1.Axial Lenth of Eye as a Risk Factor of the Branch Retinal Vein Occlusion.

Branch retinal vein occlusion(BRVO), along with central retinal vein occlusion, is second only to diabetic retinopathy as a cause of retinal vascular disease. It has been suggested that eyes with shorter axial length have smaller lamina cribrosa and a narrower scleral canal through which the central retinal vein and artery could pass, causing physical blockage in the vein which predisposes to thrombus formation. The subsequent narrowing of the venous lumen will lead to trubulence. reduced flow, venous stasis and thrombus formation at the level of the arteriovenous crossing. This study was conducted to determine whether there was any difference between the axial lengths of eyes with BRVO and those of control eyes. The axial lengths of the eyes in the group of 37 patients with a unilateral BRVO was compared with those of eyes in the control group. The BRVO group consisted of 23 male and 14 female patients and their mean ages were 61.5+/-7.0 years(51~75years) in male and 63.7+/-7.0 years(52~76years) in female. The control group consisted of 46 male and 28 female patients and their mean ages were 62.5+/-6.5 years(50~73years) in male and 63.9+/-6.1 years(52~75years) in female. The mean axial length of eyes of the BRVO group was 23.08+/-0.57mm(21.92~24.19mm) in male and 22.71+/-0.65mm(21.08~24.00mm) in female. The mean axial length of control eyes was 23.74+/-0.73mm(22.46~25.24mm) in male and 23.18+/-0.81mm(21.62~24.76mm) in female. The difference between the mean axial length of eyes of the BRVO group and that of control eyes was statistically significant(p<0.05). In conclusion, the short axial length may play as a risk factor in the occurrence of a BRVO.

2.Clinical and Histopathologic Analysis of Ductaol Carcinoma In Situ.

A retrospective analysis was performed on 69 follow-up cases of ducal carcinoma in situ(DCIS) treated from Jan. 1981 to Dec. 1995 at Department of Surgery, Seoul National University Hospital. The results are as follows. The mean age was 45.4 years that ranged from 28 to 68. The proportion of DCIS to breast cancer was increased from 1.3% to 7.2% for 15 years. Clinical presentations of DCIS were mass, nipple discharge and suspicious mammographic findings. Patients presenting mass were decreased, while mammographically detected DCIS were increased. In 49.3% of cases, the tumor size was smaller than 2cm and in 21.3% of the cases, the tumor was not palpable. Treatment modality was changed to more conservative procedures. All of the patients survived well without local recurrence or distant metastasis for a mean follow-up of 36.3 months(range 7 to 146). Histopathologic review was also performed on 52 cases of which paraffin blocks were saved, by one pathologist and diagnosis was confirmed. Pure DCIS composed 16 cases(30.8%), and DCIS with microinvasion was 36 cases(69.2%). Clinical presentation of DCIS, such as age, nipple discharge, calcification on mammography and mass size were not related to the microinvasion, but mass palpability was related to microinvasion(p=0.018). There was no histopathologic features to predict progression to invasive carcinoma, regardless of comedo type, histologic grade and nuclear grade. The expression of cathepsin-D, nm23, p53 and c-erbB-2 protein, was also evaluated using immunohistochemical methods. The relationship between expression of immunohistochemical antibodies and prognostic variables such as tumor size, histologic grade, nuclear grade, and microinvasion was assessed. The overexpression of c-erbB-2 protein was associated with lower nuclear grade(p=0.033), and a strong correlation was seen between tumor size and cathepsin-D and p53 protein(p=0.035, p=0.016). In this study, we conclude that the fine classification and analysis of DCIS subtype, not only by histopathologic but also by immunohistochemical study can be helpful to predict biological behaviour of DCIS and decide the modality of treatment.

3.Two Cases of Refractory Status Epilepticus Treated with Ketamine.

Clinical observations and recent experimental studies have suggested that the longer status epilepticus (SE) persists, the more difficult it is to control SE pharmacologically. These findings imply that there are fundamental pathophysiologic processes, which make more resistant to intervene in the refractory SE. Recently, it has been recognized that ketamine, N-methyl-D-aspartate receptor antagonists, are effective agents in the treatment of the late stages of SE in the animal model. However, only one clinical experience has been reported. Here, we report two cases with refractory SE, responsive to ketamine.

4.A Longitudinal Study on Moral Judgment Development in Nursing and Medical Students.

PURPOSE: In this longitudinal study, we examined the moral judgment level and its related factors, such as individual characteristics. The result of this study will provide baseline data to establish policy of ethics education for college students and healthcare professionals. METHOD: We enrolled 37 nursing students and 20 medical students as the subjects in a university in Suwon, Korea. We conducted a questionnaire on the subjects using Korean version of Defining Issues Test(DIT) to analyze the subjects' moral judgment level. Collected data was coded using ASCII document and scored using Fortran program for computer. Then the data was statistically analyzed by SPSS Version 10.0. RESULT: Nursing students' moral development score at each stage were consistently higher at stage 5A across 4-years rather than other stages. On the other hand, medical students' moral development score were consistently higher at stage 4 than other stages. There was no significant difference in the change P(%) score at each academic year in both groups. In the perspective of the subjects' general characteristics, P(%) score showed no significant in both groups. CONCLUSION: Based on the results of this study, further studies will examine the correlation between curriculum and moral judgment development in detail. Moreover, we suggest that the current ethics education should be developed and evaluated in more realistic manner.
5.Study on the Relationship between the Nutrient Intake and Level of Depressive Symptoms: Using the Data from the Korea National Health and Nutrition Examination Survey 2016

The purpose of this study was to examine the relationships between the nutrient intake and level of depressive symptoms. A total of 5,761 subjects with depression were selected for the main data analysis from a Korean National Health and Nutrition Examination Survey, KNHANES (2016), and were divided into low depressed and high depressed groups. The data were analyzed by descriptive analysis, χ2-test, logistic regression, and t-test using SPSS 24.0 statistics. The results revealed significant differences in the prevalence of depression according to sex (P < 0.001), age (P < 0.001), income (P < 0.001), and education (P < 0.001). Regarding the health behavior, higher stress levels (P < 0.001) and smoking habit (P < 0.001) appeared to be strongly associated with high levels of depression. On the other hand, the possibility of experiencing depressive symptoms was lower when the number of walking days per week was more than 3 days (P < 0.05), when the number of working days per week was more than 5 days (P < 0.01), and the rate of aerobic physical activity was higher. Based on the ‘Dietary Reference Intakes for Koreans 2015’, the subjects in the low depression group showed better intake of energy and 22 nutrients than the high depression group.

