Recently MRI has been shown to be a sensitive and specific study for the noninvasive detection of meniscal tears and several MRI findings of bucket-handle meniscal tears have been reported. Therefore, the MRI findings of the menisci proven to be the bucket-handle tear with arthroscopy were evaluated and the typical MRI findings seen to be the bucket-handle meniscal tear of the knee were analysed. 76 patients with proven bucket-handle meniscal tears arthroscopically were reviewed retrospectively, in which MRI was performed in 24 patients. The 3 typical findings of bucket-handle meniscal tears were assessed: double PCL (posterior cruciate ligament) sign, flipped meniscus sign, and fragment in the intercondylar notch. In 245 cases of meniscus tears, bucket-handle meniscal tears were 76 cases (31.0%) with 52 cases at medial meniscus and 24 cases at lateral meniscus. As to clinical and physical findings of the bucket-handle meniscal tears, McMurray test, locking, joint line tenderness and giving way sense were presented in orders. The sites of bucket-handle meniscal tear were medial meniscus in 17 cases and lateral in 7 cases. The double PCL sign was seen in 10 cases (58.8%) of in medial bucket-handle meniscal tears and in 3 cases (42.9%) of 7 lateral bucket-handle meniscal tears. The flipped meniscus sign were seen in 3 cases (17.6%) of medial bucket-handle meniscal tears and in 3 cases (42.9 %) of lateral bucket-handle meniscal tears. Fragments were identified in the intercondylar notch in 8 cases (47.1%) of medial bucket-handle meniscal tears and in 3 cases (42.9%) of lateral bucket-handle meniscal tears. There were 19 cases (79.2%) in which at least one or more typical findings were presented on MR images. The MRI is a sensitive study for detection of bucket-handle meniscal tears but precise interpretation of MRI is needed for diagnosis of bucket-handle tears. In this respect, the above three typical findings are helpful and reproducible for diagnosis of bucket-handle meniscal tears with MR images; double PCL sign, fragment in the intercondylar notch and flipped meniscus sign.

PURPOSE: A hypoechoic lesion is the most common finding of prostate cancer, but its diagnostic value is still an area of debate. The diagnostic significance of a hypoechoic lesion and the increased blood flow observed on transrectal ultrasonography (TRUS) were evaluated for the detection of prostate cancer. MATERIALS AND METHODS: The records of 266 patient's, between January 2003 and December 2005, were retrospectively reviewed. The cancer detection rates were compared according to the presence of a hypoechoic lesion in TRUS, and estimated with respect to stratification of the prostate- specific antigen (PSA) value and digital rectal examination (DRE) findings. RESULTS: The overall cancer detection rate was higher in the hypoechoic (40 of 101 cases, 39.6%) than in the isoechoic group (30 of 157 cases, 19.1%) (p<0.05). When the cases were divided into three groups, according to their PSA level (<4, 4-10 and >10), the cancer detection rate apparently increased in those with a hypoechoic lesion in TRUS (10.3 vs. 0%, 25 vs. 13.6% and 68.2 vs. 31% respectively) (p<0.001). Furthermore, the cancer detection rate was further increased in patients with a hypoechoic lesion than in those with an isoechoic lesion after division into two groups according to the presence of a hard nodule on DRE (with hard nodule, 56.9 vs. 46.7%; without hard nodule, 20 vs. 8.1%, respectively) (p<0.05). However, hypoechoic lesions with an increased blood flow did not improve the detection rate compared to those without (p>0.05). CONCLISIONS: The hypoechoic finding on TRUS not only supports the detection of prostate cancer, but also has independent diagnostic value. However, the findings of color Doppler are not helpful in the detection of prostate cancer.

Human leukocyte antigen (HLA) class II molecules are polymorphic cell surface glycoproteins that are crucial for the cellular interaction in immune response. The expression of class II molecules is regulated in a tissue-specific and cytokine-inducible manner, and is mainly restricted to the antigen presenting cells. However, some tumor cells also express class II molecules, and in some class-II-negative tumor cells, class II expression is inducible by interferon (IFN)-gamma. However, their expression varies, even though the tumor cells originate from the same histological origin; some tumor cells show strong expression, others show weak or no expression. To determine whether this differential expression of class II molecules on tumor cells is transcriptionally regulated, FACS analysis and Northern hybridization were performed using a panel of melanoma cell lines, IGR3, Malme-3M, SK-Mel-24, and SK-Mel-28 to analyze the cell surface expression and mRNA transcription rate of HLA-DR before and after treatment with IFN-gamma. FACS analysis showed that before IFN-gamma treatment, IGR3 and Malme-3M cells barely expressed HLA-DR. On the contrary, almost all of the SK-Mel-24 cells (> 90%) and a relatively high rate (> 50%) of SK-Mel-28 cells expressed HLA-DR. After IFN-gamma treatment, HLA-DR expression was induced in Malme-3M cells and SK-Mel-28 cells which displayed elevated levels of HLA-DR expression in a time-dependent manner. However, IGR3 cells never responded to IFN-gamma. Northern analysis showed that treatment with IFN-gamma led to the steady-state mRNA augmentation of the HLA-DR gene in Malme-3M and SK-Mel-28, whereas in IGR3, IFN-gamma did not augment the transcriptional rate of the HLA-DR gene. To further clarify this differential modulation, sequencing analysis of PCR product of the HLA-DR proximal promoter region was done, since the transcription rate of the class II gene is controlled by the well-conserved proximal promoter region. Six independent clones from PCR products of the HLA-DRA proximal promoter region and 16 clones from PCR products of the HLA-DRB proximal promoter region were isolated from the above cell lines and sequenced. Comparison of the nucleotide sequences of all 6 clones of DRA promoter showed that the sequences are extremely similar in both regulatory sequences and their intervening sequences. Sixteen clones of HLA-DRB promoter showed sequence variations such as substitution and insertion/deletion, and these 16 clones could be further grouped into 6 homologues with sequence homology. These data established that the melanoma cell lines studied here showed a differential susceptibility to IFN-gamma on the modulation of HLA-DR molecules, that this modulation is transcriptionally regulated, and that the difference in promoter activity by sequence variation might contribute to such a differential transcriptional regulation at the promoter level.
