1.Development of a Critical Pathway for Patients with Uterine Artery Embolization.

PURPOSE: The purpose of this study was to develop a Critical Pathway for Uterine Artery Embolization patients. METHOD: There were 6 steps that were taken. Step 1 was selecting a diagnosis, and Step 2 was organizing a development team consisting of 7 experts. Step 3 analyzed the medical records, and Step 4 drew up a preliminary Critical Pathway. Step 5 tested the clinical validity of the preliminary Critical Pathway, and Step 6 developed the final Critical Pathway. RESULT: The contents of the medical practices observed in the medical records were investigated in seven areas: monitoring/assessment, treatment, medication, diet, activity, consults, and education/discharge plan; and a total of 73 items was identified. The validity of the 73 items was examined by a group of specialists. 68 items were adopted, 4 items revised, 1 item removed, and 1 item was added. Using the results, a preliminary Critical Pathway was drawn up. According to the results from examining the clinical validity of the preliminary Critical Pathway with five patients for five weeks, 3 items which showed discrepancy were revised and another 3 items were added. Then, the final Critical Pathway was completed. CONCLUSION: This Critical Pathway needs to be clinically applied and continuously to measure its effects in terms of the length of stay, cost?effectiveness, and the patients' and staffs' satisfaction.

2.Dementia Knowledge and Related Factors in Middle-aged Adults.

PURPOSE: The purpose of this study was to identify the level of knowledge about dementia and to find the related factors in middle-aged adults. METHOD: The research design was a descriptive study by using a constructive self-report questionnaire with 34 items. Data were collected from 397 middle-aged adults using structured questionnaire. Data analysis was done by SPSS/WIN Programs using frequency, range from minimum to maximum, percentage, mean, SD, chi-square-test, t-test, ANOVA, and Duncan's Multiple Range test. RESULTS: The rate of correct answer of middle-aged adults was 68.7%, showing the mid-level of knowledge about dementia. There were significant differences according to age, education, marital status, job, income, information and information sources for dementia in middle-aged adults. CONCLUSION: These results indicate that Korean middle-aged adults need more education and counseling on dementia. Further studies are needed to overcome regionalism in sampling of this study and to develop standard measuring tools for the level of knowledge about dementia.

3.Childhood Obesity and Familial Environmental Factor according to the Developmental Stages: the Korea NHANES Study.

BACKGROUND: To investigate the association between childhood obesity and its risk factors according to specific childhood developmental stages. METHODS: We performed an analysis of data for 1922 children and adolescents aged 2 to 18 years obtained from the Third Korea National Health and Nutrition Examination Survey conducted in 2005. Weight and height were measured by trained interviewers. Childhood obesity was defined as BMI > or =95th percentile of the BMI cut-off point based on the Korean child growth curve. Data on socioeconomic characteristics such as age, education, occupation, income, physical activity and time spent watching television were collected using a well-established questionnaire and/or interview. RESULTS: The prevalence of obesity defined by using the Korean child growth curve was 4.1% in children aged 2 to 6, 6.3% in children aged 7 to 12, and 8.7% in adolescents aged 13 to 18. In the multiple logistic regression model, parental obesity, and time spent watching television were associated with increased risk of obesity in children aged 2~6. Parental obesity, family income level, birth weight, and time spent watching television were positively associated with obesity in children aged 7~12. In adolescents aged 13~18, participation in vigorous physical activity and attempts to control weight were associated with adolescent obesity. CONCLUSION: The prevalence and risk factors of childhood obesity vary substantially according to developmental stage. Differential approaches are needed for effective control of childhood obesity.

4.A Case of Herpes Simplex Virus Esophagitis in a Renal Transplant Child.

Herpes simplex esophagitis can occur in those with normal immune function, but is more often seen in those who are immunocompromised. In one series, 5 percent of post-kidney transplant recipients had herpes esophagitis. We experienced a case of herpes simplex esophagitis, following renal transplantation in a 9 year old male. He complained of epigastric pain, nausea and blood-tinged vomiting. Endoscopic examination showed volcano ulcer, mucosal friability and multiple confluent ulcers covered by whitish exudates on elevated margin in the middle and lower esophagus. Microscopic findings revealed multinucleated giant cells, margination of chromatin, intense nonspecific inflammation and strong positive for herpes simplex virus immunohistochemical staining. Esophageal lesions and symptoms improved after acyclovir therapy.

6.Effects of Low- or Moderate-dose Whole Body-X-ray Radiation on the Immune System of C57BL/6 Mice.

PURPOSE: Increase in the use of diagnostic imaging or occupational exposure to radiation have brought upon concerns on the safety and biological effects of low- or moderate-dose radiation. However, limited information is available on the effects of low or moderate dose radiation on human health. METHODS: Using C57BL/6 mice, we aimed to evaluate the biological effects of low- and moderate-dose radiation on the immune system. X-rays was chosen as a radiation source and we analyzed complete blood counts, various lymphocyte subsets and various cytokine levels after single fraction x-ray exposure (0.1 Gy, 1 Gy). RESULTS: No significant changes in the immunologic parameter of C57BL/6 mice were observed after radiation, except LIX (a cytokine equivalent to human CXCL5), that showed higher level after 0.1 Gy radiation compared to the control. CONCLUSION: We observed that a single fraction of low or moderate dose of X-ray radiation does not cause significant changes in the immune system of C57BL/6 mice. Further studies are necessary to elucidate the mechanism underlying our results.

7.Influences of Critical Thinking Disposition, Nurse Managers’ Transformation and Transactional Leadership Style on Nurses’ Competency with Evidence Based Practice

Purpose:
This study was conducted to identify the influences of critical thinking disposition and perceived nurse managers' leadership style on nurses' competency with evidence based practice in hospitals.
Methods:
The participants were 147 nurses working in two university hospitals in Seoul, Korea. Data were collected during February and March in 2017 and were analyzed with Multiple Regression Analysis using SPSS/WIN 18.0.
Results:
The most influential factor on nurses' competency of evidence based practice was critical thinking disposition (β=.55, p<.001), followed by transformation (β=.20, p=.011) which together explained their competency of evidence based practice up to 53.0%.
Conclusion
It is necessary to develop intervention programs and curriculum to enhance competence for nurses. It is also necessary to draw plans to develop nursing managers’ transformational leadership. Therefore not only clinical nurses but also throughout the organization, strategies and curriculums should be developed to help nurses perform evidence based practice.
8.Two Children with Saethre-Chotzen Syndrome Confirmed by the TWIST1 Gene Analysis.

Saethre-Chotzen syndrome is an autosomal dominant craniosynostosis syndrome, usually involving unior bilateral coronal synostosis and mild limb deformities, and is induced by loss-of-function mutations of the TWIST1 gene. Other clinical features of this syndrome include ptosis, low-set ears, hearing loss, hypertelorism, broad great toes, clinodactyly, and syndactyly. The authors of the present study report 2 children with clinical features of Saethre-Chotzen syndrome who showed mutations in the TWIST1 gene, and is the first molecular genetic confirmation of Saethre-Chotzen syndrome in Korea. The molecular genetic testing of the TWIST1 gene for patients with coronal synostoses is important to confirm the diagnosis and to provide adequate genetic counseling.

Valproic acid-induced hyperammonemic encephalopathy (VHE) is a very rare but serious complication. Discontinuation of valproic acid is the first and critical step for treatment. VHE can occur in people with normal liver function, despite normal doses and serum levels of valproic acid, therefore it is very hard to predict. Recently, we experienced a case of VHE. Here we will present the clinical, laboratory and electroencephalography findings in this patient.
