1.Manager's Attitude about Health Management of Workers in Coal Mine Industry.

This study was planned to investigate employer's attitude about health management of workers in coal mine industry. The sample size was 38.3%(178 coal mine industries) of total 463 coal mine industries. The mailing survey was used to collecting data of coal mine industry and manager of coal mine industry. Distribution of attitude about health management of workers in coal mine industry, specifically necessity and availability of some items of health management and some apparatus of working environment, was skewed to positive attitude. While recognition of susceptability of coal workers' pneumoconiosis was low, that of seriousness of incidence of coal workers' pneumoconiosis was high.

2.Prevalence and Psychiatric Comorbidities of Internet Addiction in a Nationwide Sample of Korean Adults.

The goal of the study was to examine the prevalence and psychiatric comorbidities of Internet addiction in a nationwide sample of Korean adults. Among a total of 6,022 participants involved in Korean Epidemiological Catchment Area study in 2011, 3,723 Internet users were assessed for their Internet addiction using Young's 8-item Diagnostic Questionnaire. The results showed that of the 3,723 Internet users, 39 (1.0%) had Internet addiction. Addicted Internet users were younger, more likely to be males and unemployed compared to non-addicted Internet users. Internet addiction was associated with alcohol and nicotine use disorders, depressive disorders, and anxiety disorder after controlling for age, gender, and occupational status. Internet addiction was associated with suicidal plans even after controlling for psychiatric disorders and socio-demographic factors. This study showed that Internet addiction is highly associated with variety of different psychiatric disorders and suicidal plans. Therefore, clinicians should carefully evaluate them when treating addicted Internet users.

3.Childhood Exposure to Psychological Trauma and the Risk of Suicide Attempts: The Modulating Effect of Psychiatric Disorders.

OBJECTIVE: We examined whether childhood exposure to psychological trauma is associated with greater suicidality and whether specific psychiatric disorders modulate this association in a representative sample of Korean adults. METHODS: The Korean version of the Composite International Diagnostic Interview 2.1 was administered to 6,027 subjects aged 18-74 years. Subjects who experienced a traumatic event before the age of 18 years, the childhood-trauma-exposure group, were compared with controls without childhood trauma exposure. RESULTS: Childhood exposure to psychological trauma was associated with lifetime suicidal ideation (OR=3.19, 95% CI=2.42-4.20), suicide plans (OR=4.15, 95% CI=2.68-6.43), and suicide attempts (OR=4.52, 95% CI=2.97-6.88). These associations weakened after further adjustment for any psychiatric disorders, but they were not eliminated. The risk of suicide attempts related to childhood trauma increased with the presence of a concurrent alcohol use, depressive, or eating disorder. CONCLUSION: In terms of clinical implications, patients with these disorders who have a history of childhood trauma should be carefully assessed for their suicide risk and aggressively treated for psychiatric disorders.

4.Association of Body Mass Index with Suicide Behaviors, Perceived Stress, and Life Dissatisfaction in the Korean General Population.

OBJECTIVE: The purpose of the present study was to investigate the association between Body Mass Index, suicide, perceived stress, and life dissatisfaction in a general population sample of Korean adults. METHODS: A total of 6,022 nationally representative adults aged 18 to 74 were selected using a multistage cross-sectional cluster sampling method. Questionnaires regarding suicide behaviors, perceived stress, and life satisfaction were completed by the participants. They also reported their heights and weights, which were used to calculate BMI. Psychiatric disorders were diagnosed according to the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, using the Korean version of the Composite International Diagnostic Interview. RESULTS: The results showed that being underweight was associated with higher risk for suicide ideation [odds ratio (OR), 1.6; 95% confidence interval (Cl), 1.18–2.05] and suicide attempt (OR, 2.0, 95% Cl, 1.23–3.31). Likewise, obesity also increased the risk of suicide ideation (OR, 1.3; 95% Cl, 1.11–1.56) although not suicide attempt. Furthermore, underweight individuals were more likely to report severe level of perceived stress (OR, 1.7; 95% Cl, 1.26–2.17) and life dissatisfaction (OR, 1.3; 95% Cl, 1.07–1.68). All of the results remained significant after adjusting for age, gender, education, and psychiatric illnesses. CONCLUSION: This study found that being underweight is a significant risk factor for suicide and poor subjective wellbeing in Korea. It suggests that BMI status may be an important modifiable factor for improving mental health in Korea.

5.Effectiveness of Endovascular Treatment of Acute Thrombosis of Arteriovenous Graft for Hemodialysis by Primary Endovascualr Treatment Strategy.

