1.Changes of Risk factors of Metabolic Syndrome among the Elderly in the Rural Area after Two Years.

OBJECTIVES: This study was performed to examine the healthy lifestyle influenced on the metabolic syndrome among the elderly in the rural area. We examined the changes of risk factors of metabolic syndrome after two years. METHOD: Out of 134 who did health screening at the J county health center from July 1 to August 31, 2006, 65 aged people confirmed to metabolism syndrome were selected as subjects. For the final analysis, 62 aged people who revisited after two years were selected our subjects. The data were analyzed using SPSS PC+ 12.0 program for descriptive, Chi-square test, t-test and ANOVA analysis. RESULT: The result of this study indicated that 62 metabolic syndrome patients in 2006 decreased into 53, and women were higher than men. The difference in the metabolic syndrome came from level of education, smoking, health practices, and past illness history. CONCLUSION: Therefore, the change of lifestyle needs to be induced by the level of education, and for the group of smoking, non-exercise, and past illness history not only regular checkups to prevent metabolic syndrome but also education to have interest on themselves may need intensively.

2.Measurement of Ventilation-Perfusion Ratio Using Multiple Inert Gas Elimination Technique in the Endotoxin-Induced Septic Rabbit.

BACKGROUND: Endotoxin is a complex lipopolysaccharide molecule situated within the outer membrane of Gram-negative bacteria. Sepsis and acute respiratory failure (ARDS) can be induced by endotoxin. In order to introduce and develop the experimental model of ARDS in sepsis, we induced sepsis with the endotoxin and investigated the change of respiratory pathophysiology during sepsis using a multiple inert gas elimination technique (MIGET). METHODS: Ten New Zealand white rabbits were anesthetized and ventilated with a Harvard apparatus. In 5 rabbits, 2 mg/kg of lipopolysaccaride from E. coli was infused intravenously for 30 min (Toxin group). At 1, 2, 3, and 4 hours after endotoxin infusion, arterial blood gas, and hemodynamic profiles were checked. To perform the MIGET, six inert gases (SF6, krypton, desflurane, enflurane, diethyl ether, acetone) of widely varying solubility were infused peripherally and the excretion and retention data was determined from measurements of inert gas tensions in pulmonary arterial, systemic arterial blood samples and mixd expiratory gas sampling of pre and post septicemia using gas chromatography. We transformed and analysed the data into a V/Q distribution curve to find out the change of V/Q distribution curve. After the experiments, the animals were dissected and the lungs were extracted for wet/dry weight ratio (WW/DW) and microscopic examination. RESULTS: In the Toxin group, the pulmonary arterial pressures were increased and arterial oxygen tensions were decreased after the endotoxin infusion. The lung WW/DW were increased and inflammatory findings were seen in microscopic examination. In the MIGET, shunt, deadspace and log SDQ were increased in the toxin group, though there were wide V/Q distributions in the control group. CONCLUSIONS: We developed a successful endotoxin induced septic animal model, V/Q distribution curve and data using MIGET. The accomplishment of the experiment will not only allow us to better understand pulmonary pathophysiology of endotoxin induced sepsis using MIGET, but it will also contribute to other pulmonary physiology experiments associated with sepsis.

4.Development of Infection Control Guideline for Postpartum Care Centers.

BACKGROUND: This study aimed to develop feasible and detailed infection control guidelines (ICG) and audit protocols for health care workers and auditors in postpartum care centers (PCC). METHODS: PCC ICG and audit protocols were developed in several steps: 1) review of previous ICG for PCCs; 2) establishment of frameworks for updating guidelines and audit protocols; 3) review of new ICG, regulations, etc.; 4) drafting of PCC ICG and audit protocols and revision based on feedback from the research committee; 5) Delphi survey to solicit opinions from infection control, infant care, and maternal care professionals as well as PCC health care workers; 6) re-revision after discussion with the research committee; and 7) finalization of PCC ICG and audit protocols. RESULTS: We developed ICG with 4 categories and 26 sub-categories, and internal and external audit protocols with 163 and 85 items, respectively. CONCLUSION: The ICG and audit protocols are valid and feasible; we recommend their utilization as auditing tools, baseline data for the development of national infection control policies, and as educational materials for PCC healthcare workers.