6.Generation and maintenance of type II collagen-specific T-cell line expressing conserved TCR-CDR3 motifs among patients with rheumatoid arthritis Author.

BACKGROUND: To determine the molecular structure of type II collagen-specific T-cell receptors associated with rheumatoid arthritis (RA). METHODS: We generated CII-specific T-cell lines of 8 RA patient s by prolonged in vitro culture with bovine CII (bCII) and the immunogenic peptide (256-270) of human CII. The proliferation response towards CII stimulation was measured from the uptake of 3 H-thymidine. Changes in the secretion of Th 1 and Th2 cytokines in the culture supernatent were measured by ELISA. The TCR clonotypes of these T-cells were examined by RT-PCR/ SSCP analyses of all 22 V beta chains. RESULTS: T-cells from patients' tissue exhibited strong proliferation index upon CII stimulation, which was maintained up to 6 months in the culture. The secretion of INF-gamma from these T-cells increased along with the duration of culture time, while the amount of IL-4 production did not show significant changes. The SSCP band patterns of patients' T-cells appear as discrete bands unlike the smeary streak produced from normal samples. Some SSCP bands, each representing selected expansion of a TCR containing certain subtype of V beta peptides, appeared to be identical in more than one patients. Among these, the expansion of SSCP band representing the V beta 14 CDR3 region persisted after switching the antigen to the immunogenic human peptide (256-270). CONCLUSION: CII-reactive T-cells expressing distinct CDR3 motifs are selectively expanded in the peripheral blood and synovial fluid of RA patients, and their persistent proliferation upon CII stimulation, as well as the production Th 1-type cytokines, may play pivotal roles in RA pathogenesis.

8.A Case of Rhabdomyolsis and Acute Renal Failure Associated with Tsutsugarnushi Disease.

We first report a case that demonstrates superi-mposed acute renal failure associated with rhabdo-myolysis in a patient with tsutsugamushi disease. The clinical presentation was that of insidious onset of fever, abdominal pain, back pain, renal failure and respiratory failure over about 20 days period. The renal function was progressively deteriorated and the serum creatinine concentration increased from 1.4 to 8.4mg/dL, while the patient was in a state of septic shock caused by tsutsugamushi disease. The laboratory data including increased muscle enzymes such as CK, LDH, SGOT, myoglobin, positive test for urine myoglobin, and increased uptake of bone scan were consistent with rhabdomyolysis. On consideration of the cause of rhabdomyolysis in this patient, endotoxin made in the state of septic shock had triggered a muscle necrosis and acute renal failure aggravated by dehydration and metabolic acidosis. She was recovered by the proper use of a IV doxycycline as the therapy of tsutsugamushi disease with a maintenance of renal blood flow by hydration, mannitol, bicarbonate and protection of hypotension. We suggest that rickettsial disease, especially tsutsugamushi disease can result in the acute renal failure by rhabdomyolysis.

9.Berberine Inhibited the Growth of Thyroid Cancer Cell Lines 8505C and TPC1.

PURPOSE: Thyroid cancer is the most common malignancy in Korean females and can be treated with good prognosis. However, drugs to treat aggressive types of thyroid cancer such as poorly differentiated or anaplastic thyroid cancer have not yet been established. To that end, we analyzed the effects of berberine on human thyroid cancer cell lines to determine whether this compound is useful in the treatment of aggressive thyroid cancer. MATERIALS AND METHODS: The two thyroid cancer cell lines 8505C and TPC1, under adherent culture conditions, were treated with berberine and analyzed for changes in cell growth, cell cycle duration, and degree of apoptosis. RESULTS: Following berberine treatment, both cell lines showed a dose-dependent reduction in growth rate. 8505C cells showed significantly increased levels of apoptosis following berberine treatment, whereas TPC1 cells showed cell cycle arrest at the G0/G1 phase. Immunobloting of p-27 expression following berberine treatment showed that berberine induced a little up-regulation of p-27 in 8505c cells but relatively high up-regulation of p-27 in TPC1 cells. CONCLUSION: These results suggest that berberine treatment of thyroid cancer can inhibit proliferation through apoptosis and/or cell cycle arrest. Thus, berberine may be a novel anticancer drug for the treatment of poorly differentiated or anaplastic thyroid cancer.

10.Histologic confirmation of huge pancreatic lipoma: a case report and review of literatures.

Pancreatic lipomas are commonly diagnosed based on radiologic images, although the prevalence of lipomas has not been established. Histologic confirmation of pancreatic lipomas is extremely rare because surgical treatment is unnecessary in most cases. Endoscopic ultrasound-guided fine-needle aspiration cytology has been suggested to avoid unnecessary surgery to distinguish between a lipoma and a well-differentiated liposarcoma; however, surgery would be needed when the tumor is associated with symptoms or difficult to distinguish from a liposarcoma. We present a case of a pancreatic lipoma in a 54-year-old male patient that was histologically-confirmed by subtotal pancreatectomy.