PURPOSE: Acute thrombosis of arteriovenous graft (AVG) has been treated by surgical thrombectomy; however, endovascular treatment became an alternative treatment option recently. The purpose of this study is to evaluate the effectiveness of endovascular treatment for acute thrombosis of AVG (EndoAVG) by primary endovascular treatment strategy. METHODS: This is a retrospective study of patients who underwent EndoAVG from January 2003 to December 2010 in Inha University Hospital. The patients' clinical characteristics and EndoAVG procedures were reviewed by electronic charts and X-ray films. Clinical success was defined as the residual stenosis below 30% or success of hemodialysis. RESULTS: Thirty-eight patients were enrolled. The mean age was 64+/-15.4 years in the success group and 58+/-17.2 years in the failed group. The mean duration from AVG formation to endovascular thrombectomy was 19.2+/-29.5 months. The success rate of EndoAVG was 84.2% (32/38). There were three complications after EndoAVG: two brachial artery thromboses and one rupture of a vein at the ballooning site. Six months, twelve months and twenty-four months secondary patency rate were 77.5%, 65.5%, and 42.5%, respectively. CONCLUSION: The success rate of EndoAVG was 84.2%. Its result is compatible with open thrombectomy and can be a good alternative option for the treatment of acute thrombosis of AVG. Larger number of cases is required for stronger study.

6.Management of diabetes insipidus during anesthesia for brain germinoma resection in a child : A case report.

Diabetes insipidus caused by impaired production or reduced responses to vasopressin, can occasionally be seen postoperatively in neurosurgical patients, but rarely occurs during anesthesia and surgery. An 8-year old female patient with suprasellar germinoma was scheduled for tumor resection. Anesthesia was induced smoothly and maintained mainly with sevoflurane. Several hours after anesthesia and surgery, urine output was increased with increased serum sodium concentration, indicating the occurrence of diabetes insipidus. To prevent sodium increase and replace fluid loss, 2.5% dextrose half saline was used. Though sodium concentration did not increase further, the concomitant increase of glucose complicated anesthetic management. After the completion of anesthesia and surgery, serum sodium increased further but then gradually returned to normal with conservative management. The patient was discharged without any complications.

7.Diabetes, Depression and Doctor-Patient Relationship.

Although diabetes mellitus (DM) is treatable, it is still not curable. Its chronicity is associated with a high prevalence of psychiatric disorders, especially depression in type 2 DM and learned helplessness in type 1 DM. In turn, this depression and helplessness may affect a patient's adherence to medical appointments, compliance to treatment, and effective doctor-patient relationships, which are vital to promising outcomes. This study reviews the existing literature regarding the interactional relationships between depression, DM and the doctor/patient relationship, and also suggests certain aspects of the doctor/patient relationship which can contribute to more successful treatment outcomes.

8.Revised subjects of the Current Korean Oriental Pharmacists' Licensing Examination.

This study is designed to draw out new integrated subjects of the Korean Oriental Pharmacists' Licensing Examination (KOPLE). In 2004, for the revision of subjects, we have analyzed the curriculums of the Oriental Pharmacy department, the oriental pharmacist's (OP's) job description book, and the elementary items of KOPLE. We also examined the system of the Chinese Herb Pharmacists' Examination and other health personnel licensing examinations and studied the data of items and compared them with KOPLE. We heard the public opinion on the present KOPLE. We developed a subfield of 18 subjects, a middle category of 188 items, and a small category of 1,026 items. We proposed a new KOPLE that consists of three subjects: basic oriental pharmacy, applied oriental pharmacy, and laws and regulations.

9.Validity of the Intraining Examinations and the Board Examinations -An Experience in the Korean Society of Otolaryngology.

The korean society of otolaryngology has had and experience on intraining examination since 1992. We also had the fortieth annual board examination for specialist in 1997. But we have no evidence on the validity of these tests yet. The aim of this study is to examine the validity of the intraining examinations as a tool of formative evaluation, to present a personal progress index demonstrating constructive validity, and to examine the validity of the board examination as a tool of summative evaluation. We did statistic analysis on the consecutive personal scores of 1995 and 1996 intraining examinations, and 1997 written and oral board examinations. Analysis of the averages, standard deviations, distribution curves, and Wilcoxon singed rank test on the scores of 1995 and 1996 intraining examinations demonstrated the constructive validity. Chi-square test revealed that those who had low scores in intraining examinations of two consecutive years had low scores in 1997 board examinations and personal progress index demonstrated the predictive validity. Correlation and linear regression analysis demonstrated a strong correlation between 1997 written and oral board examination. Analysis of the averages, standard deviations, distribution curves, and Spearman rank correlation coefficient revealed that 1997 written board examination had higher concurrent validity than the that of oral examination.

10.A Case Report of Displaced Anterior Junction Line Mimicking.

On PA chest radiography, the anterior junction line(AJL) is seen to project from the upper right to the lowerleft of the upper third of the body of the sternum and represents the visceral and parietal pleura of each lungand a small quantity of mediastinal fat. In a patient with volume loss or expansion of a hemithorax, the AJL showsconsiderable shift and on PA chest radiography may mimic pneumothorax or pneumomediastimum. In such cases,widening and hyperlucency of the retrosternal space, seen on lateral view, which represents herniated lung with ashift of AJL, may be helpful for differentiation from pneumothorax or pneumomediastinum.
