Objectives: This study was aimed to assess the usefulness of the quantitative assessment of clock
drawing test (CDT) combined with the Mini-Mental State Examination (MMSE) compared to that
of the Montreal Cognitive Assessment (MoCA) or the MMSE alone for screening of dementia in
Parkinson disease (PD) in patients with a low educational level. Methods: A representative sample of
91 PD patients was administered MMSE, MoCA and CDT. The discriminative validity of the MMSE,
MoCA, and a MMSE+CDT combination for dementia screening was determined by estimating the
sensitivity and specificity of each test and by testing integrated discrimination improvement (IDI).
Results: The mean age and educational years were 69.0 (years) and 7.3 in the study population. The
best screening cut-off points for the MMSE, MoCA, and MMSE+CDT were 25/26, 21/22 and 41/42.

5.A Case of Peutz-Zeghers Syndrome.

A case of Peutz-Zeghers syndrome in a year and 8 month old girl was Presented. Her chief complaints were abdominal pain, vomiting and melanin pigmemtation on the lips and perioral area. On family history, her mother and elder sister suffered from same symptom. Physical examination showed melanin pigentation on the lips and perioral area, and markedly distended abdomen. Abdominal simple X-ray film showed fluid level, and so diagnosed as small bowel obstruction. Operation performed at 2nd admission day, and those findings were intusussusception of jejunojejunal type and multiple polyps. The diagnosis was confirmed by family history, characterisic pogmentation and multiple small intestinal polyps of jejunum. Brief review of related literatures were also presented.

9.Successful Immunoglobulin Treatment in Severe Cryptogenic Organizing Pneumonia Caused by Dermatomyositis.

In connective tissue diseases, autoantibodies cause pulmonary interstitial inflammation and fibrosis, and patients require treatment with an immunosuppressive agent such as a steroid. Dermatomyositis is an incurable, uncommon form of connective tissue disease that occasionally causes diffuse pulmonary inflammation leading to acute severe respiratory failure. In such cases, the prognosis is very poor despite treatment with high-dose steroid. In the present case, a 46-year-old man was admitted to our hospital with dyspnea. He was diagnosed with dermatomyositis combined with cryptogenic organizing pneumonia (COP) with respiratory failure and underwent treatment with steroid and an immunosuppressive agent, but the COP was not improved. However, the respiratory failure did improve after treatment with intravenous immunoglobulin, which therefore can be considered a treatment option in cases where steroids and immunosuppressive agents are ineffective.

A 25-year-old woman complaining of dyspnea on exertion, coughing, fever and chills was admitted in January, 1998. Her clinical, radiological, and pathological findings were compatible with hypersensitivity pneumonitis and precipitin antibody to Trichosporon cutaneum was detected in her serum. Although some cases of T. cutaneum-induced hypersensitivity pneumonitis developed during the winter season were reported in Japan, there had been no report of such a case in Korea. This case suggests that T. cutaneum-induced hypersensitivity pneumonitis can develop beyond the summer season in certain environments.
