2.Factors Associated with Work-Related Injuries of Nurses in Small and Medium Sized Hospitals.

PURPOSE: This study was conducted to examine the factors associated with work related injuries of nurses in small and medium sized hospitals. Method: A cross-sectional survey was conducted with nurses in eight hospitals from October 2007 to January 2008. A questionnaire was designed to collect information on nurses' work related injuries, and individual and job related characteristics. The response rate was 65.1%. Data from 294 nurses were analyzed. Multiple logistic regression analysis was performed to determine factors associated with work related injuries. Result: Of the 294 nurses, 19.1% (n=56) responded as having at least one injury during their job performance. The logistic regression analysis revealed that the significant factors influencing work related injuries were job satisfaction, stress recognition, and hospital's location. Nurses with a higher job satisfaction were less likely to experience work related injuries (OR=0.58). Nurses with a higher stress recognition (OR=2.57) and those working at hospitals in metropolitan cities (OR=3.28) were more likely to experience work related injuries. CONCLUSIONS: The result of this study indicated that a substantial proportion of nurses in small and medium sized hospitals had experienced injuries related to nursing job. Interventions to prevent work related injuries among nurses should take into account the job satisfaction, stress recognition, and hospital characteristics.

3.A study on the preorthodontic prediction values versus the actual postorthodontic values in Class III surgery patients.

The purpose of this study was to find out and evaluate discrepancies between preorthodontic prediction values and actual postorthodontic values and factors contributing to it in 45 patients(17 male, 28 female) who were diagnosed as skeletal Class III malocclusion and received presurgical orthodontic treatment and orthognathic surgery at Yonsei university dental hospital. Lateral cephalograms were analyzed at pretreatment(T1), orthodontic prediction(T2), immediately before surgery(T3) and designated the landmark as coordinates of X and Y axes. The samples were divided according to ALD, upper and lower incisor inclination(U1 to SN, IMPA), COS, extraction, the position of extracted teeth and the statistical significance was tested to find out the factors contributing to the prediction. The results were as follows : 1. Differences between preorthodontic prediction values and actual postorthodontic values(T2-T3) were statistically significant(p<0.05) in the x coordinates of U6mbc, L1x and in y coordinates of U1i, U1x, U6me, U6mbc, L6mbc 2. The accuracy of prediction is relatively higher in horizontal changes compared to vertical changes. 3. The statistical significance(p<0.05) between prediction and actual values is observed more in the landmarks of the maxilla than the mandible. 4. Differences between prediction and actual values of incisor and first molar were statistically significant(p<0.05) according to extraction vs non-extraction, extraction type, ALD in the maxilla and according to ALD, IMPA in the mandible. Discrepancies between preorthodontic prediction values and actual postorthodontic values and factors contributing to the prediction must be considered in treatment planning of Cl III surgical patients to increase the accuracy of prediction. Furthermore future investigations are needed on the prediction of vertical changes.

4.Screening of Urine Culture Specimens by Gram Stain, Urinalysis and Urine Microscopic Examinations.

BACKGROUND: The purpose of this study was to discover ways to screen urine culture specimens through Gram stains, urine stick analyses and microscopic examinations for the laboratory cost saving. METHODS: One hundred and fifty-eight urine specimens for culture were included. Fifty uL of urine were inoculated onto one well each of 10-well slide, dried on the hot plate, and Gram-stained. The results combined with routine urinalyses including urine nitrite and leukocyte esterase, and pyuria, were compared with the routine culture results. RESULTS: The screening of bacteriuria by Gram stains, urinalyses and microscopic examinations revealed the high sensitivity (91.9%) and negative predictive value(95.5%) with cost saving of 41.8% of inoculating media. Not considering the Gram stains, the screening revealed 83.8% sensitivity and 92.5% negative predictive value, even if the cost saving of inoculating media were as high as 50.1%. CONCLUSION: It was demonstrated that it was sensitive and economic and produced rapid preliminary results to screen bacteriuria by the Gram stains combined with urinalyses and microscopic examinations.

