1.An Autoplex Treatment in a Hemophilia A Patient with High Titer of Anticoagulant FVIII Antibody.

Hemophilia A is a bleeding disorder resulting from a congenital deficiency of factor VIII. The mainstay of successful hemophilia therapy for either treatment or prevention of acute hemorrage is prompt and sufficient intravenous replacement of factor VIII to hemostatic levels. One of the most serious complications of hemophilia and its therapy is the development of polyclonal antibodies to factor VIII that neutralize factor coagulant activity. APCCs is otherwise known as bypass therapy, products that bypass the requirement for factor VIII for clot formation and moderately effective in controlling bleeding in high inhibitor patients. We experienced a case of hemophilia A with high inhibitor titer in a 13 year-old boy who presented with incarcerated right inguinal hernia and a successful therapeutic effect of APCCs(Autoplex) for controlling post-operative hemorrhages.

2.Augumentation Rhinoplasty Using Autogenous Cranial Bone Graft.

Augumentation rhinoplasty using autogenous cranial bone graft (outer table) can be used more successfully than other methods. In patients with congenital or posttraumatic severe saddle nose deformity and lateral deviation, cranial bone graft is an excellent method of augumentation. The advantages of cranial bone graft compared with traditional method of bone graft are summarized as follows; 1. Easy to reach donor site 2. Abundance of material 3. Little pain and functional disability 4. Shorter hospitalization period 5. Inconspicuous donor scar 6. No secondary deformity of donor site 7.Appropriate curvature can be obtained by proper selection of donor site. With the above advantages, we conclude that augumentation rhinoplasty using split cranial bone graft is a good method in correction of congenital or posttraumatic deformity of nose.

3.A Clinical Study of Herpes Zoster with Generalized Varicelliform Eruptions.

BACKGROUND: Herpes zoster with generalized varicelliform eruptions occurs in 2 to 10% of patients with herpes zoster. It occurs mainly in old or debilitated persons especially those who have immunologic defects such as lymphoproliferative diseases, AIDS, or recipients of immunosuppressive therapy. The reported incidence of herpes zoster with generalized varicelliform eruptions is variable. OBJECTIVE: The purpose of this study was to elucidate the incidence and clinical features of herpes zoster with generalized varicelliform eruptions. METHODS: We reviewed the clinical data of 962 patients with herpes zoster by retrospective methods. The annual incidence, age, sex, seasonal variation, predilection sites, and associated conditions of herpes zoster with generalized varicelliform eruptions were analyzed from January 1990 to December 1996 (7 years). RESULTS: 1. Among 962 patients, 8 patients with herpes zoster revealed generalixed varicelliform eruptions (0.83%). 2. The age ranged from 20 to 85 and the majority of cases occurred in the 6th decade. There were 4 females and 4 males. 3. Past histories of malignancy were observed in 2 patients. However, there were no signs of malignancy at the time of diagnosis of herpes zoster with generalized varicelliform eruptions. 4. The most common site of initial lesion was the thoracic dermatome,followed by the lumbar and the cervical ones. CONCLUSION: From our observation, it is suggested that herpes zoster with generalized varicelliform eruptions may occur in patients without underlying malignancy or immunosuppressive disorders. Sudden incidental uprising of herpes zoster with generalized varicelliform eruptions was observed in 1996.

4.Determinants of White Coat Effect in Essential Hypertension.

