5.Histologic Changes of Normal Human Skin in Organ Culture.

Since adult human skin can be grown in chernically defined medium without serum, the skin organ culture has gained a great interest as a method for studies concerning skin biology, pharmacology and toxicology. however, serum supplementation has extensively been used to improve the viahility of tissue culture. This study was undertaken to evaluate the effect of serum on the histologic changes ohserved during the organ culture of the normal human skin. The general architecture of the skin was well maintained for 6 days with or without seru. After then, fetal calf serum or autologous human serum was found to enhance the viability of the epidermis. A confluent layer of necrotic spinous ceils was ovserved earlier and more widespread without serum. The addition of serum had an impressive effect on epibolization. In the absenee of serum, the formation of the epibolus was not only minimal, but also, susceptible to degeneration, and no epibolus remained at 10 days rif incubation. No difference can be found between fetal calf serm and autologous human serum in the formation of the epibolus. There was no favorable effect of serum on the formation of new stratum corneum. The thickness of new straturn corneum increased in parallel with the number of parakeratatic cells, increasing most rapidly between 6 and 8 days of incubation. Parakeratosis was more prominent in the presence of serurn.

Paget's disease is classified, according to location, as mammary or extramammary disease. Extramammary Paget's disease especially in the vulva is extremely rare. Several hundred cases have been described worldwide. Patients with Paget's disease of the vulva are usually white and postmenopausal. The cause of which remains elusive. A case of vulva Paget's disease is presented with brief review of literature.

7.Expression of p53 Protein and Proliferating Cell Nuclear Antigen in Epstein - Barr Virus-associated Gastric Adenocarcinoma.

PURPOSE: Recently, it has been reported that Epstein-Barr virus (EBV) is associated with some gastric cancers. But EBVs role in EBV-associated gastric carcinomas (EBVaGCs) has not been fully elucidated. This study was undertaken to evaluate the characteristics of EBVaGCs and to compare those with non-EBVaGCs. MATERIALS AND METHODS: EBV infection was studied using paraffin-embedded tissue blocks of 119 cases of gastric adenocarcinomas by in situ hybridization for EBV-encoded small RNAs (EBERs). In EBVaGCs and non-EBVaGCs, molecular characteristics were evaluated by immunohistochemical staining for latent membrane protein (LMP)-1, p53 protein, and proliferating cell nuclear antigen (PCNA). RESULTS: EBERs were detected in 12 cases (10.1%) of 119 gastric adenocarcinomas. LMP-1 was negative in all carcinomas tested, p53 protein was positive in 7 cases (58.3%) of 12 EBVaGCs and in 51 (47.7%) of 107 non-EBVaGCs, the difference between two groups being not significant. Mean PCNA index was 38.2+-26.1% in EBVaGCs and 22.8 +- 20.0% in non-EBVaGCs. The index was significantly higher in the former than in the latter. CONCLUSION: These results suggested that neoplastic progression in EBVaGCs was implicated with high expression of PCNA, but not consistently with overexpression of p53 protein or LMP-1.

9.Successful Desensitization of A Case of Anaphylaxis to Insulin.

A small proportion of diabetics treated with exogenous insulin, reportedly ranging from 0.1% to 2%, experience systemic allergic reactions to insulin. We saw a 55-year-old male patient with insulin-dependent diabetes mellitus who had developed an anaphylatic reaction immediately after subcutaneous injections of neutral protamine Hagedorn (NPH) human recombinant insulin. Skin tests with insulins and a passive transfer test demonstrated that his reaction was IgE mediated. Since there were no therapeutic alternatives, desensitization with insulin was done following a slow protocol. It was successful and he was able to resume insulin therapy. During two year follow-up period, he did not developed any adverse reaction to insulin.

10.A Case of Congenital Smooth Muscle Hamartoma.

Congenital smooth musele hamartomas appear at birth as hypertrichotic patches or plaques with or without hyperpigmentation. Histologic characteristic is hyperplasia of dermal smooth muscle bundles. We report a case of congenital smooth muscle harnartoma in 5-year-old female, who showed localized excessive-hairy, skin-colored patch on the lateral surface of left. elbow, which were found at birth. Biopsy specimen showed hyperpigmentation of the basal layers of epidermis, melanophages in upper dermis, and hyperplasia of smooth muscle bundles in lower dermis.
