3.A Case of Dermatitis Artefacta Treated with Pimozide.

We report a case of 64-year-old female patient who has had factitious skin lesions for 20 years. She complained formification sense and severe itching on her face. These symptoms were improved with squeezing and bleeding. There were scuare shaped ulcerated nodule on the right cheek and white depressed scar on the left cheek. She was treated with wet dressing and occlussive dressing, and skin lesions were nearly cleared within 3 weeks, but new lesion appeared. Pimozide 2mg #1 p.o were given under the diagnosis of delusion of parasitosis from June, 1984. Her symptoms were improved without recurrence till now. Treatment with pimozide in monosymptomatic hypochondriacal syndrome is discussed.

4.A Case of Dermatomyositis Associted with Maxillary Carcinoma.

A 30-year old female had proximal muscle weakness, periungal telangiectasia and purplish red erythema and edema on the face. Diagnosis of dermatomyositis was estabiiished by clinical manifestations, muscle enzyme study, electromyogram, histologic findings of skin and muscle. She was treated with systemic steroid with some improvemellt. About 1 year after the onset of dermatomyositis, aggravated skin lesion and exophthalmos were noted on right eyelid. Orbit computerized tomography scan showed maxillary carcinoma. She treated with systemic steroid, 5-fluorouracil and cis-platin with partial remission and some improvement of dermatomyositis.

5.Study on the Body Temperature Measuring Time and Accuracy and Reliability of Tympanic Thermometer.

This study was to investigate the method for shortening the body temperature(BT) because it takes a long time and is impractical to measure axillary or oral BT with mercury thermometer. The first approach was to identify BT change according to the measuring time and determine the clinically not statistically available and optimal BT measuring time. The second was to test the accuracy of tympanic thermometer. It can measure BT within a few seconds, so if it is approved accurate, we can save BT measuring time by substitute tympanic thermometer for mercury thermometer. This study was conducted from 1, to 30 June, 1996. The subjects were 12 men students of medical college and 29 women students of nursing school. The results were as follows ; 1) The 3, 5, 7, 9, 11, 13 minute-measured axillary BT and 3, 5, 7, minute-measured BT showed somewhat linear relationship with time. It was difficult to find the optimum measuring time which were clinically significant. 2) For axillary temperature, the measuring time which were not statistically different was 11 and 13 minute. But the real BT difference between 3 and 13 minute, or between 5 and 13 minute were very small and was within the range of daily variation. 3) For oral temperature, there was no intervals which showed the statistically insignificant. But like as axillary temperature, the difference between 3 and 7, or 5 and 7 minute were trivial by 0.3degrees C and by 0.1degrees C respectively. 4) tympanic temperatures were lower than oral BTs which were measured with mercury thermometer by 0.26degrees C(with ear tug) and 0.15degrees C(without ear tug). 5) The reliability of repeated measure tympanic temperature was better than without ear tug. With above results, we can't determine the optimal and clinically significant oral and axillary measuring time using mercury thermometer. However, because the real differences between measuring times were very small, so we recommend further study for the aged, the infants and the febrile patients. And we can't sure the accuracy of tympanic temperature but the reliability was better with ear tug than without ear tug.

6.Repair of Surgical Wounds After Basal Cell Carcinoma Removal.

BACKGROUND: Basal cell carcinoma(BCC) is the commonest type of skin malignacy, and its incidence is increasing. As a result, the number of cases requiring treatment by dermatologists may also be increasing. OBJECTIVE: The aim of this study was to clarify the presentation pattern of BCC and to assessce the surgical treatment used in the dermatological surgery clinics in Seoul National University Hospital and Seoul City Boramae Hosptial between 1994 to 1998. METHOD: We reviewed the medical records and checked clinical aspects and surgical treatment methods. RESULTS: The Total number of patients was 33. There were 21 Females and 12 males. The Mean age was 63.1. The most common anatomical site was the face and scalp(30 cases, 91%). The mean size of the tumor was 12.2 by 9.6 mm. BCCs were removed by complete surgical excision with a normal skin margin of 2 to 4 mm. The local flap was most commonly used to repair surgical wounds (55%) and primary closure was used in 39% of patients. A Pedicle flap and full thickness skin graft were also used in surgical defect reconstruction. CONCLUSION: With increasing public awareness and earlier presentation there may be a reduction in tumor size at the time of diagnosis. As most BCCs are found on the head and neck and are usually more or less than 1cm in diameter, it is thought that appropriate education of surgical skills should be needed and offered.

7.The Effects of a Support Group Intervention on the Burden of Primary Family Caregivers of Stroke Patients.