BACKGROUND: White coat effect (WCE) and white coat hypertension (WCH) are relatively prevalent in clinical situation (20 - 57% of WCH in the hypertensive population). The aim of this study was to analyze the determinant factors of WCE. METHODS: A prospective study was carried out in outpatient clinic in a consecutive hypertensives without receiving pharmacologic treatment. Twenty-four hour ambulatory blood pressure monitoring (ABPM) was performed following more than two resting blood pressure (BP) determinations carried out with the interval of 1 - 2 weeks by mercury sphygmomanometer. WCE was calculated for systolic and diastolic BP as the difference between a clinic BP (CBP) and an average daytime ambulatory BP (ABP). WCH was defined as;BP in the clinic > or = 140/90 mmHg with a mean daytime BP by ABPM < or =137/< or =89 mmHg. RESULTS: 1) Two hundred thirty-five patients (mean age 49.7 years, females 74%) were studied. Thirty-seven percent in male and forty-six percent in female patients fulfilled WCH criteria. 2) CBP was significantly correlated to daytime ABP (systolic BP;r=.47, p<.001 and diastolic BP;r=.65, p<.001). 3) The magnitude of WCE was greater in the group of WCH (28.9+14.6/19.3+6.9 mmHg) than ambulatory hypertensives (15.1+15.7/13.0+8.2 mmHg) (p<.001). 4) The magnitude of WCE is significantly correlated with female (r2=.12, p<.001) and the stage of CBP according to JNC-V (r2=.23, p<.001) in systolic BP, the stage (r2=.08, p<.001) in diastolic BP, and the stage (r2=.09, p<.001) and weight (r2=.15, p<.01) in mean BP. CONCLUSIONS: The magnitude of WCE in essential hypertension diagnosed at the clinic is significantly correlated with female, the magnitude of clinic BP, and weight.

5.Restenosis and Compliance with Self-Care Among Acute Coronary Syndrome Patients Undergoing Follow-up Angiograms.

BACKGROUND: Compliance with self-care and lifestyle modification is recommended for all patients with acute coronary syndrome to prevent a secondary attack. Data is limited regarding the association between compliance with self-care and restenosis. This study was conducted to examine the association between compliance with self-care and restenosis and to determine the predicting factors for restenosis and low compliance with self-care. METHODS: A total of 166 acute coronary syndrome patients (64.2+/-10.8 years) hospitalized for an angiogram during routine follow-up or symptom management were conveniently recruited from a university hospital cardiovascular care unit. Self-administered questionnaires were used to evaluate the level of compliance with self-care and symptom-related satisfaction. RESULTS: In-stent restenosis was found in 40.4% of the subjects and 36.1% were persistently smoking. There was no significant relationship between low compliance with self-care and restensosis. A lower symptom-related satisfaction significantly predicted restenosis and low compliance with self-care. A longer period after initial diagnosis predicted restenosis and a shorter period after initial diagnosis predicted low compliance with self-care. Persistent smoking was independently associated with low compliance with self-care. CONCLUSIONS: Patients who continued to smoke and had lower symptom-related satisfaction with daily living showed a stronger tendency for lower compliance with self-care and lifestyle modification. Effective educational strategy focusing on smoking cessation is needed to increase compliance with self-care in patients with ACS, especially during admission with a first heart attack.

6.Animal Experiment for the Analysis of Postmortem Inhalation Degree by Drowning Medium Containing Methylene Blue.

Drowning is a leading cause of accidental deaths worldwide, but its pathological diagnosis remains a challenge for forensic pathologists owing to a lack of pathognomonic findings in drowning deaths and inconclusive autopsy findings caused by postmortem changes. The aim of the present study was to investigate the pathway taken by inhaled drowning medium through the airway after death in a variety of experimental conditions, including underwater pressurization. We used methylene blue dye to monitor the spread of drowning medium to the lungs. Results of theses experiments demonstrated that it is possible for a significant amount of downing medium to enter the airway during immersion after death. Our results suggest that autopsies of immersed bodies and interpretation of these findings should be performed with special care.

7.Survey for Hygiene Behavior on Healthcare Personnel by Hygiene Inventory 23.

