2.A Clinical Study of Arthroscopic Meniscectomy

Ninety cases of meniscus injury were treated by arthroscopic meniscetomy between March, 1988 and March, 1992. Eighty four cases were treated by arthroscopic partial, 5 cases by subtotal, and 1 case by total meniscectomy. All cases were followed up for more than 1 year. The results were as follows. 1. The ratio of male to female was 57 to 31, and that of right to left was 52 to 38. 2. There were 41 cases of medial meniscus and 49 cases of lateral meniscus. Longitudinal tears were 35 cases (39%), oblique tears were 20 cases (22%), horizontal tears were 15 cases (17%), and radial tears were 9 cases (10%). There was no difference in the shape of tear between medial and lateral meniscus. 4. The most common associated injury was anterior cruciate ligament tear. 5. The Tapper and Hoover scale was used to record the clinical results, and satisfatory results (exellent and good)were in 86% of cases.

5.Tear Secretion Test by the use of Hamano Fibers.

We measured the tear secretion rate in normal and dry eyes by using a cotton thread method. This new method measures basal tears by means of a yellow colored cotton thread(Hamano fiber) impregnated with phenol red. The 3mm bent end of a 70mm long thread is placed in the inferior conjunctival sac on the temporal side of the eye for 30 seconds. The length of the thread that is wet by the tears changes color soon from yellow to red as a result of the change in pH. We obtained the following results by using of Hamano thread test, as a modified Schirmer test. 1. The proper test time of this test was 30 seconds in view of the analysis of the amount of tear secretion with time. 2. Average wet lengths of normal male was 16.6mm, that of female was 15.7mm from 30 seconds test. 3. Distribution of wet lengths in 300 normal eyes were ranged from 4.0mm to 34.0mm and most of them(61%) were within 10 to 20mm. 4. Wet lengths with and without anesthesia showed no statistical significance(paired t-test, p>0.05). 5. Reproducibility of Hamano fiber test that was tested 3 times in same eye represented significance in statatics(paired t-tast, p<0.05).

8.A Biomechanical Study on the Pull-Out Strength of Pedicle Screw Augmented with Polymethylmethacrylate(PMMA)- Cadaveric Study -.

OBJECTIVE: The authors evaluate the differences in the pedicle screw pull-out strength between an unaugmented screw and a screw augmented by high pressurized PMMA in an embalmed human lumbar cadaveric spine. METHODS: Pedicle screws were inserted in the 50 pedicles of cadaver lumbar spine after measuring the bone mineral density(BMD) of the specimens. The pedicle screw was inserted without augmentation on the left pedicle and with augmentation by a vertebral reconstruction with high pressurized PMMA on the right side. The pedicle screw was inserted before the hardening of bone cement on the right side. A computed tomographic scan was done to evaluate the location of the PMMA and PMMA/ screw interface. After that, a screw pullout strength test was performed. RESULTS: There were positive correlations between the pull-out strength and BMD in the control group (r=0.7339). The average pull-out strength of the augmented group was 168% greater than that of the control group(p<0.05). The pull-out strength of the augmented group was correlated with in the volume of the PMMA. CONCLUSION: A pedicle screw that is augmented with PMMA will provide a more rigid fixation than that of an unaugmented screw, especially in the low BMD group of the specimens.

9.Pityriasis Rubra Pilaris in Down Syndrome.

A 23-year-old woman with Down syndrome presented erythematous keratotic plaques with whitish scales on the elbows, forearm, knee and leg. Histopathological findings revealed acanthosis with broad and short rete ridges, alternating orthokeratosis and parakeratosis oriented in both vertical and horizontal directions, and dermal superficial perivascular lymphocytic infiltration, compatible with pityriasis rubra pilaris.

10.Generalized Erythema Multiforme-like Eruption is the Novel Cutaneous Manifestation of Vibrio Vulnificus Septicemia.

Cutaneous manifestations of Vibrio vulnificus septicemia are bullae, vesicles, necrotic ulcers, localized swelling, cyanosis, and gangrene, which begin to occur mostly on the legs. The characteristic cutaneous lesions associated with pain often give an informative clue for the suspicion of Vibrio vulnificus infection in endemic area. We report a case of Vibrio vulnificus septicemia with atypical cutaneous manifestation of generalized erythema multiforme-like eruption in a Korean man, who died within 9 hours following hospitalization in spite of the intensive therapy. We suggest EM-like eruption of V. vulnificus septicemia is the novel cutaneous manifestation and may indicate poor prognosis.
