1.A survey on child battering among elementary school children and related factors in urban and rural areas.

To determine the incidence rate of child battering and related factors, a questionnaire survey was conducted on 1,255 children in 4th and 5th grades of two elementary schools (one in the upper economic class area with 519 students and the other in the lower economic class area with 504 students) in Taegu and two schools in rural areas of Kyungpook province (120 and 112 students, respectively) from 1st May to 10th May 1990. Total number of children who were battered during one-month period (1-30 April 1990) prior to the survey was 918 (73.1%)> Among the battered children 87 (6.9%) were severely battered (twice or more in a month by kicking or more severe method) and 831 children (66.2%) were moderately battered (all other battering than severe battering). The percentage of battered children and degree of battering were not significantly different between two schools in Taegu and between urban and rural areas. Common reasons for battering were disobediance (61.9%), making troubles (34.9%), and poor school performance (33.3%). However, 16.1% of severely battered children responded that the perpetrators battered them to wreak their anger and 5.7% of them did not know the reason why they were battered. A majority of the battered children (65%) regretted their fault after being battered but 20.7% of the severely battered children wanted to run away and 9.2% of them had an urge to commit suicide. While most of the physical injuries due to battering were minor as bruise (52.7%) but some of them were severe, e.g., bone fracture (2.5%), skin laceration (1.5%), and loss of consciousness (0.2%). The common psycho-behavioral complaints of the severely battered children were unwillingness to study (31%), unwillingness to live (17.2%), and reluctance to go home (13.8%). The incidence rate of severe battering was significantly higher (p=0.018) among the children living in a quarter attached to a store (14.0%) than the children living in an apartment (6.6%) and individual louse (6.2%). The incidence rate of severe battering was higher among children living in a rental house (8.4%) than children living in their own house (6.3%) (p=0.005). The children of father only working (5.1%) and mother only working (4.5%) had a lower incidence rate of severe battering than the children of both parents working (9.1%) and both parents unemployed (20.7%) (p=0.006). More children were battered when there was a sick family member (80.8%) compared with the children without a sick family member (71.4%) (p=0.001). The incidence rates of severe and moderate battering increased as the frequency of quarreling between mother and father increased (P=0.000). The percentage of unbattered children was higher among children whose father's occupation was professional (39.4%) than that of the total study subjects (26.9%) (p<0.001).

2.Isolation of a Partial Human cDNA Encoding a Factor Binding to the Perfect Palindrome of Enhancer A of HLA Class I Promoter, Homologous to NF-kB2.

Genes encoding sequence-specific DNA binding proteins have been isolated by screening cDNA libraries constructed in rgt11 expression vector with recognition site DNAs. We isolated a rgt11 recombinant human cDNA clone, designated to C2, using a DNA probe consisted of heptamer of the perfect palindrome (PP; GGGGATTCCCC) of enhancer A (Enh A) of HLA dass I promoter. Sequencing analysis showed that this clone contained a partial cDNA homologous to NF-kB2. Lysogenic E. coli containing the C2 was generated and crude cell extract was prepared. Immunoblot using anti-B-galactosidase antibody showed that this lysogenic E. coli expressed B-galactosidase fusion protein. Electrophoretic mobility shift assay (EMSA) and DNase I footprinting assay were done using crude cell extract and their patterns were compared with nuclear protein extracted from an EBV transformed B lymphoblastoid cell line (BLCL). EMSA showed that crude cell extract prepared from E. coli lysogen speci5cally bound to the PP of Enh A region of HLA class I gene. DNase I footprinting assay showed that the binding sequence of this recombinant B-galactosidase fusion protein was identical to that of nuclear protein extracted from a BLCL. Our data indicate that a Agt11 recombinant cDNA clone was isolated from a human cDNA library using the PP of Enh A of the HLA class I promoter and this clone encoded a B-galactosidase fusion protein capable of binding to the PP and belongs to a NF-xB subunit.

4.Surgical Treatment for Burst Fracture of the Thoracolumbar Spine: Anterior approach vs posterior approach

We present a prospective, randomized study of acute burst fracture of the thoracolumbar spine. Forty-one patients were treated either by anterior decompression and stabilization with Kaneda device or by posterior distraction instrumentation using the A-O fixateur interne. The mean follow up was 21 months. The result were as follows; 1. The mean preoperative kyphotic angle was 19.2° in those patients treated by anterior surgery and 21.4° in those patients treated by posterior surgery. At last follow-up the mean correction in kyphotic angle was 13.2° in the anterior group and 9.2° in the posterior group. There is no statistically significant difference between those two groups. 2. The mean preoperative midsagittal diameter of the canal compromise 47.4% in anterior group and 49.3% in posterior group. Postoperatively, this was reduced to 2.5% and 7.9%. There is a statistically significant difference between these two groups(P < 0.05). 3. The mean preoperative canal enchroachment 52.)% in anterior group and 47.6% in posterior group. Postoperatively, this was reduced to 3.2% and 6.0%. There is a statistically significant difference between these two group(P < 0.05). 4. Neurologic recovery was 81% in posterior surgery and 85% in anterior surgery. The improvement in Frankel grade was 1 grade in average, and showed no difference between two groups. 5. There was two cases of pedicle screw breadage in posterior group and one case of pyothorax in anterior group but no early or late vascular or neurologic complication.

