A retrospective analysis of 132 patients who underwent nephrectomy in the Department of Urology, Kyung Hee University Hospital, during the period from January 1972 to December 1980 was presented. The results were as follows: 1. The total number of in-patients during the above period was 2,301 and nephrectomies were performed in 132 of 1,427 total urologic operations (9.3%). 2. Age distribution showed the highest in 4th decade with 22.7%, the youngest was 11 months and the oldest 78 years, and male to female ratio was 68 to 64. 3. The causative diseases of the nephrectomy were renal tuberculosis 48 cases (36.4%), tumor 15 cases 11.4%. hydronephrosis 11 cases (8.3%), pyonephrosis 10 cases (7.6%), anomaly 8 cases (6.1%) and hypertension 6 cases (4.5%) in order. 4. Anterior abdominal extraperitoneal transverse approach was the most frequently used method in 103 cases (78.0%) 5. Post-operative complications were seen in 29 cases (22.0%), of which wound infection was common in anterior abdominal extraperitoneal transverse approach.

OBJECTIVE: As an electronic health record system is implementing in Korean health care sectors, concerns about key functionalities of electronic health record systems are increasing. The purpose of this study was to identify core functions and set the priority in electronic health record systems under the Korean contexts in order to assure and improve the quality of the systems. METHODS: A survey was conducted using questionnaire developed by the study team based on literature review. The subjects were medical record administrators working at medical record department in general hospitals. RESULTS: The response rate was 59.8%(55/92). The functions which more than ninety percent of subjects responded as necessary right now and/or in near future related to 'drug alert', 'clinical guideline', 'chronic disease management', 'automated real-time surveillance', 'coded data', 'result reporting', 'de-identifying data', 'disease registry', and 'provider-provider communication and connectivity'. CONCLUSION: The results showed the high prioritized functions were decision support and health information/data management.

PURPOSE: Bone involvement is known to develop in 40-70Yo of pediatric acute leukemia. We aimed to analyze the clinical course and result of therapy in pediatric acute leukemia with bone involvement. METHODS: Twenty-seven patients diagnosed as pediatric acute leukemia at Dong San Medical Center from Jan. 1996 to Aug. 1998 were evaluated. According to bone X-ray and whole body bone scan, the patients were divided into two groups. RESULTS: Twenty-seven patients were enrolled in this study with 14 patients(52Yo) showing definite bone involvement on simple X-ray or bone scan. Mean age of patients with bone involvement was 5.5 years. Regarding the type of leukemia, 9 patients(64%) were acute lymphocytic leukemia. Ten patients(71%) out of 14 with bone involvement complained of bone pain at the involved bony site. Site of involvement was most frequent in the lower extremity. On simple X-ray, osteolytic lesion was found in 7 patients(50%), diffuse osteopenia in 2 patients(14%) and pathologic fracture in 2 patients(14%). In bone scan, radioactivity was increased in whole cases of patients with bone involvement. Thirteen patients(93%) were completely remitted by chemo-therapy, but, one AML patient died due to induction failure. CONCLUSION: Bone involvement occured in 52% of pediatric acute leukemia. Bone involvement was more frequent in male patients in the lower extremity, and osteolytic lesion was the most frequent finding on simple X-ray. There was no relevence between bone involvement and prognosis. Further study will be needed to evaluate long-term survival and prognosis. (J Korean Pediatr Soc 2000;43:806-813)

The clinicolaboratory findings, cardiac catheterization, and outcome of operation were analyzed in 78 patients with tetralogy of Fallot, which were diagnosed by echocardiography, cardiac catheterization, and a cardiac angiography and confirmed by operation at Dong San Hospital, Keimyung University, during a 4(1/2)-year period from January 1984 to June 1988. The following results were obtained : 1) Out of the 78 cases, 45 were male and 64 were female. 2) The electrocardiogram showed right axis deviation in 71 cases(91%) and right ventricular hypertrophy in 66 cases(84.6%). 3) Chest X-ray revealed that, the cardiothoracic ratio was normal or decreased in 68 cases(87.3%), and cardiac apex elevation was noticed in 57 cases(73%). 4) Pulmonary stenosis were chiefly valvular and infundibular type(53.8%). 5) The associated heart diseases with TOF were patent foramen ovale(64.1%), right sided aortic arch(19.2%), and secundum ASD(10.3%), in that order. 6) There was an intimate correlation between secondary polycythemia and thrombocytopenia. 7) Among the 78 Cases, 73 cases had total correction, 1 case had shunt operation, and 4cases had total correction after shunt operation. The highest mortality rate occurred in the cases of total correction after shunt operation(25%). The cases with a main pulmonary artery size of 1/3 to 2/3 against, the aorta had a higher mortality than the other group, and no cases expired in the group with a ratio of more than 2/3. The overall surgical mortality was 8.9%.