5.Anti-Tumor Necrosis Factor Monoclonal Antibody Treatment for Chronic Behcet's Disease: 3 Cases.

PURPOSE: Despite intensive, chronic immunosuppressive therapy, permanent loss of vision resulting from relapsing ocular inflammation occurs frequently in patients with Behcet's disease. Since tumor necrosis factor (TNF) might have an important pathogenetic role in Behcet's disease, monoclonal anti-TNF antibody treatmant has introduced for chronic Behcet's disease. We report three cases of chronic Behcet's disease that were treated by monoclonal anti-TNF antibody. METHODS: We describe the use of the anti-TNF monoclonal antibody, infliximab (Remicade(R), Schering Plough), in three patients with Behcet's disease who exhibited a severe ocular involvement refractory to standard treatment. RESULTS: Remission of ocular inflammation was evident within the first 24~48 hours, and no relapse occurred during a 5~12 month follow up. No side effects were noted. CONCLUSIONS: We suggest that anti-TNF monoclonal antibody (infliximab) is a rapid and effective new therapy for sight-threatening ocular inflammation in Behcet's disease.

6.Renal Cell Carcinoma Involving the Renal Vein and Vena Cava.

A retrospective analysis of our experience with six patients of renal cell carcinoma involving the renal vein and vena cava are reported. Since neither chemotherapy nor radiation has significantly altered the course of these advanced cases, radical nephrectomy with complete removal of the tumor tissue seems to be the only effective treatment. Knowledge of the accurate extent and nature of caval involvement is essential in planning the appropriate surgical procedure. Venacavography should be performed in large renal tumors and nonvisualization of the kidney containing tumor.

8.Needs Assessment for Functionalities in Electronic Health Record Systems in General Hospitals.

OBJECTIVE: As an electronic health record system is implementing in Korean health care sectors, concerns about key functionalities of electronic health record systems are increasing. The purpose of this study was to identify core functions and set the priority in electronic health record systems under the Korean contexts in order to assure and improve the quality of the systems. METHODS: A survey was conducted using questionnaire developed by the study team based on literature review. The subjects were medical record administrators working at medical record department in general hospitals. RESULTS: The response rate was 59.8%(55/92). The functions which more than ninety percent of subjects responded as necessary right now and/or in near future related to 'drug alert', 'clinical guideline', 'chronic disease management', 'automated real-time surveillance', 'coded data', 'result reporting', 'de-identifying data', 'disease registry', and 'provider-provider communication and connectivity'. CONCLUSION: The results showed the high prioritized functions were decision support and health information/data management.

9.Visual Field Cluster Map Corresponding to Retinal Nerve Fiber Layer Sectors in Glaucoma.

PURPOSE: To generate a map relating visual field (VF) test points to corresponding areas of the retinal nerve fiber layer (RNFL) measured with optical coherence tomography (OCT) in patients with localized RNFL defects. METHODS: Twenty-four patients with preperimetric glaucoma and 173 patients with perimetric glaucoma, all with localized RNFL defects, underwent standard automated perimetry (SAP) and OCT measurements. To define zones of related point, factor analysis of the mean thresholds for the SAP test points was performed, independently for each hemifield. A map relating the VF zones to the 12 OCT sectors was plotted based on the strongest correlations between both techniques. RESULTS: Factor analysis divided the VF points into five VF zones for each hemifield. Distribution of the VF zones for the superior and inferior hemifields was slightly asymmetric. Linear regression results showed that superior VF zones corresponding to the superior arcuate and nasal step regions were best correlated with 6- and 7-o'clock RNFL sectors (inferior and inferior temporal) of thickness (r = 0.51-0.59). RNFL thinning (defined by abnormal sector at p < 5%) and regional decreases in SAP sensitivity (defined by abnormal pattern deviation at p < 5%) were topographically related. CONCLUSIONS: A newly developed VF cluster map revealed significant topographical structure-function relationships, especially in the arcuate and nasal step region of the VF.
