factors on Posterior Stability.

The goals of treatment in the injuries of posterior cruciate ligament(PCL) are restoration of normal tibiofemoral stability and reduction of long term osteoarthrosis of the knee. The purpose of this study is to analyze the factors on posterior stability and functional result after PCL reconstruction. From March 1996 to April 1997, twenty operation on PCL reconstruction were carried out in our hospital. The functional results according to the criteria of the Lysholm knee score and stress radiograph for posterior stability were evaluated. The average functional results evaluated by Lysholm knee score was 87. There was no significant difference in functional result according to selected grafted material, combined knee injury, type of injury and arthrotomy, etc. Difference in posterior tibial translation on average were 2.8mm in acute and 7.7mm in chronic injury. In posterolateral combined injury, average posterior tibial translation in pos- terolateral repair was 3.2mm and in posterolateral reconstruction was 9.4mm. In PCL reconstruction using hamstring with repair of PCL remnant was 2mm. In conclusion, to obtain the good result on posterior sta- bility, early PCL reconstruction using hamstring with repair of PCL remnant, and appropriate treatment on combined posterolateral injury were recommended.

Varicella zoster virus (VZV)-induced brachioplexitis is a rare disease, characterized by exacerbating ipsilateral upper extremity pain, muscular weakness, and delay in recovery. We report a 54-year-old female patient who made an early recovery from VZV-induced brachioplexitis via the treatment with stellate ganglion blocks, which may have prevented pain sensation, vasoconstriction, and nerve scarring during the early treatment period.

10.Effects of TNF-alpha and Minoxidil on Human Hair Growth in Vitro.

BACKGROUND: The factors that regulate hair follicle growth are still poorly understood. In vitro models may be useful in elucidating some aspects of hair follicle biology. TNF-alpha is potent inhibitor of hair follicle growth. TNF-alpha induces the formation of a club-like hair follicle, similar to catagen morphology of the hair bulb. Minoxidil induces generalized hypertrichosis when administered systemically, or localized hair regrowth when applied topically to sites of alopecia. OBJECTIVE: The purpose of this study is to investigate the effect of TNF-alpha and minoxidil on human hair growth using in vitro model. METHODS: Healthy human hair follicles without any visible damage were collected, they were cultured in Williams E medium with several combinations of supplements and TNF-alpha and/or minoxidil were added to the media. The results were evaluated by measuring linear hair fiber growth and hair follicle morphology on light microsocopy and by measuring radioisotope uptake of (methyl-3H)thymidine of hair follicle. RESULTS: The following results were obtained from this study. TNF-alpha have inhibitory effect on the rate of linear hair growth in cultured hair follicles. Minoxidil has no stimulatory effect on the rate of linear hair growth and no protective effect on the TNF-alpha induced growth inhibition in cultured hair follicles. CONCLUSION: TNF-alpha has growth-inhibitory effect and minoxidil has no protective effect on the TNF-alpha induced hair change.
