Xanthoma disseminatum (XD) is one of cutaneous nonhistiocytosis X, and characterized by multiple, widely distributed red, brown, yellowish papules and nodules that tend to coalesce. We present a case of XD with characteristic clinical findings and treated with the combination therapy of COz laser vaporization and high dose steroid.

2.Bilateral Segmental Neurofibromatosis Showing Different Dermatomal Distribution.

A 43-year-old woman presented with numerous cutaneous neurofibromas, limited to the left anterior chest(T2-3) and the right lower back(L1-2). These had been present for 10 years. Neither cafe-au-lait spot, intertriginous freckle, nor Lisch nodule was found. The family history was negative for neurofibromatosis. Biopsy specimens showed circumscribed, nonencapsulated neurofibromas. The present case was a rare form of bilateral segmental neurofibromatosis in that while most of the reported cases involved the same dermatome bilaterally, she had bilaterally different dermatomal neurofibromas.

3.The Treatment of Cutaneous T-cell Lymphoma with Triple Combination of Interferon alfa, Retinoid, and Photochemotherapy.

Cutaneous T-cell lymphoma(CTCL) is a rare cutaneous malignant disease and is typically a disease of older adults. There is no optimal treatment for CTCL, which ranges from topical steroid to systemic chemotherapy. Hence until curative therapy is found, therapies that keep CTCL in check and prevent progression to more advanced lymphoma may be desirable alternatives and may presetve quality of life. Herein we report our experience in treating a stage 11B CTCL patient with triple combination of interferon alfa, oral retinoid, and psoralen plus WA(PWA) therapy.

4.A Case of Congenital Vellus Hamartoma.

We report a case of congenital vellus hamartoma, which consists of numerous matured vellus hair follicles and sebaceous glands. This patient is 4-month-old male with multiple, variable sized nodules on the right epicanthsl fold area since birth. Histopathologically, some of the infundibular portion of the vellus hair shows irregular and reticulated hyperplasia. There are numerous well-formed vellus hair follicles surrounded with thick fibrous sheath and some of them are associated with rudiment sebaceous glands.

5.Clinical Effects of Photodynamic Therapy on Carcinoma In Situ of the Skin.

BACKGROUND: Photodynamic therapy(PDT) is a type of photochemotherapy that is designed to kill targeted tumor cells. OBJECTIVE: The Clinical effects of PDT were analysed for response rates, post-treatment healing and adverse effects on several cutaneous carcinoma in situ. METHOD: PDT with topical 5-aminolevulinic acid-based irradiation of corresponding 630+5nm light was performed in 6 carcinoma in situ patients who had actinic keratosis, Bowen' disease or cutaneous squamous cell carcinoma. RESULT: In all patients the clinical results were exellent with respect to initial complete responses and cosmetic outcome. CONCLUSION: PDT might be chosen as a first line treament for cutaneous carcimoma in situ.

6.Annular Atrophic Lichen Planus.

Annular atrophic lichen planus(AALP) is the most unusual variant of lichen planus that results from elastolytic activity of inflammatory cells. We report a case of AALP in 22-year-old man who had two annular pruritic plaques on the axilla. He had a two year history of skin lesions. Each lesion had elevated erythematous annular border with hyperpigmented atrophic macular center. Histopathologically, it showed typical features of lichen planus in the border of the lesion while a pattern of resolved lichen planus in its center. Elastic fibers was not found in the papillary dermis either at the border or at the center of the lesion.

7.A Case of Subcutaneous Juvenile Xanthogranuloma.

Juvenile xanthogranuloma is a congenital or perinatal tumor, 1 to 2 cm in diameter, usually located on the head. The extracutaneous lesions can occur on the eye, the lung, the epicardium, the oral cavity or the testicles. Subcutaneous form of juvenile xanthogranuloma has been reported very rarely in the literature. We report a unique case of a subcutaneous juvenile xanthogranuloma that showed 4 × 4 cm sized plaque and located on the extremity of 9-year-old girl.

9.Urodynamic Analysis of the Functional Voiding Disorders in Young Adult.

Functional voiding disorder is characterized by symptoms referable to the genitourinary organs without organic obstruction or neurovesical dysfunction. In an attempt to understand possible mechanisms of symptoms of functional voiding disorder in young male adult, we retrospectively reviewed the urodynamic findings of 51 men with functional voiding disorder between the ages of 19 and 51 (mean 40.7 years). Presenting symptoms are divided into three groups: 1) Sx. of irritation(irritation group) in 17 patients; 2) Sx. of bladder outlet obstruction(BOO group) in 25 patients; 3) Sx. of bladder outlet obstruction with episodes of overdistention (overdistention group) in 9 patients. On filling cystometry, time of first filling sense and urge sense was delayed significantly in overdistention group and involuntary contraction was found in 5 patients with BOO(20%), in 3 patients with irritation(18%), and in 1 patients with episodes of overdistention. In overdistention group, maximal voiding detrusor pressure was significantly lower and amount of postvoiding residual urine was significantly larger than other two group on pressure/flow study. Total voiding time and time to maximal uroflow rate were delayed significantly in BOO and overdistention group. Pseudo detrusor-sphincter-dyssynergia was found in 3 patients among 25 patients with BOO. Therefore, we were able to find correlations between specific symptomatic group and urodynamic parameters by this result. Results of urodynamic assessment were divided into 5 categories: Impaired detrusor contractility(IDC), detrusor instability(DI), obstruction, sensory urge syndrome, and equivocal studies. DI was found in 20% of patients with irritation or BOO. IDC was found in 80% of patients with episodes of overdistention and in 24% of patients with BOO. Thorough care is necessary in determining the treatment modality, and in follow up evaluation for patients showing IDC, DI or obstruction on urodynamic study. Conclusively, urodynamic study is thought to afford the opportunity in identifying these specific voiding disorder.
