1.A Case of Congenital Smooth Muscle Hamartoma.

Congenital smooth musele hamartomas appear at birth as hypertrichotic patches or plaques with or without hyperpigmentation. Histologic characteristic is hyperplasia of dermal smooth muscle bundles. We report a case of congenital smooth muscle harnartoma in 5-year-old female, who showed localized excessive-hairy, skin-colored patch on the lateral surface of left. elbow, which were found at birth. Biopsy specimen showed hyperpigmentation of the basal layers of epidermis, melanophages in upper dermis, and hyperplasia of smooth muscle bundles in lower dermis.

2.Development of the Health-Information System using a Multimedia Tool.

BACKGROUND: As people have been interested in healt,h and as the microcomputers with multimedia steeing have been supplied more and more, there is increasing need of multimedia information about health. The CD-ROM, which is a representative multimedia tool, is capable of interacting between information-provider and users, offering audiovisual interface, and saving great deal of knowledge. METHODS: We developed the health-information system to provide ordinary people with hea1th information using multimedia tools. It, has 7 parts ; self-diagnosis, exploring human body, drug information, hospital information, first-aid treatment, Q and A, and medical common sense. We got image data such as figures, photos, X-rays, electrocardiograms and pathologic specimens with scanner and film scanner, also moving image(video) with video-capturing program. RESULTS & CONCLUSIONS: We hope the multimedia health-information system to become a useful methodology of patient education and to be used as an information source in medical information network in the near future.

3.A Case of Nasopharyngeal Teratoma Showing Upper Airway Obstruction.

Neoplasms of the nasophrynx are rare in neonates and infants. The most common nasopharyngeal tumor encountered in this age group is teratoma-neoplasm containing multiple heterotopic tissues. Four histologic types of nasopharyngeal teratoma occur-dermoid, teratoid, true teratoma, epignathiof which dermoid is most frequently seen. We have experienced a case of nasopharyngeal teratoma in a female neonate with signs and symptoms of upper airway obstruction. It was diagnosed by cervical. CT and confirmed as ture teratoma by histological examination. Nesopharyngeal teratoma was removed by contact Nd:YAG laser. In addition, we briefly reviewed the literature of neonatal nasopharyngeal teratoma.

4.Reconstruction of median sternotomy dehiscence.

Complications after a median sternotomy incision, which is used currently in most open heart surgery, are serious, although it is infrequent. Reconstruction of the sternal defect resulting from dehiscence of median sternotomy is still big challenge to the most plastic surgeons. Since vascularized greater omentum was transposed to eliminate mediastinal wound problems, many vascularized regional muscle flaps became mainstay in reconstruction of median sternotomy wound. We treated 13 patients with median sternotomy dehiscence between October of 1993 and March of 1998. In two patients, the wound problems were so confined to superficial tissue that debrided and closed primarily. Eleven patients with deep wound infection were managed with vigorous debridement of all necrotic tissues and resultant defects were covered with regional muscle flaps: rectus myocutaneous flap(3) and bilateral pectoralis advancement flap(8). We used the pectoralis major advancement flaps without counter incision at humeral insertion site and the dissections were limited only medial to the anterior axillary line to preserve the axillary fold. In five patients with larger defects, we elevated muscle and cutaneous flaps separately to make these flaps more mobile. Large portion of two rectus abdominis flaps could not survive, whereas pectoralis advancement flaps had mo special wound problems. Only one patient developed fistula due to remained wire, regardless to flap surgery.

6.A Clinical Study of the Ankle Fracture

The ankle is a modified hinge joint consisting of tibial plafond, medial and lateral malleolus, talus and many soft tissue structures, which plays important role in weight bearing and walking. Hence, the ankle injuries include not only fractures of bone but often also the rupture of ligaments and soft tissues, so if the structures were not accurately repaired, many complications may develop. The authers analysed 98 cases (96 patients) of the ankle fractures which were admitted and treated in Orthopaedic Department, Soon Chun Hyang University Hospital from January 1979 to August 1983. The results obtained are as follows: 1. Of the 98 cases, male was 70 cases(71.9%), female was 28 cases and the average age of the patient was 31,1 years. 2. The most common cause of the fracture was traffic accident and the other causes were slip down, sports injury, fall down and industrial injuries in order. 3. 83 cases(84.7%) out of the 98 cases of ankle fractures were closed fracture and the rest was open fracture. 4. The most common type of the ankle fracture, according to the classification of Lauge-Hansen, was supination external rotation type. 5. 68 cases were treated with open reduction and internal fixation and 30 cases were treated with closed reduction, among the cases of closed reduction, the 14 cases were indicated to open redu- ction but it was impossible due to improper skin condition, severe comminution and anesthetic problem. 6. The average duration of the cast immobilization in closed treatment was 10.1 weeks and in operative treatment was 6.3 weeks. 7. The results of the treatment was better in those cases of the open reduction, the early reduction and the mildly displaced cases than the results of closed reduction, delayed reduction and severely displaced cases. 8. The incidence of complication, such as traumatic arthritis and nonunion occured higher in the cases treated with closed method.