5.Intraventricular Hemorrhage in Full-Term Neonate.

Intraventricular hemorrhage is a common hemorrhagic cerebral disorder in premature, but occurs much less frequently in the full-term newborn. In order to obtain documentation of clinical and ultrasonographic characteristics of intraventricular hemorrhage in full-term, we performed cerebral ultrasonography on 602 newborn infants who had been admitted to neonatal intensive care unit at the Deppartment of Pediatrics, Yonsei University College of Medicine between December 1989 and June 1991. 15 cases (2.5%) of intraventricular hemorrhage were analysed. The results were as follows: 1) Among 15 cases with intraventricular hemorrhage, there was no obstetrical and perinatal complication in 8 cases (53%) and 13 cases (87%) were neurologically normal at birth. 2) Sudden onset of dramatic neurologic abnormalitis inclued seizures, fever, bulging fontanel, irritability, lethargy, vomiting in cases (73%) and the remainder (27%) had no clinical symptoms and signs. 3) The age at diangosis of hemorrhage was before 72 hours in 5 cases (33%), between 4 and 7 days in 2 cases (13%), at 8 to 28 days in 8 cases (53%), 8 of 15 cases (53%) had Grade 1 IVH, four with Grade 2, cases with Grade 3, and 1 case of these infant demonstrated Grade IV IVH. The source of hemorrhage was subependymal germinal matrix in 12 cases (80%) and choroid plaxus in 3 cases (20%). 4) Precipitating factors were cerebral venous infarction in 7 cases, hypoxic injury in 5, and 3 other infants had no identifiable medical risk factors. 5) Among all 15 survivors, 2 of 3 infants with choroid plexus hemorrhage required placement of a ventriculo-peritoneal shunt, in the other case, ventriculomegaly decreased spontaneously.

8.A Case of Tsutsugamushi Disease.

Tsutsugamushi disease, an infectious disease by Rickettsia(R.) tsntsugamushi, is characterized by eschar, fever and rash. Eschar caused by chigger bite, is pre sent on 47 94% of patients with documented R. tsutsugamushi. We recently noticed one case of R. tsutsugamushi infection. Patient had fever, heaclache, lymphadenopathy, erythematous maculopapules and eschar. The R. tsu tsugamushi was isolated from serum of the patient. Histopathologic findings of eschar show epidermal ulceration overlying a zone of coagulative necrosis of upper dermis and necrotizing vasculitis. E]ectron microscopic findings of endothelial cells of capillary show rickettsia with cell division.

9.A Case of Congenital Limbal Dermoid Cyst.

dermoids are unusual congenital anomalies consisting of abnormal mesoblastic tissue covered by epithelium. They usually start at limbal region and extend centrally. The authors have experienced a case of congenital limbal dermoid cyst which is not associated with other anomalies in a 17 year old male. Cyst was locally excised with no significant difficulties and it was histologically typical. Authors review the literature dealing with dermoid cyst and clinical finding.

10.Clinical Study of Ipsilateral Fracture of the Femur and Tibia

Usually the condition of ipsilateral fracture of femur and tibia is called floating knee, it is termed applied to the flail knee joint segment. The key point of the treatment is focused to the early restoration of the knee function. We experienced 86 consecutive cases during the period from March 1978 to February 1991 in Yonsei University Wonju College of Medicine. Among them, 51 cases(above 16 years old) were analysed with the mean follow up period of 2.1 years(1.2-5.6 years). The results obtained were as follows: 1. Main cause of these fractures is traffic accident. Among them, the pedestrian accident is more than in-car accident. 2. The cases of open fracture were 13 cases (25.5%) in femur and 31 (60.8%) in tibia. The degree of open fracture was more severe in tibia. 3. There were 2 cases (3.9%) of fat embolism syndrome preoperatively. 4. At an average 25 months follow up after injury, the good and excellent result of the group 1 and 3 was 85.2%, and the group 2 and 4 was 71.4% by Karlström and Olerud criteria. 5. At the last follow up examination, the mean loss of range of motion of the knee was 5.6° in group 1 and 3, 10.9° in group 2 and 4(P < 0.05). 6. The operative treatment of the both side is better than case of one side operation and other side conservative treatment.