5.Prospective and Retrospective Incidence and Post-exposure Reporting of Needlestick Injuries.

BACKGROUND: Most studies on the incidence rate (IR) and post-exposure reporting rate (RR) of needle-stick injuries (NSIs) were performed using retrospective surveillance, which is vulnerable to recall bias. This study aimed to identify the agreement between IRs and RRs obtained from prospective and retrospective surveillance. METHODS: The prospective surveillance was performed with 716 nurses working at 3 hospitals from August to September in 2012. They prospectively reported when they experienced the NSIs, and the investigator retrospectively calculated the RR from records in the infection control unit or health care unit during the same periods when they reported the number of NSIs. The retrospective surveillance was carried out with 312 nurses who participated in the prospective surveillance. They retrospectively answered the question on the number of NSIs and post-exposure reporting after recalling the experienced NSI from August to September in 2012. RESULTS: The IR of NSIs was 9.8 per 100 nurses by the prospective surveillance and 36.4 per 100 nurses by the retrospective surveillance, which was statistically significantly different (P<0.001). The RR of NSIs was 14.3% by the prospective surveillance and 8.5% by the retrospective surveillance, which was not statistically significantly different. CONCLUSION: We recommend using a prospective approach for calculating the IR of NSIs to reduce the risk of recall bias. However, the RR of NSIs can be calculated using both prospective and retrospective approaches.

6.A Clinical Study for Intrauterine Fetal Death.

The intrauterine fetal death(IUFD) is the death of the fetus prior to complete expulsion or extraction from its mother when the fetus is over 20 weeks gestation or weights more than 500 gm. This is a clinical study of 262 cases of IUFD and 262 control cases among 18542 deli-veries at Chung Goo Sung Sim Hospital during 10 years from 1987 to 1996. The results obtain-ed were as follows: 1. The incidence of IUFD was 1.41%. 2. The risk of IUFD was high in women older. 3. The risk of IUFD was high in women with parity of three or more, there was a previous history of IUFD in 11.2% of the IUFD cases and in 6.3% of the control cases, but there was no difference between the two groups of women in history of spontaneous abortion. 4. The sex ratio of male versus female was 1.24:1 in the IUFD cases, 1.11:1 in the control cases. 5. The low birth weight and preterm infants were much more frequent in the IUFD cases. 6. The mode of delivery IUFD was induced labor(77.5%), laparotomy(12.2%), spontaneo- us delivery(10.3%). The indications for laparotomy were placental abruption, placenta previa, transverse lie, previous cesarean section status, cephalopelvic disproportion and uterine rupture. 7. The most common cause of IUFD was unexplained causes(45.1%). 8. There were 67 cases(25.6%) of maternal complication, and the most common complica- tion was fever(35.8%). 9. In the incidence of abnormal coagulation test, the low platelet count(<100,000/mm3) was 8.0%, the prolonged prothrombin time(>14 seconds) was 7.2%, the prolonged partial thr- omboplastin time was 1.0%, the hypofibrinogenemia(<150mg/dl) was 7.2% and the elevated fibrinogen degradation products(>40 ug/ml) was 8.4%. 10. The risk of IUFD was lowered as the number of antenatal care was increased.

7.Blood Level of IL-31, Stem Cell Factor, and B Cell-Activating Factor in Persistent Allergic Rhinitis Patients.

BACKGROUND AND OBJECTIVES: Stem cell factor (SCF), B cell-activating factor (BAFF) and Interleukin (IL)-31 are related to allergic diseases such as atopic dermatitis or asthma. But they have not been sufficiently investigated in connection with IgEmediated allergic rhinitis. In this study, we evaluated the association between plasma concentrations of these cytokines and allergic rhinitis. SUBJECTS AND METHODS: A comparative study of the concentrations of plasma SCF, BAFF and IL-31 were conducted between persistent allergic rhinitis patients and a healthy control group using ELISA. RESULTS: Plasma BAFF and IL-31 concentrations were significantly increased in allergic rhinitis group (BAFF, 1,255 pg/mL, Plasma BAFF and IL-31 concentrations were significantly increased in the allergic rhinitis group (BAFF, 1,255 pg/mL, p<0.001;IL-31, 221 pg/mL, p<0.001) than in the control group (BAFF, 1,089 pg/mL;IL-31, 153 pg/mL). But the level of SCF did not exhibit any difference between the allergic rhinitis patients and the control group. CONCLUSION: Our results suggest that plasma BAFF and IL-31 might be associated with the inflammatory mechanism of persistent allergic rhinitis patients.