BACKGROUND: The purpose of this study was to survey hygiene behavior of healthcare personnel according to the Hygiene Inventory 23 (HI23) and to use the results in education and research for promoting hygiene behaviors. METHODS: We sampled a total of 400 people with 50 from each job category. The sample was obtained through convenient sampling among 8,200 employees working at a tertiary-care hospital with 2,600 beds in Seoul. The HI23 consisted of 5 subscales and 8 questions on general hygiene, 3 on household hygiene, 3 on food-related hygiene, 5 on hand hygiene methods, and 4 on personal hygiene, resulting in a total of 23 questions. The researcher also added 14 questions on the general characteristics of the subjects. The self-administered questionnaires were distributed on October 2009 and analyzed blindly. RESULTS: The average total score for hygiene behavior was 3.04+/-0.44 (maximum of 4). In the subcategory of hygiene behavior, the mean score was 3.20+/-4.24 for general hygiene, 3.07+/-0.68 for household hygiene, 3.07+/-0.68 for food-related hygiene, 2.91+/-0.69 for hand hygiene methods, and 2.97+/-0.48 for personal hygiene. When factors affecting the level of hygiene behavior were identified through multivariate regression analysis, independent factors were found to be gender (female), job category (food-related job), experience in hand hygiene education, and experience in teaching hand hygiene or general hygiene. CONCLUSION: Programs and education adapted to the characteristics of each job category in hospitals are needed to promote hygiene behaviors such as general hygiene, hand hygiene methods, household hygiene, food-related hygiene, and personal hygiene.

8.Pelvic Drainage Following Radical Hysterectomy with Pelvic Lymphadenectomy for Cervical Cancer: Is it Necessary? .

OBJECTIVES: This study was to determine whether drainage after radical hysterectomy and bilateral pelvic lymphadenectomy can reduce the risk of postoperative morbidity as compared with no drainage. METHODS: 165 patients with stage Ia2 to Ilb underwent radical hysterectomy and bilateral pelvic lymphadenectomy between January 1995 and May 1997, and those medical records were analyzed, Closed suction drains were placed in group I (n 102), not in group II (n 67). All surgeries were perforrned by the same surgeon in a uniform method. RESULTS: The characteristics of two groups were similar for mean age, preoperative weight, total protein, serum albumin, tumor cell histology, invasion depth, and tumor stage. There was no difference in mean operation time in two groups. But mean estimated blood loss was more in group I than group lI(p<0.05), Postoperative ileus and postoperative stay was similar in both groups. The incidence of pelvic lymphocyst and febrile morbidity in two groups were comparable with 17% in group I and 27% in poup II, but not statistically significant(p>0.05). Rehospitalization rate was higher in group I than in group II. CONCLUSION: Pelvic drainage didn't reduce the postoperative febrile morbidity and lymphocyst formation. Therefore the author could not find any necessity to perform the drainage following radical hysterectomy and pelvic lymphadenectomy.

10.Quantitative Analysis of Quartz, Mica, and Feldspar in Respirable Coalmine Dust in Taebaek Area by Fourier Transform Infrared Spectrophotometry.

A Fourier transform infrared spectrophotometric method was described for the determination of quartz, mica(sericite) and feldspar(potassium feldspar) in respirable dust in Taebaek area. The results were as follows; 1) The concentration of minerals were determined from the intensity of absorption peak of quartz at 799 cm-1, sericite at 539 cm-1, and potassium feldspar at 648 cm-1 respectively. 2) The precision(C. V. %) for the quartz determination was 7.70+/-2.68 % from 10 to 200 microgram of quartz. 3) The precision for the sericite determination was 16.34+/-6.82 % from 30 to 500 microgram of serictite. 4) The precision for the potassium feldspar determination was 5.28+/-1.74 % from 30 to 500 microgram of potassium feldspar. 5) The concentration of respirable dust in Taebaek area was 4.90+/-3.29 mg/m3(0.4-93.7%), percent quartz was 1.80+/-4.14% (0.01-20.56%), percent sericite was 11.37+/-6.43% (0.00-29.69%), percent potassium feldspar was 8.15 % (n=7, 3.41-19.70%). 6) The difference of respirable coal dust, quartz, and sericite concentration in drilling, coal cutting, hauling and separating was significant respectively (p<0.05).