A retrospective analysis of 9 patients of bladder tumor who underwent total cystectomy in the Department of Urology. Kyung Hee University Hospital, during the period from May, 1977 to April, 1981. The results were as follows: 1. The age of the patients ranged from 52 to 65 years, the average age 59.8 years, and male to female ratio was 7 to 2. 2. Hematuria was the most commonly found in 9 patients, frequency in 5 patients, and dysuria in 3 patients. 3. Errors in clinical staging were also common-1 case was overstaged, 3 cases understaged, 3 cases corresponded and 2 casts inadequate for the clinical staging, but the pathologic grade proved to be equal that estimated from the 2 cases of transurethral biopsy. 4. There were 8 cases of total cystectomy and 1 case of radical cystectomy, the methods of urinary diversion and remodeling of urinary tract were ileal conduit in 6 cases, bilateral cutaneous ureterostomy in 3 cases. 5. Operative death was none but the complications were followed by total cystectomy and urinary diversion in 8 cases as wound disruption, pneumonia, atelectasis, irritation dermatitis, vaginal vault bleeding, pyelonephritis, stromal stricture, stricture of ureteroileal anastomosis site and delayed wound healing.

Clear cell sarcoma of the tendon and aponeurosis was first described by Enzinger in 1965, and has become a well-accepted clinicopathological entity as malignant melanoma of soft parts. Although it posesses the ability to produce melanin, it is quite different from conventional melanoma in that, it is more deeply located, usually associated with tendon and aponeurosis, displays pale-staining round to oval-shaped tumor cells and has a longer clinical course and better prognosis.Several cases have been reported in Korea, and we experiened an additional case of this rare tumor that showed ulceration.

KSLA published our first version of treatment guidelines for dyslipidemia in 1996, which was based on health examination data gathered by the National Health Insurance Corperation in 1994. A number of academic societies including the Korean Endocrine Society, the Korean Society of Cardiology, the Korean Society for Laboratory Medicine, the Korean Society for Biochemistry and the Korean Nutrition Society participated in the development of this guideline. In 2003, the second version of our guidelines was published based on the Korean National Health and Nutrition Survey (KNHANES) data which was collected in 1998. In 2006, the second version was modified and expanded with using KNHANES data collected in 2005. This article summarizes the recommendations included in the expanded second version of treatment guidelines. PURPOSE: We classified the Orbscan anterior elevation maps in normal eyes (under myopic, emmetropic and hyperopic conditions) and in those after myopic refractive surgery. We did this classification to demonstrate how Orbscan anterior elevation maps are useful in screening for the existence and extent of previous myopic refractive surgery. Such a classification can help clinicians interpret preoperative and postoperative topographies. METHODS: We measured for visual acuity and refractive power in 4800 eyes. After a slit-lamp examination, a corneal topography exam was performed with an Orbscan corneal topography system. The eyes were divided into two groups, with Group I representing those who had not had refractive surgery (4438 eyes). Group II included those who had undergone previous refractive surgery to correct myopia (362 eyes). RESULTS: In Group I, the central island type (43.0%) was the most common, followed by the temporal ridge (25.8%), the with-the-rule regular ridge (16.7%), the against-the-rule regular ridge (6.6%), the nasal ridge (4.0%), and the saddle type (2.1%). In Group II, the depressed lake type (69.9%) was most common, followed by the de-centered ablation type (21.3%). The trend line of the postoperative central anterior surface elevation (E) and the ablation power of refractive surgery were calculated. Ablation power of refractive surgery=0.0047 E+0.0083 CONCLUSIONS: This study demonstrates that it is possible to use Orbscan anterior elevation maps to screen for the extent of previous refractory surgery used in the correction of myopia. This study may also be useful in understanding the shapes of Orbscan anterior elevation maps before and after myopic refractive surgery as well as in determining the degree of ablated myopic refractive power and decentration.
