3.A Clinical and Histopathological Review of Sweet Syndrome in Korean Children and Adolescents

Background:
Sweet syndrome is characterized by tender erythematous plaques and nodules with predominantly dermal neutrophilic infiltrates. Sweet syndrome is relatively rare in children and adolescents. To date, only a few cases have been reported in Korean literature.
Objective:
The aim of this study was to investigate the clinical and histopathological features of Sweet syndrome in Korean children and adolescents.
Methods:
A retrospective study was conducted on 15 pediatric patients (aged <18 years) who were diagnosed with Sweet syndrome between 1991 and 2019. We reviewed the clinical and histopathological features of Sweet syndrome.
Results:
The age of the 15 patients ranged from 4 months to 17 years. Among the 15 patients with Sweet syndrome, nine patients were females and six patients were males. Most patients (80%) had lesions on the upper extremities. Fever and tenderness (60%) were the most commonly associated symptoms. Transient infections such as upper respiratory infection or gastroenteritis were the most common identifiable cause, observed in 40% of patients. Histopathologically, dermal neutrophilic infiltration was observed in all patients. All patients were treated with systemic corticosteroids and showed a good response, although 26.7% of the patients experienced symptom recurrence.During the follow-up period, there were no incidences of any complications or extracutaneous manifestations in the patients.
Conclusion
In contrast to previous reports of pediatric Sweet syndrome, female predominance was observed in this study. Transient infection was the most common factor. All patients responded well to systemic corticosteroid therapy without complications or extracutaneous manifestations during the follow-up period.

5.Coinfection of Viral Agents in Korean Children with Acute Watery Diarrhea.

Currently, there are a few reports on viral coinfection that causes an acute watery diarrhea in Korean children. So, to evaluate the features of coinfectious viral agents in children with acute watery diarrhea, we enrolled 155 children with acute watery diarrhea from July 2005 to June 2006. Fecal samples were collected and evaluated for various viral infections such as rotavirus, norovirus, adenovirus and astrovirus. The mean (+/-standard deviation) age of the children was 2.71+/-2.37 yr. The detection rate of viral agents was most common in children between the ages of 1 and 3 yr. Rotavirus was detected in 63 children (41.3%), norovirus in 56 (36.2%), adenovirus in 11 (7.1%), and astrovirus in 1 (0.6%). Regarding rotavirus, there were 38 (60.3%) cases with monoinfection and 25 (39.7%) with coinfection. For norovirus, there were 33 (58.9%) cases with monoinfection and 23 (41.1%) with coinfection. Coinfection with rotavirus and norovirus was most common, and occurred in 20/155 cases (12.9%) including coinfection with adenovirus. So, rotavirus and norovirus were the most common coinfectious viral agents in our study population with acute watery diarrhea.

6.The comparison of quality of life and stress among healthy, allergy and other-diseases groups.

BACKGROUND AND OBJECTIVE: In assessing the health-related quality of life of subjects with allergy, it is important to measure outcomes which are relevant to the concerns of patients. The goals of this study were to compare health-related quality of life and stress level among allergy, healthy and other-diseases groups, and also to compare the relationship of health-related quality of life and stress. METHOD: The study group consisted of 129 subjects with allergy, 139 healthy subjects and 68 subjects with other-diseases. The allergy group was divided into two sub groups: pure allergy group(65) and allergy group with other diseases(64). They completed a questionnaire measuring perceived health-related quality of life, stress and demographic characteristics. The health-related quality of life score was measured by Notingham Health Profile and the stress level was measured by Ok-Suk Lee's instrument. RESULTS: (1) In allergy group with other diseases, the total scores of health-related quality of life were significantly higher than for healthy, pure allergy and other-diseases groups. The results indicated that the perceived health-related quality of life of allergy group with other diseases was in the lowest status among the four groups. (2) Forty-two percent of allergy group with other diseases were in need of treatment for stress level. (3) Level of stress and scores of health-related quality of life have shared linear correlation in healthy and allergy group; higher stress indicates lower status in the quality of life for both groups. CONCLUSION: This study was examined the QOL and the stress level of allergy subjects. When compared with healthy and other-diseases groups, the scores of QOL and the level of stress of the pure allergy group were higher than those of the healthy group, and those of the allergy group with other diseases were also significantly higher than those of the other-diseases group. These results suggest a necessity for futher assessment to improve the QOL and to reduce the stress in allergy subjects.

7.Perforating Granuloma Annulare Mimicking Papulonecrotic Tuberculid.

Perforating granuloma annulare (PGA), a rare variant of granuloma annulare, is characterized by transepidermal elimination of altered collagen that clinically manifests an umbilicated papule with a central crust. It can be confused with papulonecrotic tuberculid (PNT) because of their similar appearance. Unlike PGA, PNT is usually related to tuberculosis infection with a typical histologic finding of wedge-shaped dermal necrosis. Here, we report the first Korean case of PGA mimicking PNT both clinically and histologically. A 43-year-old Korean woman presented with erythematous papules localized on the extensor surface of her limbs for one year. Some of these papules had a central umbilication or a crust. Regarding comorbidity, she had latent tuberculosis diagnosed with QuantiFERON®-TB Gold test about five months ago. She was on antituberculous medication. Initially, a diagnosis of papulonecrotic tuberculid accompanied by latent tuberculosis was considered. However, despite taking the antituberculous medication for five months, her skin lesions were not improved. Biopsy specimen from her arm lesion showed wedge-shaped area of necrosis in the dermis. Additionally, there were multiple focal mucin depositions and palisading granulomatous inflammation throughout the dermis. A diagnosis of PGA was made and she was treated with topical corticosteroid. After two weeks of applying topical corticosteroid, most of her skin lesions disappeared, leaving some hyperpigmented scars.
