2.Clinical Analysis about the Injuries of Tendons , Vessels and Nerves in the Wrist

The wrist joint is exposed to trauma easily which is a small portion of human body. Because there are many structures(tendons, vessels and nerves) in the wrist, it is necessary to examine carefully and treat properly when it is damaged. We have treated 230 cases of soft tissue injuries around the wrist and the result can be summarized as follows; 1. Among the 230 cases, the male ratio was 85% and the highest incidence was in the third decade. 2. Right side injuries were more than left and flexor surface injuries were more than extensor surface. 3, The main cause of injury was glass, 149 cases(64.8%) and the next on was knife, 37 cases(16%). 4. Injuries in the night were more than daytime. 5. Injuries in the summer were more than winter.

3.The Effects of Video Programs of Cardiopulmonary Cerebral Resuscitation Education.

PURPOSE: The aim of this study was to identify the effect of video programs of cardiopulmonary cerebral resuscitation (CPCR) education of cardiopulmonary cerebral resuscitation of nurses. METHODS: The subjects of the study were 64 nurses working in a university hospital. Nurse's CPCR performance have been measured four times (pre-test, post-test at immediately, 3 months and 6 months after intervention). Data were collected from February to August 2013. RESULTS: There were significant differences in knowledge, attitude, self-efficacy, and performance between groups by measure time. And there were significant interactions in knowledge, self-efficacy, and performance between groups, within groups, except for the attitude. The video programs of CPCR interventions appear to be effective in the improvement of knowledge, self-efficacy, and performance, as compared to the control group. CONCLUSION: The video programs of CPCR education was an effective intervention to improve and retain the level of knowledge, attitude, self-efficacy and performance. And the video program of CPCR education have an advantage of self-learning effect for nurses with shift work. Therefore video programs of CPCR education will be utilized for continuing nurse's education.

4.Arterial Priapism: Superselective Embolization using Autologous Blood Clot.

A case of arterial priapism was treated by superselective transcatheteric autologous clot embolization of cavernosal artery 25 days after blunt periheal trauma. No immediate improvement was noted but the priapism subsided on 48th day after the procedure. Normal physiclogic erectlie function was achieved on 53rd day after the procedure, and the period of postprocedure impotence was shortened, as compared with previously reported cases. Autologous blood clot embolization may be an effective treatment method even in delayed priapism, and superselective technique is thought to be more effective to shorten the postprocedure erectlie dysfunction

BACKGROUND: The Cobas(R) Integra is a new automated chemistry analyzer with continuous and random-access features for routine chemistries, specific proteins, electrolytes, therapeutic drugs and drugs of abuse. The system maintains 68 test-specific reagent cassettes on board and have test principles of absorbance photometry, immunoturbidometry, fluorescence polarization and ISE measurements. We evaluated the usefulness of Cobas(R) Integra in the aspect of accuracy, precision, linearity, comparison study, test speed and general performance. METHODS: We evaluated the analytical performance of the Cobas(R) Integra for T. bilirubin, CRP, BUN, creatinine, sodium, potassium, chloride, valproic acid, phenytoin and carbamazepine according to NCCLS guidelines (EP5-T2, EP6-P, EP9-T and GP10-T). RESULTS: In accuracy study, error ranges(%) of most test items, except sodium and chloride, were within analytic goals. In within-run and between-run precision study, Coefficient of variation (CV, %) of most items, except sodium, were lower than limits suggested by College of American Pathologists Survey 1991. The linearities were maintained well in the range of medically significant levels and were statistically acceptable (p<0.001). The comparison study for most items, except sodium and chloride, indicated good correlation with Hitachi 747 and TDx FLx(TM) and correlation coefficients (r) were above 0.98. Throughput was about 450 tests/hr. CONCLUSIONS: Cobas(R) Integra showed satisfactory accuracy, precision, linearity, good correlation with other analyzers and high throughput. So, we concluded that Cobas(R) integra is highly suitable for not only routine use but also emergency tests in medium to large-sized hospitals because of its high productivity and reliability.