8.Analysis of Retinal Nerve Fiber Layer Thickness in Pediatric Migraine Using Optical Coherence Tomography

Purpose:
We investigated disparities in retinal nerve fiber layer (RNFL) thickness surrounding the optic disc among pediatric migraine patients based on the presence of a concomitant visual aura.
Methods:
We used optical coherence tomography to measure RNFL thickness around the optic disc in children with migraines and no organic diseases, along with a control group of healthy children. Differences in RNFL thickness were analyzed based on the pattern of prognostic symptoms. Additionally, we investigated the effects of migraine duration and frequency on RNFL thickness.
Results:
In total, 45 children with migraine and 30 healthy controls were enrolled in this study. Among the children with migraine, 15 experienced visual aura, whereas 30 did not exhibit visual aura. Visual prognostic symptoms included blurred vision, scotoma, and scintillating scotoma. Seven children presented with blurred vision and scotoma, whereas eight reported scintillating scotomas. The mean RNFL thickness around the optic disc was significantly lower in patients with blurred vision and scotomas (89.19 ± 7.99 μm) compared with the control group (98.79 ± 8.49 μm), patients without visual aura (100.55 ± 6.43 μm), and patients with scintillating scotoma (102.21 ± 10.13 μm, p < 0.05).
Conclusions
RNFL thickness around the optic disc was significantly reduced in children with migraine who reported symptoms of blurred vision and scotoma.
9.A Case of Dual Ectopic Thyroid.

Ectopic thyroid is an uncommon status characterized by the presence of thyroid tissue in a site other than in its usual region. It is rare for dual ectopic thyroid to have two ectopic foci simultaneously. Three cases of dual ectopic thyroid have been reported in the Korean literature. Clinically, most cases in ectopic thyroid were noted at adolescence or postpartum due to increased physiologic hormonal demands. Clinical manifestation, thyroid function test, and radiologic imaging should be employed to make a diagnosis and therapeutic plan of this disease. We present a patient with dual ectopic thyroid tissue in the suprahyoid and infrahyoid area without thyroid tissue in its normal thyroid bed.

10.The Effects of Ischemic Preconditioning and K(ATP) channel Activation on the Expression of the PKC-epsilon, NF-kappaB and AP-1 in Ischemia-reperfused Rat Heart.

This study was aimed to elucidate the effects of K(ATP) activation during IPC on the PKC-epsilon, NF-kappaB and AP-1 in ischemia-reperfused rat hearts. SD male rats weighting from 300 to 350 g were split into 9 groups, such as sham control (S), IPC, 3 cycles of 5 min ischemia and 5 min reperfusion, continuous preconditioning (CP), 8 cycles of 5 min ischemia and 5 min reperfusion, K(ATP) opening (KO) with pinacidil (1.0 mg/kg), K(ATP) blocking with glibenclamide (1.0 mg/kg) injection, ischemia (IS), 30 min ischemia, IPC followed by IS, 8) K(ATP) blocking and IPC followed by IS (KB+IPC+IS), IS and K(ATP) opening (KO+IS). Heart were subjected to ligation of left descending coronary artery and reperfusion in groups of IPC, CP, IS with or without IPC. Immunohistochemistry and Western blotting for PKC-epsilon, NF-kappaB and AP-1 were performed at 3, 6, 24 hours after reperfusion or treatment. Immunoreactivities against PKC-epsilon antibody were observed stronger in the groups of IPC, KO, IPC+IS and KO+IS than groups of KB, IS and KB+IPC+IS. NF-kappaB activation and translocation were only observed in the groups of including 30 min ischemia and reperfusion. AP-1 activation and translocation were opposite to the results of PKC-epsilon activation. In the group of CP, KB, IS and KB+IPC+IS, reactivities of AP-1 antibody were stronger than IPC+IS, KO+IS, and weaker in the groups of S, IPC and KO. These results suggest that K(ATP) opening with IPC or pharmacological methods may direct effect on the PKC-epsilon activation and that K(ATP) blocking has effect on the AP-1 activation and translocation in the heart of ischemiareperfused of rats.
