1.Extensive Adenomatoid Odontogenic Tumor of the Maxilla: A Case Report of Conservative Surgical Excision and Orthodontic Alignment of Impacted Canine.

The present report describe the surgical therapy, clinical course, orthodontic treatment and morphological characteristics of an adenomatoid odontogenic tumor in the maxilla of an 11-year-old patient. The cystic tumor filled the maxillary sinus and involved a tooth. Marsupialization was accompanied by partial enucleation and applied traction to the affected tooth by a fixed orthodontic appliance. Healing was uneventful and no local recurrence was observed during a 1-year period of follow-up control.

2.Succesful Treatment of Pruritus in Pregnancy with Broadband UVB Phototherapy.

A 31-year-old pregnant woman with pruritus in pregnancy was described; she presented with a one-week history of generalized pruritus at gestational age 6 weeks without primary skin lesion. She had no dermatologic problem before pregnancy. Her pruritic symptom was not con-trolled with conventional emollient and topical corticosteroids. Broadband Ultraviolet B(UVB) phototherapy successfully improved her pruritus. UVB phototherapy also worked well after recurrence of pruritus at pregnancy 23rd week. We report herein a case of pruritus in pregnancy successfully treated with broadband UVB phototherapy.

6.Risk Factors for Neonatal Sepsis in Premature Infants Admitted to Neonatal Intensive Care Unit.

PURPOSE: The purpose of this study is to find out the risk factors of sepsis except the prematurity itself. METHODS: Seventy-seven premature infants less than 37 weeks of gestation, who were admitted to the neonatal intensive care unit of Hanyang University Kuri Hospital between July 1995 and December 1996, were enrolled in this study. All 77 premature infants, 14 patients proven to have sepsis by blood culture. They were matched with 24 controls by gestational age and date of birth. The risk factors for neonatal sepsis between the groups were compared. RESULTS: Fourteen of 77 subjects(18.2%) had culture-proven sepsis. Among them, all but one, who had confirmed sepsis on the first day of life, were late-onset >72 hours of age(16+/-6 days of life). Gestational age and birth weight of sepsis group(mean+/-SD) were similar to those of controls: 31.6+/-3.6 week vs 31.5+/-3.3 weeks; 1673+/-832g vs 1651+/-513g, and survival rate was also similar between the two groups(n=ll, 79% for sepsis group vs n=20, 77% for the control). In the analyses of risk factors of sepsis only parenteral nutrition and duration of parenteral nutrition were significantly more frequent and longer in the sepsis group compared to the control(n=8, 73% vs n=6, 30%, P=0.022; 20+/-15 days vs 7+/-4 days, P=0.0364). CONCLUSION: Incidence of neonatal sepsis in premature infants <37 weeks of gestaton admitted to neonatal intensive care unit was 18.2%. Ninty-three pevcent of the infants was late-onset sepsis. Sepsis group had more frequent use and longer duration of parenteral nutrition. In order to reduce neonatal sepsis in premature infants, use and duration of paventeral nutrition should be restricted.

7.A Case of Median Raphe Canal of the Penis.

Median raphe canal is an uncommon disease and represents a defect abnormality of the male genitalia. It occurs along the ventral median raphe from the glans penis to the anus. Histologically the canal is lined by stratified squamous epithelium which dose not communicate with the urethra. Surgical excision is the treatment of choice. Recently we observed a 25-year-old male patient who had had a median raphe canal located in the ventral aspect of penis.

8.Clinical Observation of Subdural Effusion.

63 cases with subdual effusion who had been admitted to the pediatric department of Hanyang university hospital from May 1972 to July 1977. And following result was obtained. 1) Among 63xases, Age distribution of patients were under 6 months of age with peak incidence under one month of age (82.5%). Sex incidence revealed a male predominance with the ratio of 4 : 1 2) The chief complaints on admission were as follows fever (47.6%), Poor sucking (30.1%), Jaundice (30.1%), Vomiting (20.5%), Dyspnea(17.4%), Irritability(17.4%). 3) 28 cases had abnormal findings in transillumination (65%) among 48 cases, exudate in 13 cases, hemorrhage in 2 cases, 2 of them showed H. influenza, 2 D. pneumonia, 1 N. meningoeocci, 1 L-hemolytic streptococci. 4) The etiologic factor of subdural effusion was thought: Birth injury in 34 cases (54.1%) infection in 13 cases (20.6%), idiopathic in 14 cases (22.2%). 5) 10of these 62 cases were expired and motality rate was 15.8%

9.Cutaneous Cytomegalovirus Infection Presenting as Perianal Ulcers.

A 68-year-old Korean woman was consulted to the department of dermatology to evaluate ulcerated lesions on the perianal area, which developed about 3 weeks after a cadaveric renal trans-plantation. Histopathologic examination showed large atypical cytomegalic cells in the upper dermis. Polymerase chain reaction(PCR) study revealed positive cytomegalovirus(CMV) deoxyribonucleic acid(DNA) from the skin tissue. We herein present a case of cutaneous CMV infection presenting as perianal ulcers.

10.Treatment Strategy for the Retention Cyst of the Maxillary Sinus

Retention cysts are commonly found in the maxillary sinus and are discovered incidentally on imaging studies. Obstruction of the ducts of the seromucous glands of the paranasal sinus lining is thought to induce the development of retention cysts. Since retention cysts are asymptomatic and tend to degenerate, they do not require definite treatment. However, in clinical practice, surgical treatment is often performed without clear indications. Retention cysts need to be surgically managed when accompanied with relevant clinical symptoms or with obstruction of the natural ostium, or when involving the ostiomeatal complex or the infraorbital nerve. Endoscopic sinus surgery has become the surgical procedure of choice for removing retention cyst. Therefore, it is important for rhinologists to understand the nature and behavior of this pathology so that unnecessary surgical intervention could be avoided.