7.Comparative Study of Intramedullary Nailing and Plate for Metaphyseal Fractures of the Distal Tibia.

PURPOSE: To compare the radiological and clinical results between intramedullary nailing and plate fixation in the treatment of distal tibial fractures. MATERIALS AND METHODS: 19 cases of distal tibial metaphyseal fractures within 4 cm of the ankle joint line were enrolled. Ten patients were treated with interlocking intramedullary nail and the others with plate and screws. RESULTS: The mean union time was 14 weeks in nailing group and 16 weeks in plate group. The average angulation in AP view was 4.1 degrees in nailing group and 3.1 degrees in plate group. The average angulation in lateral view was 1.7 degrees in nailing group and 2.7 degrees in plate group. The rotational deformity was 2.8 degrees in nailing group and 1.7 degrees in plate group in average. There was no implant failure and soft tissue problem. CONCLUSION: There was no difference in clinical and radiological results between intramedullary nailing and plate in the treatment of the distal tibial fractures and, considering the preservation of the soft tissue, the intramedullary nails are a reliable method for managing distal metaphyseal fractures of the tibia.

8.Recognition of a Pseudo-Outbreak of Cladosporium Species by Continuous Monitoring of Culture Results.

BACKGROUND: Cladosporium spp. are dematiaceous fungi that are commonly isolated from indoor and outdoor environments, including hospital air. This fungus is rarely pathogenic to humans, but has been reported to cause infections of the skin and toenails, as well as sinusitis and pulmonary infections. The monitoring of culture results was conducted to identify the outbreak of an unknown black fungal infection between January and March 2006 in a University hospital, and infection control activity was performed to identify the cause of the outbreak. METHODS: An epidemiological investigation of 22 patients with infections caused by an unknown black fungus was conducted. Microscopic examination and molecular analysis on the internal transcript spacer (ITS) region was performed to identify the black fungus. To detect the source of contamination, a culture of environmental specimens was performed, and then, disinfection of the laboratory was implemented. RESULTS: The patients with black fungi belonged to various departments and wards. No symptoms of fungal infection were recognized on the basis of the survey. The black fungus was identified as Cladosporium spp. on the basis of morphological features and ITS region sequencing. Culturing of environmental specimens was performed in the laboratory. Black fungi were isolated from a specimen from a rack and had the same morphological features with Cladosporium spp. from clinical specimens. After the rack was autoclaved, Cladosporium spp. from clinical specimens was no longer isolated. CONCLUSION: Epidemiological investigation, microscopic examination, and molecular analysis revealed that the sudden increase in the isolation rate of Cladosporium spp. from clinical specimens was the result of a pseudo-outbreak caused by the contamination of a rack. To our knowledge, this is the first report of a pseudo-outbreak of Cladosporium spp. Continuous monitoring of culture results is important to avoid unnecessary labor for nosocomial infection control.

9.Herpes Zoster in Children with Malignancy.

We described the clinical manifestation of herpes zoster in thirteen children with underlying malignancies. Among the associated malignancies, hematologic malignancy including acute lymphotytic leukemia was the commonest and CNS tumors were also frequently associated. Pain was mild, and some showed high fever and abnormal liver function test results. Recurrent attacks were observed in 3 cases(23%). Thoracic segment and trigeminal nerve were commonly affected. In most cases, herpes zoster developed within two years after the diagnosis of the malignancy.

10.A Case of McCune-Albright Syndrome with Vitamin D Resistant Rickets.

We present the cytologic features of small cell neuroendocrine carcinoma of the liver metastasized from the uterine cervix. Cytologically, tumor cells were arranged in a pat- tern of solid sheet in necrotic background. The tumor cells were characterized by uniform, small cells, round hyperchromatic nuclei, and high nuclear cytoplasmic ratio. The smears showed frequent mitotic figures and rosette formation, These findings were identified with the previous histologic sections of uterine cervix. To make a diagnosis of metastatic small cell neuroendocrine carcinoma on the Papanicolaou smear, a high index of suspicion and careful review of clinical history are needed
