4.Bilateral Segmental Neurofibromatosis Showing Different Dermatomal Distribution.

A 43-year-old woman presented with numerous cutaneous neurofibromas, limited to the left anterior chest(T2-3) and the right lower back(L1-2). These had been present for 10 years. Neither cafe-au-lait spot, intertriginous freckle, nor Lisch nodule was found. The family history was negative for neurofibromatosis. Biopsy specimens showed circumscribed, nonencapsulated neurofibromas. The present case was a rare form of bilateral segmental neurofibromatosis in that while most of the reported cases involved the same dermatome bilaterally, she had bilaterally different dermatomal neurofibromas.

6.Retinal Vascular Caliber Changes in Early Type 2 Diabetic Patients without Retinopathy

Purpose:
To evaluate the retinal vascular caliber in early type 2 diabetic patients without retinopathy.
Methods:
This retrospective study reviewed the medical records of 85 patients who were diagnosed with type 2 diabetes (age, 10-48 years) and sex-matched healthy controls. Dilated fundus examinations were performed to confirm the absence of diabetic retinopathy. Fundus photographs were acquired and analyzed using software that semi-automatically measured retinal vascular caliber. Central retinal arteriolar equivalent (CRAE), central retinal venular equivalent (CRVE), and arteriovenous ratio (AVR) were determined using the revised Parr-Hubbard formula. Healthy controls were examined using the same approach.
Results:
There were no significant differences between patients and controls in terms of CRAE (149.92 ± 21.29 µm and 144.85 ± 17.14 µm, respectively; p = 0.137) or CRVE (195.85 ± 22.94 µm and 196.69 ± 18.43 µm, respectively; p = 0.829). However, AVR was significantly higher in the patient group (0.768 ± 0.088 vs. 0.737 ± 0.061; p = 0.031). In the patient group, AVR was significantly correlated with hemoglobin A1c (r = 0.325, p = 0.006), but not with body mass index (p = 0.478).
Conclusions
An increase in peripapillary AVR may be an early feature of retinal vessels observed in patients with type 2 diabetes, which is closely related to elevated blood sugar.
7.Effect of High Glucose on the Oxidative Stress in Trabecular Meshwork Cells.

PURPOSE: To investigate the effect of high glucose (HG) on the oxidative stress in cultured human trabecular meshwork cells (HTMC). METHODS: Primarily cultured HTMC were exposed to low glucose (5 mM) and HG (25 mM) for 7 days. Additionally, 1 mM L-arginine, 5 mM DAHP, 10 microgram/ml insulin, 100 micrometer L-ascorbic acid, 10, and 100 micrometer sepiapterin were co-exposed. The cellular survival and nitric oxide (NO) production were assessed by MTT assay and Griess assay, respectively. Superoxide production was measured by modified cytochrome c assay. RESULTS: HG did not affect the survival of cultured HTMC significantly. HG decreased NO production. Co-exposed DAHP decreased but DAHP and insulin increased NO production. In addition, HG increased superoxide production, which was decreased by insulin, L-ascorbic acid, and sepiapterin. CONCLUSIONS: HG decreased NO production accompanied with increased superoxide production in HTMC. Thus HG induces oxidative stress in HTMC and may cause cellular dysfunction and damage of the trabecular meshwork.

8.Cross-Over Comparison of Dilevalol and Nicardipine in Patients with Mild to Moderate Hypertension.

The antihypertensive efficacy and safety of Dilevalol, compared with Nicardipine, were evaluated in mild to moderate hypertensive patients in a randomized crossover study. Following a 3 week placebo washout (phase I), the first crossover (phase II) was initiated with 63 patients randomized to dilevalol and 62 to nicardipine. The 6 weeks of treatment was initiated with 100mg of dilevalol once daily or 40mg of nicardipine (20mg B.I.D.). After 2 weeks, patients not achieving a sitting DBP to < or =90mmHg or a decrease of > or =10mmhg were uptitrated to 200mg Dilevalol once daily or 60mg Nicardipine twice daily. A second three week placebo washout (phase III) followed by a second 6 week active treatment phase (phase IV) during which patients were crossed over to the alternative therapy as during phase II if the sitting DBP again met the entrance criteria. 18 patients were only evaluable for the first washout and treatment period because of early discontinuation or protocol violations. They were included in the safety evaluation. phass II patients treated with Dilevalol (n=63) were mean age of 52.9 years, 49% male and 51% mild hypertensives (< or =105mmHg). Phase ??patients treated with Nicardipine (n=62) were mean age of 51.2 years 55% male and 66% mild hypertensives. Both Dilevalol and Nicardipine significantly and equivalently lowered blood pressure relative to baseline (71% versus 67% normalization). Dilevalol slightly but significantly lowered heart rate (-5 beat/min versus -1 beat/min). Dilevalol significantly increased HDL cholesterol (2.1mg/dl, 4.2%) and decreased total cholesterol (9.6mg/dl, 4.2%) while Nicardipine produced only a small but not significant reduction in total cholesterol (3.2mg/dl, 1.2%). The incidence of adverse effects were lower with Dilevalol than with Nicardipine and especially flushing and ankle edema occurred significantly more often with Nicardipine. in conclusion, dilevalol is equally effective but has a superior safety profile to nicardipine in the treatment of mild to moderate hypertension.

9.Surgery for Vertical Abnormal Head Posture in Infantile Nystagmus.

PURPOSE: The purpose of this study is to assess the effect of surgical management for infantile nystagmus with vertical abnormal head posture. METHODS: We performed surgical procedures to correct the vertical abnormal head posture of more than 10degrees in 6 patients with infantile nystagmus. One patient with the chin-up head posture was treated with recession of bilateral inferior rectus muscles and resection of bilateral superior rectus muscles, and 5 patients with the chin-down head posture were treated with recession of bilateral superior rectus muscles and resection of bilateral inferior rectus muscles. The pre- and post-operative measurements of the head posture were evaluated and compared with each other. RESULTS: Preoperatively, the degree of vertical head posture ranged from 10 to 30degrees, and 4 patients were with the 20~30degrees abnormal head posture. In the average follow-up period of 35.8 months, 5 patients (83.3%) showed less than 5degrees of chin-up or chin-down head posture. In one case, 20degrees left face turn was noted postoperatively. However, duction was full in all cases. CONCLUSIONS: These results suggest that the surgical management with bilateral vertical rectus muscles was effective for the correction of vertical abnormal head posture in infantile nystagmus.

10.Osteochondroma of the Atlas Presenting as an Oropharyngeal Mass.

Osteochondroma, also known as osteocartilaginous exostosis, or just exostosis, is the most common benign bone tumor. Most of osteochondromas are solitary lesions and affect long bones; however, about 12% of osteochondromas are multiple lesions with the autosomal dominant inheritance. Only 1~4% of solitary osteochondromas originate in the spine. Symptoms vary depending on its location. We experienced a case of osteochondroma of the atlas presenting as an oropharyngeal mass. We report the case with a review of literatures.