7.The Knowledge, Awareness and Preventive Behaviors of Pneumococcal Vaccination for the Elderly.

PURPOSE: The purpose of this study was to ascertain current status of pneumococcal vaccination for the elderly and their knowledge, awareness and preventive behaviors for the vaccination and to verify the relationship between results. METHODS: This cross-sectional study was performed using a questionnaire. Data were collected from 114 elderly people over 65 years at one community center from October to November in 2011. Collected data were analyzed using SPSS. RESULTS: The vaccination rate of the elderly was as low as 19.3%. The most common reason for no vaccination was 'Because I did not know about it' (64.0%), and the intention to vaccination was to ascertain in 77.2%. The scores of knowledge, awareness and preventive behavior for vaccination were 0.31/1, 1.15/3 and 1.48/2, respectively. The knowledge for vaccination was significantly different by gender and education. The awareness of vaccination showed statistically significant difference by religion, perceived health status and experience of vaccination. A positive correlation was observed between both knowledge and awareness and awareness and preventive behavior for vaccination. CONCLUSION: The pneumococcal vaccination rate of the elderly was relatively low. To develop, therefore, the educational program for improving awareness and preventive behavior regarding pneumococcal vaccination of the elderly is necessary.

8.The Impact of Vibration Exercises on Shoulder Muscle Thickness.

PURPOSE: The objective of this study was to examine the effect of vibration exercises generated from an XCO® trainer on supraspinatus, infraspinatus, teres minor, and deltoid muscle thickness. METHODS: Thirty subjects were evenly divided into two groups. Muscle thickness was measured by a sonogram prior to the study, and at three and six weeks after the intervention. Changes in muscle thickness were analyzed using a repeated measure analysis of variance (ANOVA). The significance level for the statistical test was set at α=0.05. RESULTS: A statistically significant differences in timing, interactions between timing and the groups, and between-group changes were demonstrated for supraspinatus muscle thickness (p < 0.05). A similar finding was reported for infraspinatus, teres minor, and deltoid muscles with regard to the interactions between timing and the groups (p < 0.05), although the between-group change did not reach statistical significance (p>0.05). CONCLUSION: Significant changes were observed in the thickness of the supraspinatus, infraspinatus, teres minor, and deltoid muscles, owing to the use of vibration exercises generated from an XCO® trainer. These findings can be used as a foundation for future studies on rehabilitation training.

10.The Efficacy of Intra-Articular Hyaluronic Acid Injections in Ankle Osteoarthritis

Purpose:
This study sought to investigate the appropriate indications for intra-articular injection of hyaluronic acid in ankle osteoarthritis, its efficacy at each stage of osteoarthritis and to compare the efficacy related differences at each stage.
Materials and Methods:
A retrospective study was conducted from January 2016 to September 2019 on 43 patients (50 cases) diagnosed with ankle arthritis and given intra-articular injection of hyaluronic acid. Patients were classified according to the modified Takakura ankle osteoarthritis stages. Stage 2 comprised 24 cases; stage 3A, 15 cases; stage 3B, 8 cases; and stage 4, 3 cases. The clinical evaluation was carried out using the visual analogue scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS) score, and the Foot and Ankle Outcome Score (FAOS). The patients’ satisfaction with the injection was evaluated at 1 year after injection. A radiographic evaluation was performed to gauge the progression of osteoarthritis before and after the injection.
Results:
The VAS, AOFAS score, and FAOS at 3 months and 1 year after injection showed statistically significant improvements compared to those before the injection. There was a significant difference in the VAS for each stage over time; however, this was not the case with the AOFAS score and the FAOS. A significant improvement of the VAS after injection was seen only in stage 3A when compared with the other stages. The overall satisfaction rate with the injection was 90%. There were no ankle osteoarthritic stage changes in the serial follow-up radiograph.
Conclusion
Intra-articular injection of hyaluronic acid resulted in a significant clinical improvement up to 1 year after the injection.Therefore, the intra-articular injection of hyaluronic acid could be a treatment option for ankle osteoarthritis.