PURPOSE: The purpose of this study was to develop and evaluate the effects of a support group intervention on the burden of primary family caregivers of stroke patients. METHOD: A nonequivalent control group pretest-posttest design was used. The subjects were 36 primary family caregivers of stroke patients [experimental(N=18) and control(N=18) groups] in a neurosurgery ward of a university hospital. The experimental group members participated in six sessions of a support group intervention for two weeks and the degree of their caregiving burden was evaluated. Data was analyzed by Chi-square tests, t-tests, and paired t-tests using SPSS 10.0. RESULT: The experimental group had a significantly lower total burden score (t=2.061, p= .047)and sub-scales of emotional(t=-3.319, p= .002), time-dependent(t=-2.045, p= .049) and developmental(t=-2.656, p= .012) burden scores than the control group, while no significant differences were found in physical, social or financial burden scores between the two groups. Within the experimental group, there was a significant decrease in physical(t=2.507, p= .023), emotional(t=4.754, p= .000), social(t=2.932, p= .009), time- dependent(t=5.015, p= .000) and developmental(t=7.541, p= .000) burden scores but not the financial burden score. CONCLUSION: The results suggest that a support group intervention can be utilized as an effective nursing program to reduce the burden of primary family caregivers of stroke patients.

8.The Effects of Nurses’ Knowledge of Withdrawal of LifeSustaining Treatment, Death Anxiety, Perceptions of Hospice on Their Attitudes toward Withdrawal of Life-Sustaining Treatment

Purpose:
This descriptive study investigated the effects of nurses’ knowledge of withdrawalof life-sustaining treatment, death anxiety, and perceptions of hospice care on their attitudes toward withdrawal of life-sustaining treatment.
Methods:
Data were collected from 262 nurses at tertiary hospitals, general hospitals, or primary hospitals in Busan, Korea, and statistically analyzed using the t-test, analysis of variance, the Scheffé test, Pearson correlation coefficients, and hierarchical regression analysis.
Results:
The participants’ scores were 3.68±0.45 (out of 5) for attitudes toward withdrawal of life-sustaining treatment, 0.65± 0.15 (out of 1) for knowledge of withdrawal of life-sustaining treatment, 2.61±0.26 (out of 4) for death anxiety, and 4.06±0.43 (out of 5) for perceptions of hospice care. Furthermore, knowledge of withdrawal of life-sustaining treatment and perceptions of hospice care showed positive correlations with attitudes toward withdrawal of life-sustaining treatment, while death anxiety showed a negative correlation. The most significant factors influencing attitudes toward withdrawal of life-sustaining treatment were perceptions of hospice care, followed by having experienced caring for patients who withdrew life-sustaining treatment, death anxiety, having a spouse, and ethical values, and the overall explanatory power was 43.0%.
Conclusion
This study showed that perceptions of hospice were an important fac-tor influencing nurses’ attitudes toward withdrawal of life-sustaining treatment. Therefore,it is necessary to develop and validate educational intervention programs that can improve perceptions of hospice care.
9.Treatment of Peripheral Artery Pseudoaneurysm: Three Case Reports.

The pseudoaneurysms are resulted from complication of vascular catheterization, trauma, etc. and recently, the reports on pseudoaneurysms have been increasing. Successful treatment of pseudoaneurysms have been described usingdirect compression guided by color doppler ultrasound, instead of invasive surgical treatment. Authors experienced three cases of pseudoaneurysm ; two resulted from post-catheterization and one from trauma. We successfully treated two post-catheterization cases using C-clamp compression under the color doppler US-guidance. The traumatic case was treated by embolization instead of US-guided compression. We suggest that the compression of pseudoaneurysm using C-clamp under the US-guidance is more useful than hand or probe compression method.

10.The Sonographic Measurement of the Length of Spleen in Normal Children.

PURPOSE: The purpose of this study was to establish the guidelines for normal splenic length by sonographic measurement. MATERIALS AND METHODS: In 481 children (age range : 2-16years, M:F=1.5:1), abdominal sonography for the purpose of health screening was carried out, and the greatest length of spleen between the dome and tip was measured. Correlation and regression analysis was used to determine whether the relationship between splenic length and a patient's age, height, weight and body surface was statistically significant. RESULTS: The length of the spleen gradually increased according to age. Correlation and regression analysis showed significant positive correlation (p<0.0001) between splenic length and age (r=0.610) height (r=0.630), weight (r=0.598), and body surface (r=0.628). CONCLUSION: From the mean values of spleen length in children, an index useful for clinical application might be derived.
